This document discusses systems, devices, and methods for computer-assisted pain or paresthesia assessment and pain management in a subject. A system includes a programming aid device including a user interface, a transceiver circuit to receive information about pain management for the patient from one or more of a software-based virtual agent (SVA) or a human assistant other than the patient, and a controller circuit to initiate and manage information exchange session between the patient and one or more of the SVA or the human assistant, via the user interface, regarding pain or paresthesia sensation and pain management. Based on the information exchange, the controller determines a stimulation setting, and generate a therapy control signal to the neuromodulation device to initiate delivery of neuromodulation energy according to the determined stimulation setting.

Computer-assisted pain mapping and neuromodulation system

Systems and techniques are disclosed to generate programming parameters and modifications during closed-loop adjustment of an implantable neurostimulation device treatment programming, through the identification and application of weights determined from user input indications and rankings of therapy objectives. In an example, a system to generate programming values of a neurostimulation device performs operations that: obtains human input which indicates multiple therapy objectives for neurostimulation treatment of a human patient; operates a model (such as an artificial intelligence model) to determine parameter outputs for programming of the neurostimulation device; identifies weights, based on the therapy objectives, usable in the model; produces a composite output from the model, by applying the identified weights to a combination of the parameter outputs of the programming model; and the resulting composite output provides neurostimulation device programming values for neurostimulation treatment designed to address the therapy objectives.

User-weighted closed loop adjustment of neuromodulation treatment

An adapter to add optical stimulation to a stimulation system includes an adapter body and a connector disposed on the distal end of the adapter body. The connector includes a connector body defining a port and a connector lumen; connector contacts disposed in the connector body and arranged along the connector lumen; and a light source disposed in the connector body. The adapter also includes terminals disposed along the proximal end of the adapter body and conductors extending along the adapter body and electrically coupling the connector contacts and the light source to the terminals. The adapter may be used with an optical stylet that fits into a stimulation lead which is, in turn, coupled to the connector of the adapter. Alternatively, the adapter can include a fiber optic coupled to the light source and configured to extend into the stimulation lead.

Optical stimulation system with on-demand monitoring and methods of making and using

Techniques are described for providing a therapeutic pseudo-constant DC current in an implantable stimulator using pulses whose positive and negative phases are not charge balanced. Such charge imbalanced pulses act to charge any capacitance in the current path between selected electrode nodes, such as the DC-blocking capacitors and/or any inherent capacitance such as those present at the electrode/tissue interface. These charged capacitances act during quiet periods between the pulses to induce a pseudo-constant DC current. Beneficially, these DC currents can be small enough to stay within charge density limits and hence not corrode the electrode or cause tissue damage, and further can be controlled to stay within such limits or for other reasons. Graphical user interface (GUI) aspects for generating the charge imbalanced pulses and for determining and/or controlling the pseudo-constant DC current are also provided.

Use of charge imbalanced pulses in an implantable stimulator to effect a pseudo-constant DC current bias

A system may be used with a medical imaging system and a programming system. The medical imaging system may be configured to display a medical image and the programming system may be configured to implement a program used in programming a neuromodulation device. The system may comprise a mobile device having at least one processor, a camera and a user interface including a display. The mobile device may be configured to acquire a displayed medical image from the medical imaging system, determine based on the acquired medical image location data indicative of the position of at least one of the electrodes relative to at least one of the anatomy or at least another one of the electrodes, and provide the location data for use by the program implemented by the programming system.

Tool to use image for programming neuromodulation

An example of a neurostimulation system may include one or more sensing devices and a patient assistance device. The sensing device(s) may be configured to sense one or more signals from a patient and may include one or more non-invasive sensing devices. The patient assistance device may be configured to assist the patient in use of a stimulation device and may include a communication circuit configured to receive the sensed signal(s), a user interface configured to allow for interactions with the patient, and a processing circuit which may be configured to receive patient-specific information including the sensed signal(s), to analyze the patient-specific information with neurostimulation algorithm information representative of available therapeutic options, to produce one or more recommendations related to use of the stimulation device for treating the patient based on the analysis, and to present at least one recommendation using the user interface.

Method and apparatus for neurostimulation device therapy management

A holder for an implantable medical device (IMD) is disclosed. The holder is configured to hold the IMD in a single, operational orientation when the holder containing the IMD is implanted in a patient. The holder is designed to prevent the IMD from moving within the patient and shifting orientation. The holder may have an opening for receiving a connecting pin, for stimulating electrodes, for example, in a mating orientation with respect to a receptacle contained within the IMD. According to some embodiments, the IMD features a metal housing that can serve as an electrode and the holder is configured with a window to allow a portion of the housing to electrically contact flesh of a patient during operation. Holders for containing a single IMD and holders for containing multiple IMDs are disclosed. Holders for containing multiple IMDs are configured to maintain alignment of the IMDs with respect to each other, for example, a parallel alignment.

Methods, medical devices, and magnetic resonance imaging (MRI) systems are provided. A patient implanted with a medical device is exposed to a time-varying magnetic field having a signature, thereby inducing mechanical vibrations in at least one component of the medical device. A vibrational characteristic of the mechanical vibrations induced in the component(s) is detected. The vibrational characteristic is analyzed, and the signature of the magnetic field is identified based on the analyzed vibrational characteristic. The medical device is automatically switched from a first operational mode to a second operational mode when the signature is identified.

Systems and method for automatically detecting an MRI environment for patient implanted with medical device

An active implantable medical device (AIMD) for use with a medical lead carrying at least one lead electrode. The AIMD comprises interior electronic circuitry configured for performing a medical function via the medical lead, an electrically conductive case containing the interior electronic circuitry, at least one electrical terminal configured for electrically coupling the electronic circuitry respectively to the lead electrode(s), and an inductive element coupled in series between the electrical terminal(s) and the case. The inductive element is configured for hindering the shunting of electrical current from the at least one electrical terminal to the case that has been induced by electromagnetic interference (EMI) impinging on the medical lead.

Implantable medical device having electromagnetic interference filter device to reduce pocket tissue heating

A lead assembly includes an implantable lead. Electrodes are disposed along a distal end of the lead in an electrode array. Terminals are disposed along a proximal end of the lead in a proximal-most terminal array and a medial terminal array. A terminal extension electrically couples to the medial terminal array. A port is defined in a connector at a first end of the terminal extension. The port has a first end and an opposing second end and forms a continuous passageway therebetween. The port receives the medial terminal array. A contact array includes connector contacts that are disposed within the port and that couple electrically with a terminal array disposed along a second end of the terminal extension. The contact array couples electrically with terminals of the medial terminal array of the lead when the medial terminal array is received by the port.
