An implantable medical lead comprising a lead body defining a lumen. The lead body includes one or more tines substantially at a distal end of the lead body. An inner member extending within the lead body lumen is configured to rotate relative to the lead body and configured to cause a rotation of a dilator. The dilator is configured such that the rotation causes or enables a lateral translation of the dilator from a first position proximal to a lead body opening to a second position distal to the lead body opening. The implantable medical lead may include a probe wire configured to slidably translate through an inner lumen of the dilator.

A method for monitoring a cardiovascular pressure in a patient includes measuring, by pressure sensing circuitry of an implantable pressure sensing device, the cardiovascular pressure of the patient. The method further includes transmitting, via wireless communication circuitry of the implantable pressure sensing device, the measured cardiovascular pressure to another device. The method further includes determining, by processing circuitry of the other device, whether a posture of the patient at a time of the measured cardiovascular pressure was a target posture for cardiovascular pressure measurements. The method further includes determining, by the processing circuitry of the other device, whether to store or discard the transmitted cardiovascular pressure based on determining whether the posture was the target posture.

Measuring cardiovascular pressure based on patient state

A system and method of implanting pacing lead in a patient's heart. The system may include a catheter configured to by inserted through the coronary sinus ostium such that the distal end region of the catheter is positioned past the anterolateral vein and proximate at least one septal perforating vein. The catheter is configured to inject contrast proximate the septal perforating vein to identify an implant region for a pacing lead. Further, a controller is configured to deliver pacing therapy to the implant region.

A method for monitoring a cardiovascular pressure in a patient may include storing, in a memory of an implantable medical device system and in association with each one or more different patient postures, a respective offset value for the cardiovascular pressure of the patient. The one or more offset values may be determined based on a distance between an implantable pressure sensing device and an anatomical structure of the patient, a location of the implantable pressure sensing device within the patient, or one or more dimensions an anatomical structure of the patient. The method further includes determining a measured value of the cardiovascular pressure and a posture of the patient when the value of the cardiovascular pressure was measured, selecting a stored offset value associated with the current patient posture, and determining an adjusted cardiovascular pressure value based on the selected offset value and the measured cardiovascular pressure value.

A method for reducing small volume subcutaneous leaks of therapeutic fluids during procedures to refill an implantable medical device includes reducing pressure in a reservoir of the device. A refill needle can be percutaneously inserted into a fill port in communication with the reservoir and therapeutic fluid can be delivered through the needle into the reservoir. Reduced reservoir pressure upon withdrawal of the refill needle from the port can result in reduced subcutaneous leakage of the therapeutic fluid.

Systems, devices disclosed provide example methods comprising determining that a triggering event has occurred based on statuses for a set of physiological parameters associated with the patient, the physiological parameters indicative of the patient engaging in a patient initiated physical activity, generating a trigger output signal in response to the determination that the triggering event has occurred, wirelessly transmitting the trigger output signal to a pressure sensing device implanted in a vessel of the patient, triggering, based on receiving the trigger output signal, the pressure sensing device to sense a cardiovascular pressure of the patient; and transmitting, by the pressure sensing device, a wireless signal comprising data corresponding to the sensed cardiovascular pressure of the patient.

Devices, systems, and techniques for transitioning between different drug delivery sources or different drug concentrations may account for diffusion and mixing of drug within the fluid. In one example, a method may include determining a flow rate for a fluid to be delivered to a patient via a drug pump and a catheter in fluid communication with a reservoir of the drug pump. The fluid includes a drug. The method also includes determining a concentration profile of the drug delivered via the catheter, wherein the concentration profile identifies a volume of delivered fluid needed to achieve a target transition dose of the drug. The method further includes determining, by a processor and based on the flow rate and the concentration profile, an initial delivery period required to achieve the target transition dose by delivering the fluid at the flow rate.

Controlling drug delivery transitions

Methods and systems for transvenously accessing the pericardial space via the vascular system and atrial wall, particularly through the superior vena cava and right atrial wall, to deliver a pharmacologic agent, particularly a NO-donor drug, to the heart are disclosed. A proximal connector of an infusion catheter is coupled to an infusion pump, and a distal catheter segment having a distal infusion catheter lumen end opening is disposed in the pericardial space. The implantable infusion pump is operable in conjunction with an implantable ischemia monitor to monitor the ischemic state and trigger delivery or regulate the periodic delivery of the pharmacologic agent to optimally treat ischemia. The patient may operate a patient activator that the patient when feeling ischemia symptoms to transmit a signal that is received by the implantable infusion pump and triggers delivery of a bolus and/or continuous infusion.

Methods and systems for providing therapies into the pericardial space

A method for adjusting delivery of a therapeutic fluid to a patient suffering from or at risk of pulmonary arterial hypertension includes introducing the therapeutic fluid to a patient via a catheter at a predetermined rate. The catheter is positioned to deliver the fluid to the right ventricle or the pulmonary artery. The catheter a one-way valve configured to allow the fluid to flow from the catheter to the target location. The method further includes monitoring pressure of the target location by monitoring internal catheter pressure, and adjusting the rate at which the therapeutic fluid is introduced to the catheter based on the monitored pressure. The rate at which the fluid is introduced to the catheter is increased if internal catheter pressure increases, and the rate at which the fluid is introduced to the catheter is decreased if the internal catheter pressure decreases.

Pressure monitoring to control delivery of therapeutic agent

A method of delivering therapy to a target site. The method includes (a) obtaining a base image of the target site, (b) injecting a dose of a mix of a therapeutic agent and a contrast agent into a first injection location at the target site via a needle passing through a catheter, (c) collecting sequential fluoroscopic images of a contrast agent dispersion cloud at the first injection location, (d) determining a contrast agent dispersion area from the sequential fluoroscopic images, (e) determining a therapeutic agent dispersion area from the contrast agent dispersion area, (f) marking the therapeutic agent dispersion area on the base image of the target site, and (g) repeating (b) through (f) until the target site is substantially covered with overlapping therapeutic agent dispersion areas corresponding to a plurality of injections at a plurality of injection locations at the target site.
