The system determines an 'imaginary' component of the high frequency transmission line impedance having certain spectral changes that correspond to radially outward movements or local externalization of a conductor within a lead body allowing for the detection of conductor migration and small insulation failures.

Apparatus and methods are disclosed that facilitate dynamic adjustment of MVT parameter values in response to new and changing circumstances such as the patient's condition before, during, and after administration of MVT. Administration of MVT is automatically and dynamically adjusted to achieve specific treatment or life-support objectives, such as prolongation of the body's ability to endure and respond to MVT, specifically addressing the type of arrhythmia or other pathologic state of the patient with targeted treatment, a tiered-intensity MVT treatment strategy, and supporting patients in non life-critical conditions where the heart may nevertheless benefit from a certain level of assistance.

Adaptive medium voltage therapy for cardiac arrhythmias

A method and system for treating an individual experiencing cardiac arrest using an automatic external defibrillator (AED) includes placing a first and a second electrode of the AED in electrical communication with an exterior surface of the individual. A need to apply a high voltage defibrillation signal to the individual is automatically determined. The method also includes automatically causing the AED to apply a medium voltage therapy (MVT) signal through the first and the second electrodes to the individual. The MVT signal is applied to induce a hemodynamic effect in the individual. Alternatively, or additionally, the MVT signal is applied to induce a respiratory effect in the individual. Optionally, the MVT signal is applied before determining the need to apply the defibrillation signal.

Medium voltage therapy applications in treating cardiac arrest

Methods and apparatus for accurately and painlessly measuring the impedance between defibrillation electrodes implanted in a patient utilize a high current test pulse delivered with a sufficiently high current to produce an accurate measurement of the defibrillation electrode impedance while limiting the duration of the test pulse such that the pain sensing cells in the patient do not perceive the test pulse. In one embodiment, the test pulse is generated from the high voltage transformer without storing energy in the high voltage capacitors and is delivered to the defibrillation electrodes in the patient utilizing the high voltage switching circuitry.

Apparatus and methods for measuring defibrillation lead impedance via a high magnitude, short duration current pulse

An implanted cardioverter defibrillator (ICD) delivers an electrical therapy signal to the heart of a patient. When ventricular fibrillation or another condition of the heart requiring high voltage therapy is sensed, the therapy signal is delivered to the heart. When a partial short-circuit or other low impedance condition occurs, an over-current protection circuit will stop delivery of a shocking pulse. The ICD will then reduce the voltage of the shocking pulse and try again to deliver electrical therapy. This process is repeated until a voltage level is found that is able to deliver the electrical therapy without causing an over-voltage condition. Alternate lead configurations may also be tried in an attempt to find a signal path that is not affected by the low impedance or short-circuit condition.

An implantable cardiac stimulation device provides stimulation therapy from within the left ventricle of a heart. The device includes a pulse generator adapted to be coupled to an implantable cardiac stimulation electrode and a power supply that provides the stimulation electrode with a positive voltage. The positive voltage promotes coating of the electrode through a body coating process. The coating serves to repel formation of clots on the electrode.

Also an implantable cardiac stimulation device with an implantable housing, a stimulation pulse generator positioned within the housing, at least two implantable leads, and a controller communicating with the pulse generator to induce the generator to deliver therapeutic stimulation to a patient's heart. The leads are arranged to measure a physiological parameter along at least two different spatial orientations. The controller evaluates relative timing of the physiological parameters along the different orientations for indications of contractile synchrony of the patient's heart.

Automatic system for determining bi-ventricular pacing responders

An implantable cardiac device to induce fibrillation in the heart of a patient to allow testing of the defibrillation capability of the device. The device induces fibrillation using a direct current across the heart. The shock to the heart may be applied in a method to minimize discomfort to the patient. The heart is monitored during application of the shock. The voltage of shock at the heart is gradually increased until fibrillation is induced. Once the fibrillation is detected the shock may be stopped. This results in a minimized voltage level and duration for the shock to the heart, thereby diminishing pain and discomfort to the patient.
