A device for neurostimulation includes a pulse generator for generating current having pulses and at least one first pair of electrodes connected to the pulse generator. The device provides a user-programmable therapy strength parameter configuration and at least two current parameter configurations for neurostimulation stored in the pulse generator. The current parameter configurations are controlled by the therapy strength configuration, at least one of the current parameter configurations is associated with a level of paresthesia sensation of a patient and at least one of the current parameter configurations is associated with a paresthesia-free therapy for the patient. The association between therapy strength parameter and current parameter configurations uniquely adjusts the current parameter configurations based on paresthesia or paresthesia-free intent, when neurostimulation is performed using parameter configurations. Particularly, the current parameter configurations include parameters such as pulse trains, pulse frequencies, duty cycling, and amplitudes of the current.

Transvascular diaphragm pacing systems (TDPS) and methods are disclosed for providing respiratory therapy to a patient. The TDPS can provide rapid insertion and deployment of endovascular pacing electrodes in critically ill patients who require intubation and invasive PPMV in order to support the physiological requirements of the human ventilatory system. The systems and methods make best use of the contractile properties of the diaphragm muscle and prevent muscle disuse and muscle atrophy. This can be carried out by engaging the phrenic nerves using patterned functional electrical stimulation applied to endovascular electrodes that are temporarily and reversibly inserted in central veins of the patient, such as the left subclavian vein and the superior vena cava. The TDPS can be designed to seamlessly interface with any commercially available positive-pressure ventilatory assistance/support equipment such as is commonly in use in hospital intensive care units (ICU) for treating critically ill patients with breathing insufficiencies, pain, trauma, sepsis or neurological diseases or deficits.

A pacing system, which is particularly suitable for implantable leadless pacemakers, applies passively-balanced voltage-based pacing pulses, and periodically performs capture verification (evoked response detection) by following a pacing pulse with a current-based active balancing pulse, and then measuring any evoked response provoked by the pacing pulse. The active balancing pulse reduces residual charge on the electrodes used for pulsing, and thereby reduces polarization artifacts that could obscure measurement of the evoked response at the electrodes. The amplitude and pulse width of the active balancing current pulse are defined by measurements made in a few preceding pulses. The pacemaker preferably detects indicia of cardiac contractility, and performs capture verification only when contractility indicates that the patient is physically inactive and emotionally stable.

Electrical stimulation of a target (e.g., nervous tissue) is performed, wherein balance phases are automatically determined, and at least one of the electrodes is indirectly monitored during therapy delivery. The stimulation system is further configured to generate correction currents when a voltage accumulated at associated double layer capacitances crosses pre-defined thresholds so as to reduce or cancel the accumulated voltages without therapy interruption. A finer automatic determination of balance phases permits minimizing the stimulus artifact for evoked response sensing. Closed-loop neurostimulation may be performed based on such evoked responses.
