A prosthesis suited for orthopedic implantation possesses a multi-piece stem component that supports an artificial joint surface that can articulate with another artificial joint surface in various ways. The prosthesis can be assembled in a snap fit and/or interlocking fashion that provides positive locking means without the use of screws or other fasteners. The prosthesis can accommodate fitment of a plastic joint surface made, e.g., from ultra high molecular weight polyethylene. The prosthesis is well suited for use in an ankle replacement system that can be installed using minimally invasive intramedullary guidance established with respect to the major axis of the tibia by minimally invasive access through the calcaneus, through an incision in the bottom of the foot. The prosthesis makes possible the installation of a total ankle system using minimally invasive anterior access to the ankle joint for making bony cuts and to install prosthesis components.

Ankle replacement system

Orthopedic implants capable of compressing adjacent bone segments into which they are inserted, effective for the fusion, management and repair of bone joints and bone fractures, and methods for their installation, are described. One embodiment of such implants includes an elongated shaft coated to encourage bony in-growth, having a non-round cross section, and a flange at one end, which is laterally inserted into a preformed insertion path formed in adjacent bone segments traversing a joint or fracture location, to the point where further insertion is blocked by the flange intersecting one of the bone segments. The other end of the shaft may have a portion of male thread protruding out of the insertion path in the second of the bone fragments and adapted to be mated to a fastener having female thread. When the fastener is tightened against the outside of the second bone segment, perhaps using a washer, the joint or fracture is compressed and stabilized resulting in an effective fusion of the joint or fracture when the bone secures itself in the insertion path to the shaft's bony in-growth surfaces on both sides of the joint or fracture line.

Orthopedic implant having non-circular cross section and method of use thereof

The present invention provides methods and apparatus in which a surgical retractor comprises a plurality of mechanically coupled tissue retaining walls that are guided into position along one or more guides previously implanted into the patient. The walls are preferably coupled by pivots, so that separating some of the walls from one another opens an operating space. There are preferably two guides, which are driven or screwed into the pedicles of vertebrae, or other bone. Oversized channels are best disposed in a frame, which also serves to hold lock the walls apart. Various convenience features are contemplated including a web disposed between the walls, which expands as the walls are separated. Also contemplated are projections from near the bottoms of one or more of the walls, which can alternatively or additionally help to hold the underlying tissue in place.

Guided retractor and methods of use

The present invention provides methods and apparatus in which a surgical retractor comprises a plurality of mechanically coupled tissue retaining walls that are guided into position along one or more guides previously implanted into the patient. The walls are preferably coupled by pivots, so that separating some of the walls from one another opens an operating space. There are preferably two guides, which are driven or screwed into the pedicles of vertebrae, or other bone. Since practical considerations will usually mean that the guides are not generally parallel to one another, the guides are capable of polyaxial movement with respect to the pedicles, and retractor is provided with oversized channels to receive the guides. The channels may be conveniently disposed in a frame, which also serves to hold lock the walls apart. Various convenience features are contemplated including a web disposed between the walls, which expands as the walls are separated. Also contemplated are projections from near the bottoms of one or more of the walls, which can alternatively or additionally help to hold the underlying tissue in place.
