Wearable, automatic external, and implantable defibrillators, as well as methods of operation in such systems, are disclosed with shock delivery mitigations to avoid delivering a defibrillation shock on a T-wave. Prior to issuance of a defibrillation shock, one or more detected cardiac events are analyzed to characterize a detected event that is sensed for purposes of synchronizing the defibrillation shock. The detected event can be characterized as an R-wave or a T-wave, and the shock delivery protocol is then selected based on the characterization of the detected event to avoid shock-on-T and potential pro-arrhythmia.

Cardiac beat classification to avoid delivering shock during ventricular repolarization

An implantable medical device (IMD) system communicates to a user information regarding the surrounding biological environment and the influence of that surrounding biological environment on IMD functionality. This influence can be static or dynamic. Providing information on the biological environment can assist the user in determining therapy parameters for an individual patient with an IMD.

Visualization of biological environment impact on implanted therapy

A method and system for discriminating ventricular arrhythmia is disclosed. In an embodiment, the method can include implementing an arrhythmia discrimination algorithm that can discriminate between supraventricular tachycardia (SVT) and ventricular tachycardia (VT) using at least one programmable parameter programmed to a first value. The method can include analyzing an SVT event, where analyzing the SVT event can include sensing a physiological signal during the SVT event and identifying characteristics of the sensed physiological signal. The method can further include analyzing a cardiac signal to classify the cardiac signal as either an SVT or a VT using the arrhythmia discrimination algorithm with the programmable parameter (programmed to a second value. The second value can be determined from the identified characteristics of the sensed physiological signal.

Method and system for arrhythmia discrimination

An apparatus comprises a primary cardiac signal sensing circuit configured to sense at least a first cardiac signal, a secondary cardiac signal sensing circuit configured to sense a secondary cardiac signal, and a control circuit. The control circuit includes a noise detection circuit that has an alignment circuit. The alignment circuit is configured to align a segment of the sensed first cardiac signal with a segment of the sensed secondary cardiac signal. The noise detection circuit configured to determine a number of turns in the first cardiac signal segment, determine a number of turns in the secondary cardiac signal segment, generate an indication of noise in the first and secondary cardiac signals according to the determined number of turns, and provide the indication of noise to a user or process.

Noise detection in implantable medical devices

An implanted cardiac device detects an atrial arrhythmia and provides periodically updated atrial arrhythmia status as long as the arrhythmia is ongoing. A patient may request an indication of ongoing atrial arrhythmia status from external to the patient using a patient activator. The patient activator may include a magnet for closing a reed switch in the implanted device to provide the request or may provide the request over a telemetry link to the implanted device. The implanted device may provide the requested atrial arrhythmia status and other information in the form of an audible tone produced by the implanted device or as a message telemetered from the implanted device to the patient activator. The patient activator may include a tone detector and display for providing a visual indication of the atrial arrhythmia status indication. The magnet activator may also be employed to request or withhold atrial shock therapy.

Patient controlled atrial shock therapy

A system comprises an implantable medical device (IMD). The IMD includes at least one electrical input to receive sensed electrical activity of a heart and a sampler circuit coupled to the at least one electrical input. The sampler circuit generates sampled values of the sensed electrical activity. The IMD includes a clock circuit that generates readable values representative of absolute time. The IMD also includes a controller circuit coupled to the sampler circuit and the clock circuit. The controller circuit processes the sampled values and generate at least one marker to indicate a detected event related to the electrical activity. The controller circuit also stores a timestamp of absolute time of the detected event with the at least one marker in memory.

Synchronizing continuous signals and discrete events for an implantable medical device

Systems and methods are provided for analyzing occurrences of atrial arrhythmias. Occurrences of each of a number of classified atrial arrhythmia rhythms are detected. The classified atrial arrhythmias may, for example, include at least atrial fibrillation and atrial flutter. A duration of time associated with each of the detected atrial arrhythmia rhythms is measured. Trend data is produced with respect to a predetermined period of time using all or selected ones of the measured time durations. The detecting, measuring, and producing processes may also be performed for one or more unclassified atrial arrhythmias.
