Disclosed herein are various exemplary systems, devices, and methods for inhibiting premature implant deployment. The delivery member can include a body including a lumen extending therethrough, the body including a compressed distal portion. The delivery member can include a pull wire extending through the lumen. The pull wire can include a pull wire portion that extends radially to abut a sidewall of the body to provide frictional resistance against the body. The pull wire can be positioned to secure the implantable medical device to the delivery member, and the pull wire portion can be effective to inhibit premature detachment of the implant by inhibiting proximal translation of the pull wire due to the frictional resistance provided by the pull wire portion against the body.

Systems and methods for inhibiting premature embolic implant deployment

An example aneurysm implant can include a tubular braid that can have an open end, a pinched end, and a predetermined shape. In the predetermined shape, the tubular braid can have three segments and two inversions, one of the three segments extending between the two inversions and forming a sack. Two of the segments can form a substantially semispherical shape. The tubular braid can be implanted in an implanted shape based on the predetermined shape. The substantially semispherical shape of the tubular braid can aid in decreasing a pulsatile blood inflow rate and a pulsatile blood outflow rate for a substantially spherical aneurysm cavity.

Stents generally can include a tubular structure having circumferentially positioned undulating wires that extend over a majority of a length of the stent such that the undulations oscillate circumferentially to define a circumference of the stent. The undulations can wrap over and under adjacent undulations to form an interwoven structure. Additionally, or alternatively, adjacent wires can be joined. Wires forming the stent can be cut from elastic tubing such that each wire has a three-dimensional shape.

A method for constructing a braided implant delivery system and treating an aneurysm can include attaching a braided implant having a band attached thereto to a delivery tube, positioning the braided implant within the aneurysm, and releasing the band from the delivery tube, thereby releasing the braided implant. The band can include movable extensions that can press into an outer surface of the delivery tube to secure the band to the delivery tube then move away from the outer surface of the delivery tube to release the band. A pull wire can be engaged to the band to secure the band to the delivery tube then be pulled proximally to release the band from the delivery tube. At least a portion of the braid of the braided implant can be positioned within a lumen of the delivery tube.

Delivery of embolic braid

An implant can be implanted in an aneurysm to cause thrombogenesis within the aneurysm's sac. The implant can include a braid that can extend across the aneurysm neck and anchor to aneurysm's walls at least in the proximal portion of the aneurysm sac. The implant can further include a dome feature configured to press into aneurysm walls near the aneurysm's dome and facilitate securement of the braid across the aneurysm's neck. The braid at the aneurysm's neck and the dome feature can be joined or constricted at a junction, for instance with a radiopaque band. The dome feature can include a braid, embolic coil, and/or wire frame. The braid can be inverted based on a heat-set predetermined shape, where several predetermined shapes are suitable.

A self-expanding element delivery apparatus includes a catheter having an inner lumen, a self-expanding element, and a delivery wire disposed within and extending through the inner lumen and the self-expanding element. The self-expanding element has a proximal, distal, and an intermediate portion. The element has anchor members, a compressed configuration fitting within the inner lumen, and an expanded configuration dimensioned larger than the catheter. The delivery wire also has proximal and distal portions, an intermediate portion, a dual function bump member disposed approximate to the intermediate portion, and a pusher bump member disposed approximate to the proximal portion of the delivery wire. A bump member distance between the dual function bump member and the pusher bump member is such that so that only one of the dual function bump member and the pusher bump member can contact one of the anchor members.

A detachment system for delivering an embolic coil implant to a treatment site is provided. A pull wire through a lumen of the detachment system that engages a loop wire can include a slack section. The slack section can be one or more bends, a spiral coil, or a stretchable material. The slack section is effective to inhibit premature detachment of the implantable medical device by inhibiting movement of the distal end of the pull wire when the detachment system is traversing a microcatheter.

A tubular, folding flow diverter is provided. The flow diverter includes a plurality of sections, including a first section, a second section, and a third section. The flow diverter is shapeable to an elongated cylindrical shape and is movable to an implanted configuration. In the implanted configuration, the second section and at least a portion of the first and third sections overlap to form a three-layer shape. This three-layer shape can be positioned proximate an aneurysm neck when implanted in a patient. The overall porosity of the implant can be higher than legacy implants, because the overlapping portion of the three sections can decrease the porosity proximate the aneurysm neck.

An aneurysm intrasaccular implant is provided having one or more coated regions. The implant includes a braided mesh movable from a delivery configuration having a single-layer tubular shape sized to traverse a catheter to an implanted configuration sized to be implanted in an aneurysm sac. The braided mesh can include an anti-thrombogenic coating defining a first section along the length of the braided mesh. The braided mesh can include a cell adhesion coating defining a second section along the length of the braided mesh. In the delivery confirmation, the first section can be positioned distal to the second section within the catheter. The second section is invertible to the implanted configuration such that the second section is positioned within the aneurysm sac and the first section is positioned to occlude an aneurysm neck.

A multi-basket clot capturing device that includes a distal basket connected to a wire. The distal basket may be positionable distal of the clot and operable to capture a distal portion of a clot. A proximal basket may be connected to a hypotube that is slidably axially connected to the wire. The proximal basket may be positionable proximal of the clot and operable to capture a proximal portion of the clot. A cage can form between the proximal and distal baskets around multiple portions of the clot for capturing the clot. Sliding the wire distally, or microcatheter proximally, relative to the clot causes the distal basket to move from a collapsed state to an expanded state. The cage may be around at least two portions of the clot that are opposed.

Disclosed herein are various exemplary systems and methods for deploying an implant to a target location of a body vessel. The delivery member can include a tubular body including a lumen and compressed distal portion. The delivery member can include a loop wire with a loop opening positioned approximate the compressed distal portion. The delivery member can include a pull wire that has a proximal pull wire portion and a distal pull wire portion connected by a suture linkage. The suture linkage can include a proximal suture knot engaged to the proximal pull wire portion and a distal suture knot engaged to the distal pull wire portion. Pull wire beads positioned on the proximal pull wire and distal pull wire portion can retain the suture knots during proximal translation of the pull wire. The suture linkage can include slack that is effective to prevent premature deployment of the implant.

A braid for treating an aneurysm can include a first radially expandable segment operable to move from a collapsed state within a microcatheter to a deployed state distal of the microcatheter. The first radially expandable segment can be capable of radially expanding to form an outer occlusive sack in the aneurysm in the deployed state. The braid can also include a second radially expandable segment operable to move from the collapsed state within the microcatheter to the deployed state distal of the microcatheter, wherein the second radially expandable segment is capable of radially expanding inside the outer occlusive sack to form an inner occlusive sack in the outer occlusive sack in the deployed state. An expansion mechanism can be included and be disposed at a proximal end of the first and second radially expandable segments.

The present invention provides a braided implant with a retractable dual proximal layer and methods for administering the braided implant to treat aneurysms. The implant can include a tubular braid that can be set into a predetermined shape, compressed for delivery through a microcatheter, and implanted in at least one implanted position that is based on the predetermined shape and the geometry of the aneurysm in which the braid is implanted. The implant can also have a retractable ball segment at the proximal end of the device made of the same braid that can be heat treated into an ellipsoid shape. The retractable ball segment can be movable from a position outside the aneurysm to a position at least partially enclosed within the implant to increase or decrease the height of the implant relative to the aneurysm or better conform the implant to the neck of the aneurysm.

A method of occluding an aneurysm includes positioning a braided implant within an aneurysm sack such that an outer non-inverted layer contacts a wall of the aneurysm and an inverted layer apposes the outer non-inverted layer to form a double layer of braid across a neck of the aneurysm. A radiopaque end of the braided implant can be positioned centrally within the aneurysm so that the radiopaque end extends adjacent or across a plane defined by a fold between the outer non-inverted layer and the inner inverted layer. Embolic coils can be inserted into the aneurysm sac and inhibited from exiting the aneurysm at the neck by the double layer braid across the aneurysm neck.
