Systems, devices, and methods for determining advancement of a surgical laser fiber in an endoscope and providing interlocking feedback for the surgical laser are disclosed. An exemplary method includes directing light from a distal end of an endoscope to a target, optically detecting an amount of the light reflected from the target, transmitting the optically detected amount of light through a laser fiber extending through a working channel of the endoscope, determining, based on the optically detected amount of light, a position of a distal end of the laser fiber relative to the distal end of the endoscope, and generating a control signal to the surgical laser system to adjust laser emission through the laser fiber.

Disclosed herein are systems and methods for monitoring a condition associated with a medium proximate a target region. The system comprising a surgical laser, a surgical fiber coupled to the surgical laser for delivering laser light to a target region and receiving at least a portion of the laser light reflected from the target region. The system further comprising a processor configured to analyze the collected reflected laser light and determine, based at least in part on the analysis of the reflected laser light a value of a parameter associated with the medium proximate the target region.

Monitoring liquid medium conditions proximate a laser lithotripsy treatment area in real time

A laser can be controlled based on different tissue compositions, such as in real time. After a first time period, a first composition of a in vivo target site can be identified. Based on the first composition, a plurality of lasers can be controlled to emit light at a first wavelength where controlling includes activating a first combination of the plurality of lasers. After a second time period, a second composition of the in vivo target site different from the first composition can be identified. Based on the second composition, a plurality of lasers can be controlled to emit light at a second wavelength, such as can include activating a second combination of the plurality of lasers. The first combination of the plurality of lasers can be different from the second combination of the plurality of lasers.

Laser combination with in vivo target feedback analysis

Systems, devices, and methods for delivering laser energy directed toward target tissue using a spectroscopic feedback. An exemplary laser feedback control system comprises a feedback analyzer to receive a signal from a target tissue using a spectroscopic sensor, and a laser controller to determine whether the received signal generally equals a first preset. If the received signal meets the first preset, the laser controller can send a control signal to a laser system to change from a first state to a second state of the first laser system. The laser system can deliver laser energy via an optical fiber towards the target tissue.

Systems, devices, and methods for determining a distance between a distal end of an endoscope and a target during an endoscopic procedure are disclosed. A surgical laser feedback control system comprises a feedback analyzer and a controller. The feedback analyzer can receive at least two reflected signals from a target in response to electromagnetic radiation directed at the target. The at least two reflected signals correspond to respective different distances between a distal end of a device of a surgical laser system and the target. The feedback analyzer can determine a distance between the distal end of the device of the surgical laser system and the target based on the at least two reflected signals. The controller can generate a control signal to the surgical laser system to perform a predetermined operation based on the determined distance.

Laser fiber-to-target distance control

Systems, devices, and methods for endoscopic mapping of a target and tracking of endoscope locations inside a subject's body during a procedure are disclosed. An exemplary system comprises an imaging system configured to capture an endoscopic image of the target that includes a footprint of an aiming beam directed at the target, and a video processor configured to identifying one or more landmarks from the captured endoscopic image and determine their respective locations relative to the aiming beam footprint, and generate a target map by integrating a plurality of endoscopic images based on landmarks identified from one or more of the plurality of endoscopic images. The target map can be used to track endoscope location during an endoscopic procedure.

Systems and methods for endoscopic image reconstruction

Systems and methods of in situ unclogging a working channel in a medical device during a procedure are disclosed. An exemplary unclogging system comprises a flow sensor to sense a flow rate through a working channel, and a control module to detect a channel state indicating a presence or absence of clogging based on the flow rate. In the presence of channel clogging, the control module can control one or more of an irrigation source or a suction source to provide respectively irrigation fluid or suction pressure to unclog the obstructed channel. The control module can adjust one or more of an irrigation flow rate or a suction flow rate through the working channel to maintain a desired pressure of the anatomical environment at the anatomical site, or to maintain a desired flow condition in the working channel, during the procedure.

Endoscope unclogging system and method

Techniques for detecting depth contours of an anatomical target and for enhancing imaging of the anatomical target are provided. In an example, a reference pattern of light can be projected across an anatomical target and an image of the reflected light pattern upon the anatomical target can be captured. The captured light pattern can be analyzed to determine contour information, which can then be used to provide 3D cues to enhance a 2-dimensional image of the anatomical target.

Depth and contour detection for anatomical targets

Systems and methods of in situ pressure control at an anatomical environment during a procedure are disclosed. An exemplary irrigation and suction system comprises a user input configured to receive from a user a desired pressure to be applied to the anatomical environment, or a desired flow condition corresponding to the desired pressure. The system comprises a pressure sensor to sense a pressure at or near the anatomical environment, and a control module to adjust one or more of an irrigation flow rate or a suction flow rate of at least one working channel of a medical device to maintain the pressure of the anatomical environment at substantially a level of the desired pressure, or to maintain the desired flow condition in the working channel during the procedure.

Systems, devices, and methods for automatic control of laser treatment of target structure in a body of a subject based on acoustic feedback in response to delivery of laser energy to the target are disclosed. An exemplary laser energy delivery system comprises a laser system to direct laser energy at a body target, and a controller circuit to receive an acoustic signal in response to delivery of laser energy to the target, and to measure acoustic properties from the acoustic signal. The control circuit may generate control signals for controlling the laser system to adjust laser energy output, or for controlling an actuator to adjust a position of a laser fiber distal end relative to the target to achieve a desired therapeutic effect.

A composition of a biological sample, such as tissue of a patient, can be estimated during an ablation procedure. A composition detector system can include a probe, an illumination source, and a spectrometer. A distal end of a probe can convey mechanical, acoustical, or ultrasonic energy to the tissue to ablate the tissue. The probe can extend through a working channel of a viewing scope. An illumination source can illuminate a portion of the tissue at the distal end of the probe or as the portion is being evacuated and collected for disposal. The illumination can generate response illumination from the portion of the tissue. The spectrometer can receive the response illumination, analyze the response illumination, and provide an estimate of the composition of the portion of tissue.

Real-time composition analysis techniques for ultrasonic ablation

A lithotripter is provided that includes a lithotripsy apparatus for treatment of a urinary tract stone by fragmentation. The lithotripsy apparatus includes a lithotripsy wave guide shaft configured to transmit an energy form to at least one urinary tract stone. The lithotripter includes a sensing device configured to provide signal data for determining optimal application of energy during treatment with the lithotripsy apparatus. The lithotripter includes a processor configured to collect the signal data and provide feedback to a user. The processor has a control logic configured to determine at least one of: a) if the lithotripsy wave guide shaft is in contact with a tissue; b) if the lithotripsy wave guide shaft is in contact with a stone; c) type of stone; d) if a user is applying force in excess of a predetermined threshold; and e) physical characteristics of a stone. A method is also provided.

Feedback dependent lithotripsy energy delivery

Systems, devices, and methods for identifying a target in vivo are disclosed. A target identification system for use in electrosurgery includes a probe, an optical splitter, and a spectroscopy system. The probe includes an optical pathway to pass a first optical signal to an anatomical target and at least a portion of a second optical signal from the anatomical target. The optical splitter includes a first port to direct the first optical signal to the optical pathway and to receive the at least a portion of the second optical signal from the optical pathway, a second port to receive the first optical signal, and a parabolic reflector to redirect the portion of the second optical signal. The spectroscopy system can identify a characteristic of the anatomical target based on the redirected at least a portion of the second optical signal.

A lithotripsy or other medical laser treatment system can include a lateral actuator to laterally displace a distal portion of a laser fiber, such as can be scanned or otherwise controlled to generate a spatial or spatiotemporal sub-targeting pattern without requiring laterally moving an endoscope carrying the laser fiber in a longitudinal passage such as a working channel. A targeted stone can be selectively weakened along the pattern, such as using lower energy pulses, before being fragmented, such as by a higher energy shock pulse.
