A surgical stapling instrument is configured to dissect tissue, apply a linear staple line along one side of the tissue dissection and apply a suture, such as a purse string suture, to the other side of the tissue dissection. The instrument comprises an elongate shaft, first and second jaws configured to open and close and a cutting element. The instrument includes a first row of staples in each of the first and second jaws having a suture extending therethrough and a second row of staples in the second jaw. A drive member is configured to translate distally through the end effector to dissect tissue with the cutting element, drive the second row of staples into the tissue on one side of the dissection and to apply the first row of staples and suture in combination to form a purse string suture on the other side of the tissue dissection.

A delivery system and method for delivering a prosthetic heart valve to the aortic valve. The system includes a delivery catheter having a steering mechanism thereon for delivering a balloon-expandable prosthetic heart valve to the aortic valve through an introducer passing into the left ventricle through its apex. The introducer may have a more floppy distal section than a proximal section to reduce trauma to the heart wall while preserving good operating field stability. The delivery catheter includes a deflecting segment just proximal to a distal balloon to facilitate positioning of the prosthetic heart valve in the proper orientation. A trigger in a catheter handle may be coupled to a deflection wire that actuates the deflecting segment, while a slider in the handle controls retraction of a valve pusher. The prosthetic heart valve may be installed over the existing calcified leaflets, and a pre-dilation valvuloplasty procedure may also be utilized.

A surgical instrument includes a mount body, a joint member, an arm, and a working end. The mount body has a top portion, a distal end, a proximal end and a bottom portion. The joint member is pivotally mounted at a distal end portion of the mount body, to allow positioning of a proximal portion of an arm extending distally from the joint member. The joint member is also configured to at least partially constrain movement of the proximal portion of the arm to a plane. The working end is mounted to a distal end portion of the arm. The surgical instrument can be configured as a heart stabilizer or a heart positioner. The joint member may further be configured as a slotted ball, a disk member, or a combination thereof.

A surgical apparatus includes a cartridge; a staple strip; and staples affixed to the staple strip. Each staple is affixed to the staple strip at a joint. The staples on each staple strip are arranged in rows. The staples in adjacent rows are staggered. A wedge assembly includes a plurality of wedges spaced from one another, each wedge including a shear bump on its upper surface. Each shear bump has a first length. The shear bumps are aligned with one another within a distance equal to the first length. The wedge assembly is slidable relative to the staples to cause at least one wedge to make contact with one of the staples to rotate the staple about the joint, and then break the staple from the joint with the staple strip upon contact between the at least one of the staples and the shear bump of the wedge.

The introducer may have a more floppy distal section than a proximal section to reduce trauma to the heart wall while preserving good operating field stability. A surgical stabilizer for use with a surgical site retractor has a base, a bendable arm, and a distal cuff adapted to resiliently hold a tube of an elongated port-access device. The cuff may have a body defining a partial enclosure within which is held a highly flexible gasket having a slit for resiliently receiving the tube. The surgical site retractor may have a collapsible ring and a flexible outer portion attached thereto, the ring being sized to pass through an intercostal incision and expand therein under adjacent ribs to prevent removal, and the flexible outer portion extending out of the incision and drawing over the stabilizer base to mutually secure the retractor and base. The port-access tube may be for a heart valve delivery system using an elongated port-access device for transapically delivering a prosthetic heart valve to the aortic valve annulus. A method involves partly installing the surgical site retractor, anchoring the base of the stabilizer with the flexible outer portion, deploying the port-access tube from outside the body through the incision and through a puncture in the heart wall, and resiliently capturing a tube of the port-access within the partial enclosure of the stabilizer cuff. A second bendable arm on the base having a clip may be used to hold still a proximal end of the port-access device.
