A prosthetic heart valve includes a collapsible and expandable stent having a proximal end, a distal end, an annulus section adjacent the proximal end and an aortic section adjacent the distal end, the stent including a plurality of struts. A cuff may be coupled to the stent so that a flat, bottom edge of the cuff lies adjacent the proximal end of the stent. A pattern of stitches may be circumferentially disposed around the flat bottom edge of the cuff, the pattern of stitches alternating between stitches sewn to the cuff only and stitches sewn to both the cuff and the stent.

The flow direction may be defined from the proximal end toward the distal end. The sealing structure may have an outer edge remote from the inner edge. The cuff may be formed of a fabric and a second material different from the fabric, the cuff having commissure peaks and a plurality of midpeaks disposed between the commissure peaks. The heart valve further includes a valve assembly secured to the stent, the valve assembly including a cuff and a plurality of leaflets, each of the leaflets having a free edge having a first end and a second end, and a second edge secured to the cuff, the second edge having a first end, a second end, a first folded portion adjacent the first end, a second folded portion adjacent the second end, and an unfolded portion between the first and second folded portions. The first and second folded portions couple the leaflet to ones of the commissure features.

A prosthetic heart valve includes a stent extending in a longitudinal direction and having a plurality of commissure features. A valve assembly is secured to the stent, the valve assembly including a cuff and a plurality of leaflets. Each of the leaflets has a pair of attachment regions and a free edge extending between the attachment regions. A deflection feature disposed in each attachment region is configured and arranged to prevent the free edges of the leaflets from contacting at least one of the stent or the cuff.
