A cystotomy apparatus includes a sound from which an advancement device having a cutting tip creates a surgical pathway from the bladder through abdominal tissue to the external abdomen along a predetermined path. The cutting tip is captured and removed from the advancement device in a capture cup located in the predetermined path by an alignment structure. The cutting tip is removed from the advancement device while within the capture cup, to allow a dilator, cannula or catheter to be drawn through the surgical pathway into the bladder.

The apparatus includes an elongate tubular body including a proximal portion and a distal portion inclined relative to one another. The distal portion extends along an axis to an end configured for receipt through a urethra. Cystotomy catheter capture device

A bladder access apparatus and method of provided access into a bladder through a abdominal wall therewith is provided. The apparatus has an annular body extending between opposite ends with a through passage extending between the ends. A tubular sheath is attached to one end of the body in coaxial alignment with the through passage. The sheath is expandable from a first diameter to an enlarged second diameter. The apparatus has an elongate stylet with a rod sized for receipt in the sheath. The rod has a length greater than the tubular sheath and a diameter substantially the same as the first diameter of the sheath. The apparatus includes a rigid, tubular body having an outer diameter corresponding substantially to the second diameter of the sheath and a rigid, cylindrical member sized for sliding receipt in the tubular body.

A knot tying apparatus for tying a pair of fishing lines to one another is provided. The apparatus includes a tubular body having a through passage extending between opposite first and second ends with a slot extending from the first end toward the second end. Further, the apparatus has an actuator coupled to the tubular body for sliding movement between the first and second ends. The actuator is moveable between the first and second ends via application of an external force to removed loops of the one of the lines from the tubular body onto the other of the lines.

The knot tyer in accordance with the present invention includes, a hollow, elongated body member with a coil expansion spring mounted tubular piston therein and a release thereon. The hollow, elongated body member has a tubular piston slidably mounted and constrained within a cylindrical, hollow bore therethrough. The tubular piston has a first shoulder, a second shoulder, and a groove extending around the diameter of the tubular piston. The cylindrical, hollow bore in the rear end of the hollow, elongated body member accommodates in free longitudinally sliding manner a first shoulder on the tubular piston as well as a coil expansion spring encircling the tubular piston. The cylindrical, hollow bore in the front end of the hollow, elongated body member accommodates in free longitudinally sliding manner a second shoulder and a groove on the tubular piston. The release is pivotally attached to the hollow, elongated body member by a pin, which pin allows the release to pivot on the external longitudinal axis of the hollow, elongated body member. The tubular piston has a grooved front end. The front end of the tubular piston is grooved so as to be open and hollow internally.

A new endocystotomy tool for suprapubic cystostomy has a rigid sound, said rigid sound having standard configuration such as a standard Van Buren urethral sound. Said rigid sound having a distal end, said distal end having a removable tip, said removable tip being rounded. A second embodiment of said new endocystotomy tool for suprapubic cystostomy has an extendable trocar slidably seated within the rigid sound. Said second embodiment of said new endocystotomy tool for suprapubic cystostomy having a proximate end thereof which is provided with an insufflation port and a spring loaded plunger, which spring loaded plunger activates said extendable trocar. Either a catheter, a sheath, or a cystoscope is passed inside the tip of the new endocystotomy tool and secured therein. Thus the catheter, sheath, or cystoscope easily slides through the abdominal wall and bladder, i.e., the diameter of the catheter, sheath, or the cystoscope being smaller than the diameter of the new endocystotomy tool.
