Compositions and methods are provided for treating joint conditions, such as osteoarthritis and/or the pain associated therewith. The compositions and methods utilize a first component, namely hyaluronic acid ('HA'), in combination with a lyophilized second component that is effective to at least temporarily reduce the viscosity of the HA. In an exemplary embodiment, the second component is one or more glycosaminoglycans ('GAG'), such as chondroitin sulfate (&#x201c;CS&#x201d;), including CS4 and/or CS6, dermatan sulfate, heparin, heparan sulfate, and keratan sulfate. The composition can optionally include other joint supplements, such as glucosamine (&#x201c;GlcN&#x201d;).

Compositions and methods for treating joints

Various exemplary methods, systems, and devices for blood flow are provided. In general, an implant can be configured to be implanted in bone and to delay clotting of blood flowing from the bone. The implant can include an anti-coagulation agent to delay the clotting of the blood. The anti-coagulation agent can be a coating on the implant, can be natural to a material forming the implant, or can be impregnated into a material forming the implant. In an exemplary embodiment, the implant is implanted in a bone in a surgical procedure for securing a soft tissue to bone, such as a rotator cuff repair procedure or an anterior cruciate ligament (ACL) repair procedure.

A biocompatible tissue repair implant or scaffold device is provided for use in repairing a variety of tissue injuries, particularly injuries to cartilage, ligaments, tendons, and nerves. The repair procedures may be conducted with implants that contain a biological component that assists in healing or tissue repair. The biocompatible tissue repair implants include a biocompatible scaffold and particles of living tissue, such that the tissue and the scaffold become associated. The particles of living tissue contain one or more viable cells that can migrate from the tissue and populate the scaffold.

A small diameter delivery device capable of delivering a tissue loaded scaffold arthroscopically to a tissue defect or injury site without reducing the pressure at the injury site is provided. The scaffold delivery device of the present invention comprises a plunger system that includes two main components: an insertion tube and an insertion rod. The insertion tube has a flared proximal end for holding a tissue scaffold prior to delivery. An elongate, hollow body extends from the flared proximal end to a distal end of the insertion tube, and defines a passageway that extends through the body for delivery of the tissue scaffold. The insertion rod has an elongate body that extends into a handle at a proximal end and a tip at a distal end. The insertion rod is configured to be removably disposed within the insertion tube for sliding along the passageway to effect delivery of the tissue scaffold through the insertion tube.

Methods and compositions are disclosed for an intra-articular injection for the treatment of osteoarthritis. The methods and compositions comprising combinations of hyaluronic acid and a bone morphogenetic protein, like rhGDF-5, can be useful for any synovial joint, including the knee, shoulder, hip, ankle, hands, spinal facet, or temporomandibular joint, both for the relief of pain and for slowing disease progression.

A treatment system comprises an outer sheath and an end effector. The end effector has a plurality of cutting features, at least a portion of which are configured to debride a tissue surface when the end effector is actuated. The cutting features may also be configured to harvest tissue. A medical fluid reservoir is in fluid communication with the end effector, such that the end effector may deliver a medical fluid to a target site such as a fistula. The medical fluid may include harvested tissue cells and a scaffold material. A vacuum cannula may be positioned coaxially within the interior cavity of the end effector, and an RF probe may be positioned co-axially within the interior cavity of the vacuum cannula. The vacuum cannula and RF probe may be used together to seal a fistula in which medical fluid has been dispensed.

Instrument for debriding fistula and applying therapeutic cells

A tissue treatment device comprises a housing having a chamber that receives a treatment fluid mixture of tissue morsels and a scaffold material. A debriding shaft including a plurality of cutters is coupled with the housing. The debriding shaft extends proximally within the chamber and extends distally from the end of the housing. The debriding shaft is inserted within a treatment site such as a fistula tract and is rotated and reciprocated such that the debriding shaft removes tissue cells along the site. The site is flushed with saline or other irrigating fluid followed by application of the treatment fluid mixture. The chamber of the housing comprises agitators positioned adjacent the debriding shaft. The treatment fluid mixture passes through the chamber of the housing along and between the outer surface of the debriding shaft and the agitators for thorough mixing before exiting the device at the treatment site.

The repair procedures may be conducted with implants that contain a biological component that assists in healing or tissue repair.
