A valve mechanism for coupling to a trocar comprises a housing having an instrument bore extending therethrough from an upstream end to a downstream end and alignable with an instrument bore of the trocar. A gate valve comprising a pair of gate elements located in the housing is operable between a closed state closing the instrument bore and an open state clear of the instrument bore. First compression springs urge the gate elements into the closed state. A pair of engagement members, which are engageable with an instrument, for example, a laparoscope in the instrument bore, and which extend into the instrument bore from respective carrier elements through guide slots. The carrier elements are coupled to operating members of the gate elements through link members for operating the gate elements () from the closed state to the open state as the engagement members () are urged downwardly in in guide slots by an instrument being entered through the instrument bore.

Apparatus () for relieving dyspnoea in an ambulatory exercising subject comprises a housing () to be worn on the subject, and within which a variable speed air blower motor () is located for delivering an air supply to the subject through a mouthpiece () at a pressure similar to average intrinsic positive end-expiratory pressure of the subject. A pressure transducer () in the mouthpiece () monitors the pressure during breathing cycles of the subject, and a control circuit () determines the average intrinsic positive end-expiratory pressure of the subject over five breathing cycles. The control circuit () operates the air blower motor () for delivering the air supply to the subject at a pressure similar to the determined average intrinsic positive end-expiratory pressure.

Apparatus and method for relieving dyspnoea

The gas mixing apparatus provides the components of a breathing gas for mixing at approximately ambient atmospheric pressure, and regulates the pressure of a selected gas to approximately ambient atmospheric pressure for mixing with air at ambient atmospheric pressure. The gas mixing apparatus includes a piston disposed within a pump chamber. A flow limiting inlet controls introduction of a first selected gas such as oxygen for mixing with a second selected gas such as air. The pressure of the first selected gas is limited to an acceptable maximum pressure, so that even if a valve for admitting the first selected gas for mixing at ambient pressure fails, breathing gas will not be provided at an excessive pressure. A demand valve is alternatively provided for reducing the pressure of the first selected gas supplied before mixing, but a pressure sensor is also provided downstream of the demand valve for detecting failure of the demand valve, to shut off the supply of the first selected gas to prevent overpressurization.
