Medication delivery pen

A medication delivery pen includes a housing having opposing proximal and distal ends. An actuator is disposed in the proximal end of the housing for setting and administering a dosage of medication. The pen also includes a medication-containing cartridge assembly having a cartridge with a pierceably sealed distal end, an open proximal end removably attachable to the distal end of the housing, and a piston in sliding fluid tight engagement within the cartridge. A drive mechanism is coupled between the actuator and the cartridge to exert an axial force on the piston to inject the set dosage of medication. The actuator triggers the drive mechanism to administer the injection of medication held in the cartridge. A processor is coupled to the actuator to determine a value equal to the dosage set by the actuator. A memory device is coupled to the processor to store at least the dosage value determined by the processor. A display is located on the housing and coupled to the processor for displaying information provided by the processor. In a first state the actuator engages with the drive mechanism to deliver the medication and in a second state is disengaged from the drive mechanism. In its second state, the actuator serves as a user adjustable input for changing and setting mode parameters of at least one preselected mode of the pen.

Electronic medical delivery pen having a multifunction actuator

A medication delivery pen includes a housing having an actuator disposed in the proximal end of the housing for setting and administering a dosage of medication, a medication-containing cartridge assembly having a cartridge with a pierceably sealed distal end, an open proximal end removably attachable to the distal end of the housing, and a piston in sliding, fluid tight engagement within the cartridge. A drive mechanism is coupled between the actuator and the cartridge to exert an axial force on the piston to inject the set dosage of medication. The actuator triggers the drive mechanism to administer the injection of medication held in the cartridge and a processor is coupled to the actuator to determine a value equal to the dosage set by the actuator. The drive mechanism includes a pair of half nuts in rotational engagement with the actuator, a non-rotatable lead screw having a distal end for exerting the axial force on the piston to inject the set dosage of medication, a proximal end, and threads extending between the proximal and distal ends. The nuts open and close radially to selectively engage with the threads of the lead screw for axially advancing along the lead screw upon rotation of the actuator.

A surgical device is described for assisting a surgeon in suturing bodily tissue using a curved, surgical needle with a suture filament attached. The surgical device comprises a handle, an elongated shaft having a proximal end, a distal end, and a longitudinal axis extending in between. The device further comprises a stationary arm having a first holder fixedly attached to and extending from the distal end of the shaft, and used for operationally engaging with the needle. The device also has a movable arm having a second holder extending from the distal end of the shaft and rotatable around the longitudinal axis of the device. The movable arm also is operationally engageable with the needle. An actuator is provided for cooperatively actuating the first and second holders, so as to be able to pass a needle along a circular path transverse to the longitudinal axis of the device through the tissue, and thus place the suture filament into the tissue.

An improved medication delivery pen is provided for injecting fluids such as insulin within body tissue. The medication delivery pen includes a mechanism that prevents the removal of a cartridge unless an injector button on the medication pen is in a predefined position, a bayonet attachment and an improved clutch assembly in a dose setting mechanism that provides improved control over the torque necessary to rotate a units counter ring in the medication delivery pen using a dose setting knob.

The handle has a blade mounting portion with a raised boss forming a slot. The blade has a boss receiving recess with a portion defined by an edge. The cartridge removeably retains the blade. It has an opening into which the blade mounting portion of the handle can be inserted. A normally arcuate spring is mounted in cantilever fashion within the enclosure. The tip of the blade abuts a stop surface proximate the unattached end of the spring. In its arcuate state, the spring retains the blade in a position where the edge of the blade can enter the slot in the handle. As the blade is moved by insertion of the handle, against the urging of the spring, the spring straightens allowing the blade to assume a position where the boss can be fully seated within the recess, mounting the blade on the handle. The blade is then removed from the cartridge by withdrawing the handle. After use, the blade is reinserted into the cartridge. A pushbutton, accessible from the exterior of the cartridge, is depressed to cause the boss to withdraw from the slot to dismount the blade from the handle. A stand for retaining multiple cartridges is provided. Each cartridge is supported by a rigid finger aligned with the pushbotton. Applying a downward force on the cartridge causes the pushbutton to depress to dismount the blade.

A surgical needle includes an elongated needle shaft and a needle head having a sharp point. A frangible neck, with a reduced cross-section, joins the needle shaft and the needle head and permits the needle head to be broken away from the needle shaft.

Surgical purse string suturing instrument and method

One or more devices for stretching skin to close a wide skin defect may be used with two long interdermal needles which are inserted underneath the skin along margins of the wound. The skin stretching device may include a pair of needle shafts each having a hook or needle at a distal end for engaging one of the interdermal needles, and a contracting mechanism provided for drawing the opposing needle hooks toward one another, thereby approximating the opposing margins of the wound. The skin stretching device may also include a control knob for limiting the amount of force applied by the device, and a tension indicator for indicating the amount of force applied by the device. Each elongated shafts may also exhibit a retaining member about which a swivel platform rotates, and to which two or more needles are attached. The skin stretching device may further include elongated shafts which, at their distal ends, angle away from the body of the shafts, each exhibiting a single needle. Finally, the skin stretching device may include curved shafts which exhibit one or more needles at their distal ends.

A purse string suturing instrument is provided for quickly and effectively forming a purse string suture in tubular tissue through relatively small cannulas placed in the body. The instrument includes a pair of elongated jaws, each having a row of spaced-apart uniform-size teeth. The jaws are hinged together at one end to clamp the tubular tissue therebetween. The two rows of teeth are offset with respect to each other so that they mesh when the jaws are closed. The jaws are inserted through a cannula and positioned to close around the tissue to be sutured. The rows of teeth then bend the tissue between them into a wavelike configuration. A pair of needles, the ends of each being connected to a single length of suture, is then inserted through passageways provided through the teeth. An encircling series of stitches are thus formed in the wall of the tubular tissue to create a purse string suture. The needles and the suturing instrument are then withdrawn through the cannula. The tubular tissue can be snugly fitted over an anastomosis ring or the like by drawing and tying the purse string suture.

There is described herein a ligating clip applier which contains: a handle assembly; a jaw assembly connected to the handle assembly and containing a shaft and a pair of jaws. The shaft defines a longitudinal axis which lies in a central horizontal plane. A magazine of clips is stored on one side of the plane. The jaws are positioned so that they are on the opposite side of the plane. This configuration allows the clip to be loaded into the jaws across the plane, resulting in greater visibility of the clips as they are loaded into the jaws. Further, there is provided a jaw closure mechanism operable in connection with the jaws of this device so that the jaws contact the legs of a ligating clip first distally and then proximally. Thereafter the jaws are parallel at least once during closure, and the closure of the clip legs is, indeed, distal and then proximal. This improved procedure insures that in virtually all instances, the angle included between the clip legs at the crown is 0.degree.. Motion of the clip feeding and jaw closing mechanisms are accomplished without lost motion, via a pair of interengaged mechanisms connected by a single tension spring to one another, and operating separately from the body of the device. This configuration results in a more efficient, easier to operate more reliable clip applying device.

A purse string suturing instrument is provided for quickly and effectively forming a purse string suture in tubular tissue through relatively small cannulas placed in the body. The instrument includes a pair of elongated jaws, each having a row of spaced-apart uniform-size teeth. The jaws are hinged together at one end to clamp the tubular tissue therebetween. The two rows of teeth are offset with respect to each other so that they mesh when the jaws are closed. A single operating mechanism is provided to articulate the pair of jaws with respect to a longitudinal axis of the suturing instrument and actuate the jaws between an open position and a clamping position. The jaws are inserted through a cannula and positioned to close around the tissue to be sutured. The rows of teeth bend the tissue therebetween into a wavelike configuration. A pair of needles, the ends of each being connected to a single length of suture, is then inserted through passageways provided through the teeth. An encircling series of stitches are thus formed in the wall of the tubular tissue to create a purse string suture. The needles and the suturing instrument are then withdrawn through the cannula. The tubular tissue can be snugly fitted over an anastomosis ring or the like by drawing and tying the purse string suture.

An endoscopic clip applying system as described which contains a venting system and a clip applying support system which prevents closure of the mechanism before firing. Also, the system ensures proper loading of the clips and prevents them from falling out during loading. Further, the system describes an easily manufacturable, positively opening system which has minimized size requirements. Furthermore, a lock-out mechanism is provided so that the mechanism may not be inadvertently fired. The shaft resists excessive torque as well as holds the clip in place during firing. There are provisions for sealant and non back-up of the clips, as well as features which make the clip properly placed for closure.

A safety trocar is provided which includes a spring-loaded shield that shields the cutting tip of the obturator after the obturator penetrates tissue. The distal end of the shield is hemispheric in profile and contains a slot which conforms to the geometry of the cutting tip. Instead of using a discrete shield component the shielding function may be provided by spring-loading the trocar tube itself to spring forward and shield the obturator tip after the tip penetrates tissue. Alternately a safety mechanism insures the tip is covered by the shield after the tip penetrates tissue.
