Methods and apparatus for posterior lumbar interbody fusion (PLIF) surgical procedures. The apparatus includes a sleeve assembly for guiding an interbody graft into position between adjacent anatomical structures. The sleeve includes a first piece having a first internal ramp and a second piece having a second internal ramp opposite the first internal ramp, and an elastic band configured to connect the first and second pieces together. The first and second internal ramps comprise a V shape with an opening therebetween through which the interbody graft is urged during the surgical procedure.

Methods and apparatus for facilitating a posterior lumbar interbody fusion procedure

Methods and apparatus for direct visual rhizotomy using an elongated tubular sheath. The sheath includes: a proximal end configured to removably receive a cannula; a distal end; a horizontal shaft having a longitudinal axis extending between the proximal end and the distal end; a first cut-away generally defining a first plane at the distal end, the first cut-away characterized by a first angle relative to a vertical plane; and a second cut-away generally defining a second plane at the distal end, the second cut-away characterized by a second angle relative to a horizontal plane. The sheath further includes an endoscope camera disposed at the distal end, the camera having a line of sight substantially orthogonal to the first plane.

Methods and apparatus for facilitating direct visualized rhizotomy

Some embodiments include a privacy/security apparatus for a portable communication device that includes a housing assembly configured to at least partially attenuate at least one of sound energy, acoustic energy, and electromagnetic energy including light, optical, and IR energy and RF radiation from passing through the housing assembly. The housing assembly includes a Faraday cage with two or more portions, and at least one protective shell coupled to or forming at least one aperture. The at least one aperture is configured and arranged to at least partially enclose the portable communication device so that at least a portion of the portable communication device is positioned within at least one portion of the Faraday cage, and the at least one seal coupled or integrated with the protective shell. The housing assembly can be an articulating assembly, a sliding assembly, and can include an active acoustic jamming or passive acoustic attenuation element.

A combination bathroom plunging and cleaning tool. Implementations may comprise a handle with a plurality of different detachable accessories. Accessories may include a plunger, a brush and a scrubber, for example. One or more of the accessories may comprise electronic components such as a vibrator or rotator to further accentuate the cleaning process. The handle and accessories may be stored in a storage caddy when not in use and the storage caddy may comprise air vents and a fan to dry the accessories after use.

A plurality of approaches for forming a horizontal and vertical electrical device cover including 1) providing hinge members on opposing sides of the cover that allow the cover to pivot along adjacent-sided axes for horizontal and vertical mounting; 2) providing a removable pin and corresponding cover and base hinge members wherein the removable pin may be placed along either the horizontal or vertical axis for selective horizontal or vertical mounting; 3) providing a pivoting hinge arm that is pivotable between adjacent sides of the unit for selective horizontal or vertical mounting; and 4) providing selectively attachable hinge members that may be attached by the installer at the time of installation to choose between horizontal or vertical mounting configurations.

An electrical box assembly that includes a box body having a screw mount aperture and a base insert slidably coupled into the box body. The base insert may include a back wall, and the back wall may have an electrical device aperture therethrough, a box mounting screw aperture therethrough, a center mounting screw aperture therethrough, and a rear face. The rear face may include a center mounting plane at the center mounting screw aperture substantially parallel to the back wall and at least one spacer extending above the center mounting plane from the rear face at the box mounting screw aperture. Other particular implementations of an electrical box assembly may include at least two removable flanges coupled along a front side of the box body.

Recessed electrical outlet cover and box

A simplified installation design for an in-use cover for an electrical outlet includes conical mounting screw holes in the outlet box and movement-restricting guides to ensure the mounting screw aligns with the mounting screw holes. Angled side mounting screw holes allow the outlet box to be installed with screws from within the box rather than the conventional nails outside the box. In particular embodiments of the invention, prongs, screws, clamps, heat stakes or other attachment components extend from the back of the insert to allow for pre-installment attachment of the electrical device directly to the insert. This also simplifies installation of the recessed in-use covers. Adaptors may optionally be included to allow the device to be adapted for use with a variety of sizes and shapes of electrical devices. An alternate embodiment of the invention includes a recessed in-use outlet formed of an electrical box, a receptacle, a cover plate and a cover to allow for simple installation with fewer parts. Various embodiments may be installed in new construction or after wall coverings have been installed.
