Knotless surgical constructs and methods of tissue repairs. A surgical construct can offer both repair and shuttling capabilities to allow for a single pass to load multiple sutures at once. A surgical construct can pass or shuttle multiple repair sutures while making a single pass through or around soft tissue.

This disclosure details surgical instrumentation and related techniques for performing bone reconstruction surgeries. The surgical instrumentation may be used to size and shape a graft, such as an allograft. The appropriately sized and shaped graft is subsequently used to reconstruct damaged bone. A surgical instrumentation set is used to prepare the graft. The surgical instrumentation set includes a graft workstation, a plurality of sizing blocks and a plurality of cutting blocks.

Instrumentation and technique for sizing a bone reconstruction graft

A fenestrated suture anchor for fixation of tissue to bone. In a preferred embodiment, the suture includes an implant, such a swivel implant with a forked tip for capturing suture. The implant is secured in a hole in bone by advancing a fixation device, such as a cannulated interference screw, to engage and fully seat the implant. The cannulated fixation device is provided with a plurality of openings or fenestrations of various dimensions and geometries to provide multiple pathways through the anchor (i.e., though the interior of the body and through the fenestrations) to allow blood to flow to increase the healing zone, for example, for rotator cuff repair, while also promoting bone in-growth.

Techniques and reconstruction systems for knotless tissue fixation using a driver with a cannulated interference device or fixation device preloaded on the shaft of the driver. An eyelet, adjustable in size, is formed of a loading suture provided at the distal end of the driver shaft. A working suture attached to the tissue to be fixed is passed through the eyelet. The size of the eyelet is adjusted by the user once the working suture is passed through the eyelet, to capture the working suture with the eyelet. The cannulated interference device is then advanced down the shaft to secure the working suture in a bone socket.

Instrumentation for an open or arthroscopic Latarjet procedure that allows more control graft handling during the formation of the hole and proper positioning parallel to the articulating bone surface. The surgical instrumentation of the present invention includes an osteotome with a plurality of markings and a depth stop, a drill guide with jaws configured to securely engage an outer surface of the graft, and an offset guide to position the graft to the bone.

Latarjet instrumentation and method

A method and device for knotless fixation of tissue. A swivel anchor having a rotatable implant is used to capture suture for surgical tissue repair without requiring suture knots. The implant may be provided with a conical metal tip which is self-punching and avoids the need for pre-drilling a hole in bone. The implant includes a closed aperture to allow free sliding of a suture strand. The swivel anchor is secured in a hole in bone by advancing a fixation device, such as a cannulated interference screw, over the body of the implant.

A bone plate with a new design that prevents the bushing from rotating and/or dislodging from the screw hole during screw insertion. The plate is provided with through-holes having internal walls with at least one flat that is configured to align with a corresponding flat provided on the outer wall of the bushing. The bushing is inserted by orienting the bushing in a direction perpendicular to the through-hole of the plate so that the center of the bushing is aligned with the center of the through-hole and the flat on the bushing is aligned with the flat on the through-hole of the bone plate. The bushing is introduced into the through hole until the centers are aligned, and then the bushing is rotated about 90 degrees so that the bushing is captured within the through-hole of the plate. Once captured, the bushing cannot rotate, dislodge from, or come out of the plate unless the bushing is turned 90 degrees. The flat on the bushing mates with the flat on the internal wall of the hole (screw hole) to prevent it from rotating within the screw hole, and avoiding the need for a bushing guide during screw insertion.
