An implantable medical device, IMD, comprises atrial and ventricular sensing units for sensing atrial or ventricular electric events. The IMD also comprises atrial and ventricular pulse generators for generating atrial or ventricular pacing pulses. A controller controls the operation of the IMD () according to a first mode, in which the ventricular pulse generator is prevented from generating a back-up pulse if an evoked response detector fails to detect evoked response to a delivered ventricular pacing pulse, and a second mode, in which the ventricular pulse generator is controlled to generate a back-up pulse if no evoked response is detected following delivery of a ventricular stimulating pulse. The controller switches operation from the first mode to the second mode based on the evoked response detector failing to detect an evoked response to a delivered ventricular pacing pulse.

Cardiac stimulating device

The present invention relates generally to medical devices for electrode positioning during implantation procedures. A cardiac signal measuring device measures cardiac signals sensitive to inherent differences between cardiac tissue and blood using at least one electrode of a medical lead arranged at a distal tip of the medical lead and at least a second electrode arranged at a distance from the distal electrode and being connectable to the measuring unit. An analyzing module acquires cardiac signals measured during predetermined measurement sessions. The analyzing module determines at least one cardiac signal value based on the cardiac signals for each measurement session and analyzes changes of the cardiac signal values between different measurement sessions to determine a position of the electrode relative a tissue border. A maximum of the change of the cardiac signal values between two successive measurement sessions indicates that the electrode has reached the tissue border.

Medical device for positioning of a lead

An implantable medical system for delivering pacing pulses to HIS bundle of a heart of a patient when implanted in said patient includes a medical lead, which is adapted to be attached with a distal end to tissue of said heart, including a at least two electrodes arranged being electrically separated from each other. The implantable medical device connectable to the medical lead includes a pacing circuit adapted to deliver the pacing pulses to said heart via the medical lead, a selection device connected between the pacing circuit and the electrodes adapted to selectively activate at least one of said electrodes, and a processing device adapted to control the selection device to selectively activate at least one of the electrodes to direct the pacing pulses to the HIS bundle.

Medical device for stimulation of the HIS bundle

A medical implantable lead of the kind being adapted to be implanted into a human or animal body for monitoring and/or controlling of an organ inside the body has a penetrating fixation element in a distal end, which is adapted to penetrate into the tissue of the organ to fixate the lead such that a distal end of the lead will be in contact with the organ. The lead also has an electrode member to receive and/or transmit electrical signals from and/or to the organ. The lead has in a distal portion a movable member, which is displaceable in an axial direction of the lead and is actuated by a resilient member to be, in an initial state, maximally protruded in a distal direction in relation to the lead and which comprises a radiopaque material for forming of a first indication marker. The lead also has a second indication marker of a radiopaque material in relation to which the movable member is displaceable, and the first and the second indication markers are arranged such that, when observing the lead in an implanted state by means of fluoroscopy, it will be recognizable from the relative positions of the first and second indication markers, whether the distal end of the lead is in close contact with the surface of the tissue or not. A comparable method ensures proper and safe attachment of a medical implantable lead to an organ.

In a medical implantable lead of the type adapted to be implanted into a human or animal body for monitoring and/or controlling of an organ inside the body and a method for connecting such a lead to an organ in the human or animal body. The lead has a fixation arrangement at a distal end, and the fixation arrangement is adapted to penetrate into the tissue of the organ to fixate the lead to the organ. An electrode member is provided to receive and/or transmit electrical signals from or to the organ. The electrode member is resiliently pre-strained toward the distal end of the lead and is provided with an electrode surface such that the electrode surface will resiliently abut toward the outer surface of the organ when the fixation arrangement is fixed to the tissue.

Medical implantable lead and method for connecting a medical implantable lead to an organ

For a suture sleeve and implantation method for one or two implantable leads, the suture sleeve is adapted to be inserted into a vein to secure and protect the lead from damage when a suture thread is positioned and tied around the vein in the region over the suture sleeve to prevent bleeding from the vein. The suture sleeve has two lead receiving through holes, into each of which a medical implantable lead is insertable. The suture sleeve also has two elongated sleeve portions, each including a lead receiving through hole, which are positioned in parallel and displaced in relation to each other such that they are connected in a connecting portion. At least one sleeve portion projects further in one direction.

A medical implantable lead has a proximal end and a distal end, a biostable and biocompatible polymeric header, which is arranged at the distal end and has a first tubular portion, a helical fixation element located within the first tubular portion and being extendable from a distal end of the header, and a radiopaque ring. The header further has a second tubular portion, which is arranged coaxially of the first tubular portion and is attached to the first tubular portion at a distal end of the second tubular portion, while having a free proximal end. The tubular portions form a circumferential pocket, wherein the first tubular portion extends from the proximal end of the header at least to said distal end of the second tubular portion. The radiopaque ring is arranged around the first tubular portion and is received in the circumferential pocket.

Medical implantable lead

An implantable lead for delivering electrical stimuli to a human heart has a stimulating electrode for transmitting electrical stimuli to the myocardium after implantation, and a mapping electrode for use during implantation. The mapping electrode is configured to deliver electrical stimuli to the heart and to sense intrinsic cardiac activity, for the purpose of finding a suitable fixation position in the myocardium. During the implantation procedure, the mapping electrode is electrically connected to the conductor. The lead is configured to electrically disconnect the mapping electrode from the conductor after a suitable fixation position has been determined.

An implantable heart stimulator has an electrically conductive housing containing a pulse generator, with an electrode lead connected to the housing. The electrode lead has a proximal portion extending substantially from the housing to a location, after implantation, which is beyond the entry of the lead into the venous system and before the entry of the lead into the superior vena cava. A current source supplies an infection control current between the housing and an electrically conductive surface on the exterior of the proximal portion of the electrode lead, for counteracting bacterial growth. The housing may have a header to which the electrode lead is connected, in which case the header is provided with an electrically conductive surface as well, which can serve as an electrode for the infection control current.

An implantable heart stimulator has a pulse generator, implantable in a subcutaneous pocket, and an electrode lead connected thereto for delivering stimulation pulses to a patient's heart. The pulse generator has an electrically conductive housing, and the electrode lead has a proximal portion which after implantation, extends substantially from the housing to a location situated beyond entry into the venous system and before entry into the superior vena cava. The proximal portion has an exterior with an electrically conductive surface, which together with the housing, form an infection control current electrode. A counter electrode is disposed outside of the subcutaneous pocket, and a current source supplies an infection control current between the infection control current electrode and the counter electrode for counteracting bacterial growth at least on an exterior of the housing.

In a cardiac stimulating device, an infection control current is generated and applied to a subject in whom a medical device is implanted by completing a circuit including an entirely electrically conductive exterior of a housing of the device, an electrode carried by an electrode lead of the device, and an electrically conductive surface of a proximal portion of the electrode lead, extending from the housing to a location, after implantation, situated beyond entry of the electrode lead into the venous system and before entry of the electrode lead into the superior vena cava. The infection control current can be a current with a square waveform, and the circuit can be completed at selected times, such as only during the refractory period of the subject's heart.
