A colpotomy assembly 10 is adapted for insertion into a vaginal cavity for use in female pelvic surgical procedures employing the inflation of an abdominal cavity with a gas to facilitate the accessability to and visibility of female pelvic organs. The assembly has a vaginal extender 20, a uterine manipulator 16 and a flexible occluder. The occluder being mounted on a shaft of the uterine manipulator such that it permits relative movement of the shaft 26 during use.

Uterine manipulating assembly for laparoscopic hysterectomy

A colpotomy assembly (10) having a vaginal extender (20) and uterine manipulator (16) for use in performing various female pelvic surgeries, including laparoscopic hysterectomies, laparoscopically assisted vaginal hysterectomies, and other female pelvic laparoscopic procedures where removal of tissue specimens through a colpotomy incision is indicated.

Vaginal extender for colpotomy surgery

A uterine manipulator and associated tip are provided for laparoscopic examinations. The manipulator hereof includes a handle pivotally connected to one end of an elongated frame and a drum positioned at the other end of the frame. The handle is connected to the drum to provide movement of the drum in a plane substantially parallel to the pivot axis of the handle. The handle may be locked relative to the frame in a plurality of desired positions by engagement of a pawl shiftable with the handle in one of a plurality of notches on the frame. The frame is provided with ports along the sides to facilitate cleaning and sterilization. The tip is mounted exclusively remotely to the frame of the uterine manipulator and includes a unique mounting system which maintains the proper orientation of the tip on the drum.

A uterine manipulating device is provided which provides an improved handle and locking element for holding the handle in a desired position relative to the frame to which it is pivotally connected. The locking element may include a latch element for holding the handle relative to the frame in a selected one of a plurality of slots defined in the frame. Alternatively, the locking element may include a clamp for permitting the handle to be held relative to the frame in any desired position through its range of motion, The inventor hereof also includes a method of supporting a uterine manipulator during surgery including supporting the manipulator by flexible material passed through the handle and tied or otherwise attached to the legs of the patient.

A uterine retractor having an elongated insertion rod having at a rear end a ratchet handle which may be rotated. The front end of the insertion rod mounts a retractor finger for rotation substantially perpendicular to the insertion rod in response to rotation of the ratchet handle. The retractor finger which may be inserted within the uterus, upon the insertion of the insertion rod within the vaginal tract of the patient, with the ratchet handle remaining outside of the patient. Movement of the retractor finger, due to movement of the ratchet handle, may thus impart movement to the uterus about two axes, allowing placement of the uterus for medical procedures. The insertion rod may mount a transilluminator in the form of a light source adjacent the front end of the insertion rod. The illumination is visible from the exterior of the vaginal tract, allowing easy location of the cervix, vaginal cuff, and veins and arteries within the vaginal tract.
