Pulsed field electroporation systems and methods

A surgical instrument and related method includes an instrument body, a guide shaft distally projecting from the instrument body, and a first surgical tool. The guide shaft has a guide sidewall, a guide lumen, and a clearance opening radially extending through the guide sidewall in communication with the guide lumen. The first surgical tool has an elongate body and a distal head configured to deflect relative to the elongate body from a first position to a second position. The distal head in the first position is positioned within the guide lumen along the central axis. The distal head in the second position is deflected from the central axis and extends at least partially through the clearance opening thereby vacating at least a portion of the guide lumen for introducing a second surgical tool through the guide lumen.

An apparatus includes a body, a shaft assembly, and a visualization assembly. The body includes first and second actuators. The shaft assembly extends distally from the body and includes a rigid proximal portion and a steerable distal portion. The steerable distal portion is positioned distally relative to the rigid proximal portion. The first actuator is operable to drive the steerable distal portion to deflect laterally relative to a longitudinal axis defined by the rigid proximal portion. The steerable distal portion is configured to fit within a nasal cavity of a patient. The visualization assembly is disposed within the shaft assembly. The visualization assembly includes a camera. The second actuator is operable to drive longitudinal translation of the visualization assembly relative to the shaft assembly.

Tools used within a surgical area may be equipped with sensors that allow them to be tracked within the magnetic field of an image guided surgery (IGS) system. The IGS system may be configured to detect various movement patterns of the tools, which may be mapped to and associated with corresponding actions or inputs to the IGS system. In one example, a registration probe may be moved along the x-axis and y-axis, with detected movements identified and received by the IGS system as movements of a mouse cursor on a display of the IGS system. In another example, the registration probe may be moved in a circular pattern, or quickly moved along any of the x-axis, y-axis, or z-axis, with each being configured to cause the IGS system to zoom a display, change a view, record video, or other actions.

A device can detect and retrieve a blood clot by advancing a catheter with a clot sensing element through a patient's vascular system. The catheter can map, using an electromagnetic sensor disposed at a distal end of the clot sensing element, the patient's vascular system. A force sensor can generate a position signal indicating the clot sensing element contacted the clot in the patient's vascular system. Once located, a blood clot retrieval device can be deployed through the catheter and a lumen in the clot sensing element to remove the clot from the patient's vascular system.

A surgical instrument for cutting a first tissue and detecting a second tissue includes a shaft, a cutting member, and a tissue monitor system. The shaft extends along a longitudinal axis and includes a shaft lumen and a shaft opening. The cutting member is disposed within the shaft lumen and configured to cyclically move from a first position to a second position relative to the shaft. The cutting member is further configured to cut a tissue portion of the first tissue for removal therefrom. The tissue monitor system is associated with at least one of the shaft or the cutting member configured to detect the second tissue distinct from the first tissue for selectively cutting and removing the first tissue relative to the second tissue.

An implant that may maintain position with a Eustachian tube of a patient, the implant includes an engagement body, an anchoring assembly, and a pressure balance assembly. The engagement body includes an exterior surface dimensioned to bear radially outward against the Eustachian tube. The anchoring assembly may lock the implant in the Eustachian tube. The pressure balance assembly includes a vented pathway that may transition between an occluded state and a vented state. The vented pathway may provide fluid communication between the proximal end and the distal end of the implant in the ventilated state while inhibiting fluid communication between the proximal end and the distal end of the implant in the occluded state.

A surgical instrument includes a handle assembly and a shaft assembly extending distally from the handle assembly and having a distal end sized to be inserted into the nasal cavity of a patient. The shaft assembly includes a cutting member configured to cut tissue within the nasal cavity, and a translating member slidably disposed over the cutting member. A navigation sensor is disposed within the distal end of the shaft assembly and is operable to generate a signal corresponding to a position of the distal end within the patient.

A virtual camera may be positioned relative to a 3-D model of a patient anatomy in order to provide a virtual camera view of the surrounding anatomy and tracked surgical instruments being deployed to the anatomy. Visual context provided by the virtual camera may be limited in some cases, such as where a surgical instrument is being used within a very narrow anatomical passageway or cavity. To provide more placement flexibility, an IGS system providing a virtual camera receives inputs defining variable visual characteristics for different segments or regions of the 3-D model, which may include hiding certain segments or making certain segments semi-transparent. With such a system, the view of the 3-D model provided by the virtual camera view may be modified to remove or deemphasize less relevant segments, to display or emphasize more relevant segments (e.g., critical patient anatomy), or both.

The heating feature is operable to ablate tissue of the Eustachian tube contacting the balloon in the expanded state. Alternatively, the heating feature may include a thermal heating element that heats the balloon inflation fluid and thereby heats the wall of the balloon. The electrodes are spaced apart from each other along a longitudinal axis and are operable to apply RF energy to tissue to thereby ablate the tissue.

An endoscope based system is configured to provide one or more procedure guidance features. The endoscope is paired with an image processing device that performs object detection and other analysis on captured images, and displays an interface to users that includes various visual guidance combined with endoscopic images. Interfaces displayed in different modes may indicate the location and extent of energy delivery tracked by the system, may guide placement of implant devices, may identify the location of nerves within tissue, and may provide step-by-step guidance for navigating the endoscope to certain anatomy. In some implementations, a removable and replaceable sheath may be coupled to the endoscope, and may enable one or more functional features of the endoscope while maintaining a sterile barrier. Sheaths may include an embedded memory that stores procedure configurations and procedure results.

A method of dilating a target anatomical structure of an ear, a nose, or a throat of a patient includes inserting a distal end of a balloon catheter into the ear, the nose, or the throat. The balloon catheter includes a balloon assembly coaxially disposed along a proximally extending shaft. The balloon assembly includes at least first and second dilation balloons that at least partially overlap one another. The method also includes distally advancing the distal end until the balloon assembly is disposed in the target anatomical structure. The method also includes selectively dilating the target anatomical structure based on a diameter of the target anatomical structure by inflating the first dilation balloon to a first diameter or inflating the second dilation balloon to a second diameter that is greater than the first diameter. The second inflation lumen is not in fluid communication with the first inflation lumen.
