A wound closure device includes a filamentary element having a proximal end and a distal end, and a plurality of barbs extending outwardly from the filamentary element. Each barb has a base connected with the filamentary element, a tip spaced from the base, and an outer edge that extends between the base and the tip. The outer edge includes a first section having a concave surface that extends between the base and a transition point of the barb and a second section having a convex surface that extends between the transition point of the barb and the tip of the barb. The outer edge transforms from the concave surface of the first section to the convex curve of the second section at the transition point of the barb. The tip of the barb has a convexly curved surface facing the distal end of the filamentary element. The unique dual radius shape of the outer edge of the barbs and the tips of the barbs minimizes resistance when the filamentary element is pulled in a first direction through tissue and maximizes resistance to movement when the filamentary element is pulled in an opposite, second direction through tissue for holding the wound closure device stationary.

Barbed sutures having contoured barbs that facilitate passage through tissue and increase holding strength

A single-armed suture assembly includes a needle, a flexible suture, and a flexible sheath. The flexible suture is coupled with the needle. The flexible suture includes first and second portions. The first portion includes a plurality of first barbs having first tips extending away from the longitudinal path in either an insertion direction or in a counter insertion direction that is opposite to the insertion direction. The second portion includes a plurality of second barbs having second tips extending opposite to the first tips. The flexible sheath is configured to prevent one of the first or second tips that extend in the insertion direction from contacting tissue as the flexible sheath is being advanced in the insertion direction. The other of the first or second tips that extend in the counter insertion direction is configured to contact tissue as the flexible sheath is being advanced in the insertion direction.

Barbed suture with removable sheath

A wound closure device having a filamentary element having a proximal end and a distal end, and a stop element coupled thereto and having a leading edge area defined by thickness and a width and having a total surface area. The leading edge area is substantially perpendicular to a longitudinal axis of the filamentary element. The device may have a ratio of the leading edge area to the total surface area that is less than 10%, a ratio of the length to the maximum thickness of the stop element that is at least 4, or a ratio of the length to the width of the stop element is at least 1 for any given thickness.

End effector for wound closure device

A suture device with a composite end effector, the composite end effector including a suture with an unmodified fixation element at a distal end and an overlying attachment piece disposed over the proximal end of the unmodified fixation element. The resulting composite end effector has improved strength and avoids the need for affixation methods such as welding or chemical affixation.

A method for approximating a wound using a uni-directional, barbed wound closure device having a needle and a stop element. The method includes taking a first pass by entering intact tissue at a location in proximity to the first apex and exiting in intact tissue at a location away from and above said first apex, pulling the wound closure device until the stop element is seated above the tissue plane, moving back toward the first apex and taking at least a second pass in a direction substantially perpendicular to the first pass at a location above or adjacent to the first apex, moving in a direction toward the second apex, approximating the wound using a continuous suturing pattern, moving toward the first apex, taking at least two passes across the wound, and cutting off a free end of the wound closure device in proximity to a surface of the tissue.

Welded end effector

A system for making braided barbed sutures includes a filament winding assembly, a barbed insert including an elongated core and barbs extending outwardly from the elongated core, and a barbed insert dispenser opening defining a passageway for orienting the barbed insert as the barbed insert is dispensed into the filament winding assembly. The barbed insert dispenser opening allows longitudinal movement of the barbed insert relative to the barbed insert dispenser opening while simultaneously preventing twisting movement of the barbed insert relative to the barbed insert dispenser opening.

Systems and methods for making braided barbed sutures

A surgical suture includes an elongated core having a first leg and a second leg, a first set of barbs projecting from the first leg of the core, and a second set of barbs projecting from the second leg of the core. The suture has a stop projecting from the core that is located between the first and second sets of barbs, and a braid surrounding the elongated core and the stop for strengthening the suture. The suture includes a pledget having first and second openings extending through the pledget, with the first leg extending through the first pledget opening, the second leg extending through the second pledget opening, and the stop extending between the first and second pledget openings.

Barbed sutures having pledget stoppers and methods therefor

A device for tensioning barbed sutures used for securing tissue or prosthetic devices includes a handle, and a shaft having a proximal end connected with the handle and a distal end remote therefrom. The distal end of the shaft includes a pressure applying surface for pressing against the tissue and the prosthetic devices. The tensioning device includes a pair of grooves extending through the shaft from the pressure applying surface at the distal end of the shaft toward the proximal end of the shaft. A barbed suture is tensioned by aligning a first section of the suture in the first groove and a second section of the suture in the second groove. After the first and second suture sections are aligned in the grooves, the proximal ends of the sections may be pulled for applying tension to the suture.

Devices for tensioning barbed sutures and methods therefor

Surgical sutures having a core extending along a length thereof between a first end and a second end, and a plurality of sets of first and second projections, wherein the first projections extend in a direction of a first end of the suture and the second projections extend in a direction of a second end of the suture. The first projections may be sized and shaped, and positioned relative to the second projections so as to be capable of substantially shielding the second projections when the surgical suture is drawn through tissue by the second end thereof. The first projections may additionally or alternatively be capable of overlapping and substantially covering a distal end of the respective second projections when the suture is drawn through tissue by the second end.

Bi-directional barbed suture

A system for making braided barbed sutures includes a filament winding assembly, and a guide assembly including at least one barbed insert dispenser opening defining a passageway for orienting a barbed insert. The guide assembly is adapted to dispense at least one barbed insert from the dispenser opening into the filament winding assembly for winding a plurality of filaments around the at least one barbed insert for making a braided barbed suture. The passageway of the dispenser opening is adapted to allow longitudinal movement of the barbed insert relative to the passageway while simultaneously preventing twisting movement of the barbed insert relative to the passageway. As the barbed insert is being dispensed, the barbed insert dispenser opening is selectively rotatable for imparting rotation to the barbed insert as the filaments are wound about the barbed insert. The passageway may be an elongated slit having a greater width than height.
