A venting catheter, system and method are provided for withdrawing blood and other fluids from a patient's heart to facilitate decompressing the heart during cardioplegic arrest and cardiopulmonary bypass, without the need for a thoracotomy and without puncturing the aorta, pulmonary artery, or heart itself. The venting catheter is configured to be introduced into a peripheral vein and intraluminally advanced through the right side of the heart and into the pulmonary artery. The venting catheter includes a lumen configured to withdraw blood at a rate of at least about 50 ml/min at a pressure of no less than about &minus;350 mmHg. A flow-directing means is provided to facilitate guiding the catheter into the pulmonary artery by being carried by blood flow through the heart. The cardiac venting system may include, in addition to the cardiac venting catheter, a cardiopulmonary bypass system to maintain circulation of oxygenated blood, and means for arresting the patient's heart.

Endovacular cardiac venting catheter and method

A surgical retractor has a frame and first and second retractor blades coupled to the frame. The retractor blades engage opposite sides of an incision in a patient's body and are relatively movable toward or away from each other along a first axis. When using the retractor in lift mode, a foot is coupled to the frame or one of the blades and engages the patient's body adjacent the incision. An actuator imparts relative movement to the retractor blades along the first axis and the foot acts as a support base with one of the blades moving relative to the frame to lift the ribs at one side of the incision above the other. To use the retractor in spread mode the foot is removed and the retractor blades spread of the ribs without lifting. The retractor may be used in various modes to facilitate a variety of surgical procedures, including, for example, harvesting the right or left internal mammary artery, repair or replacement of the mitral and aortic valves, proximal anastamosis of arterial conduits to the aorta, distal anastamosis of the conduits to coronary arteries, and any of various other procedures requiring access to the heart, great vessels, lungs, or other thoracic contents.

Endovascular cardiac venting catheter and method

Improved atherectomy catheters and methods of use are disclosed. The catheter includes a flexible body with a longitudinal passage therethrough. A housing is mounted at the distal end, with an elongated aperture formed in a lateral side. A drive cable is disposed in the passage of the catheter body, and a working element is disposed in the housing, attached to the distal end of the drive cable. Passages and openings are provided for flowing blood into the housing and for flowing blood from a proximal to a distal side of the cutting blade. An impeller can be provided proximal to the cutting blade to impel blood into the housing and past the blade. Blood flows through the housing and past the blade so as to perfuse tissue downstream from the device.

Catheter with perfusion system

Disclosed herein is a cutter capable of removing soft and hard tissue when used with an atherectomy catheter of the type having a cutter housing attached to the distal end of a catheter having a torque cable. A circular cutting blade is disposed within the housing and is secured to the distal end of a rotatable torque cable. An elongated aperture formed along one side of the hosing allows the intrusion of stenotic material which may then be severed by rotating and axially translating the cutting blade. The cutting blade may be made from a cemented tungsten carbide compound of 90 percent tungsten carbide (WC) and 10 percent cobalt (Co) which is heated, injection molded and sintered to produce a hard, durable cutting edge. The cutting edge is coated with titanium carbonitride using a physical vapor deposition process to further increase durability. Cutting edge hardness is further increased by application of an ion implantation of nitrogen using an ion-beam assisted deposition process. A variety of alternative cutter materials, coatings and geometries are defined which improve cutter performance in removing soft and hard lesions.
