Devices, systems and methods of neurostimulation for treating obstructive sleep apnea. The system is adapted to send an electrical signal from an implanted neurostimulator through a stimulation lead to a patient's nerve at an appropriate phase of the respiratory cycle based on input from a respiration sensing lead. External components are adapted for wireless communication with the neurostimulator. The neurostimulator is adapted to deliver therapeutic stimulation based on inputs.

Obstructive sleep apnea treatment devices, systems and methods

A system for restoring muscle function to the lumbar spine to treat low back pain is provided. The system may include one or more electrode leads coupled to an implantable pulse generator (IPG) and a tunneler system for subcutaneously implanting a proximal portion of the lead(s). The system may also include a handheld activator configured to transfer a stimulation command to the IPG, and an external programmer configured to transfer programming data to the IPG. The stimulation command directs the programmable controller to stimulate the tissue in accordance with the programming data. The system may include a software-based programming system run on a computer such that the treating physician may program and adjust stimulation parameters.

Systems and methods for restoring muscle function to the lumbar spine and kits for implanting the same

An apparatus for neuromuscular electrical stimulation is provided. The apparatus may be a stimulation lead having an elongated member made up of at least one conductor and an insulative sheath surrounding at least a portion of the conductor. A distal portion of the elongated member may include one or more electrodes and at least one fixation element to secure the one or more electrodes in or adjacent to a desired anatomical site for providing stimulation thereto. The stimulation lead has a strain relief portion on the proximal side of the one or more electrodes, configured to reduce axial forces on the distal region of the elongated member, and the effects thereof, to reduce the risk of, or even prevent, displacement of the one or more electrodes and to accommodate localized flexural motion. The apparatus also may include at least one fixation element sized and configured to be deployed between muscle layers to maintain the electrode position at the stimulation site.

Nerve cuff electrode including a tubular body having a longitudinal slit and a flap curled over the slit. The tubular body includes a central cathode disposed between two anodes. The region opposite the slit includes a flexible region that determines the flexibility and strength of tube opening and closing. The cuff electrode having a hinge region with a non-linear effective spring constant which can be higher at low cuff openings and lower at large opening to provide an effective yet non-damaging closing force over a wide range of cuff openings. In use, the tube body can be pulled apart using attached suture loops, with one loop and flap pulled under the nerve followed by part of the tubular body. The tubular body can be closed over the nerve and the flap closed over the tube slit.

A lead having an extendable and retractable fixation mechanism has a rotating terminal pin at the terminal end which rotates the fixation mechanism at the distal end. As the terminal pin is rotated, the fixation mechanism is extended or retracted from the distal end of the lead. A threaded collar allows for the fixation mechanism to smoothly extend and retract from the lead, and allows for a 1:1 turn ratio between the terminal pin and the fixation mechanism. A fluoroscopic ring disposed at the distal end of the lead provides information during the implantation process.

Nerve cuff electrodes and methods using nerve cuffs. Nerve cuff electrodes are provided which can include a tubular body having a longitudinal slit and a flap curled over the slit. The tubular body interior can have a central cathode formed of two opposed and electrically coupled plates disposed between two anodes each formed of two opposed and electrically coupled plates. The tube interior region opposite the slit can be free of electrode material, such that the flexibility of the polymeric tube significantly determines the flexibility and strength of tube opening and closing. Some cuffs include a hinge region having a non-linear effective spring constant which can be higher at low cuff openings and lower at large opening to provide an effective yet non-damaging closing force over a wide range of cuff openings. In use, the tube body can be pulled apart using attached suture loops, with one loop and flap pulled under the nerve followed by part of the tubular body. The tubular body can be closed over the nerve and the flap closed over the tube slit. The nerve cuff can be placed minimally invasively over the nerve and used for neuromodulation and/or sensing purposes.

A medical lead for tissue stimulation and sensing is provided, the lead having an electrode with an active fixation mechanism. The active fixation mechanism using a conductive spring element or wave washer to maintain electrical contact after extension of the active fixation mechanism. The active fixation mechanism is engaged by rotating a piston within the electrode housing which advances the fixation mechanism distally.

The insulative sleeve is coupled with the outer terminal ring with a snap-fit connection.
