An automatic method of categorizing the contact force of a catheter tip against a portion of a patient's heart based on motion of the catheter tip, the method comprising (a) capturing a series of 3D-coordinate data points of the catheter tip as a function of discrete times with a 3D medical imaging system, the 3D coordinates corresponding to an orthogonal 3-axis spatial coordinate system, (b) using a programmable computing system, computing a set of measures based on the series of 3D-coordinate data points, (c) categorizing each measure by a respective set of predetermined threshold values; and (d) combining the categorized measures to yield a relative quality of the contact force.

Cardiac catheter contact force determination

A method for automatically determining the 3D position and orientation of a radio-opaque medical object in a living body using single-plane fluoroscopy comprising: (a) capturing a stream of digitized 2D images from a single-plane fluoroscope; (b) detecting an image of the medical object in a subset of the digital 2D images; (c) applying to the digital 2D images calculations which preserve original pixel intensity values and permit statistical calculations thereon, using (i) multiple sequential determinations of a midline of the medical object image, (ii) a plurality of unfiltered raw-data cross-sectional intensity profiles perpendicular to each sequentially-determined midline, (iii) removal of outlier profiles from each plurality of profiles, and (iv) statistically combining each plurality of profiles to estimate image dimensions; (d) applying conical projection and radial elongation corrections to the image measurements; and (e) calculating the 3D position and orientation of the medical object from the corrected 2D image measurements.

Automatically determining 3D catheter location and orientation using 2D fluoroscopy only

A method for generating and displaying a 3D visualization of a cardiac-ablation balloon with a location marker and central catheter portion in a heart, the method using single-plane fluoroscopic images and comprising: placing, inflating and positioning the balloon; capturing a first burst of images from a first angle; capturing a burst of images from a second (different) angle; selecting an image from each burst to minimize cardio-respiratory phase differences therebetween; identifying the location marker in each image; placing first and second orientation markers in two images where the central catheter portion intersects a projected balloon image farthest from the location marker; associating the second-view location and orientation markers in the selected second-view image with the first-view location orientation markers; determining balloon 3D location and orientation of the balloon using the selected images and associated orientation markers; and inserting a balloon model into the 3D visualization for display.

Determining and displaying the 3D location and orientation of a cardiac-ablation balloon

An automatic method of determining local activation time (LAT) from at least three multi-channel cardiac electrogram signals including a mapping channel and a plurality of reference channels. The method comprises (a) storing the cardiac channel signals, (b) using the mapping-channel signal and a first reference-channel signal to compute LAT values at a plurality of mapping-channel locations, (c) monitoring the timing stability of the first reference-channel signal, and (d) if the timing stability of the monitored signal falls below a stability standard, using the signal of a second reference channel to determine LAT values. Substantial loss of LAT values is avoided in spite of loss of timing stability.

Time transformation of local activation times

A method for generating a 3D map of a cardiac parameter in a region of a living heart, the method using single-plane fluoroscopic images and comprising: (a) placing a plurality of catheters each having one or more radio-opaque sensors into the region such that the locations of the sensors geometrically span the region; (b) capturing a first-view digitized 2D image of the region from a first fluoroscope positioned at a first angle; (c) identifying each of the plurality of sensors in the first-view image; (d) capturing a second-view digitized 2D image of the region from a second fluoroscope positioned at a second angle which is different from the first angle; (e) identifying each of the plurality of sensors in the second-view image; (f) associating each of the plurality of identified sensors in the second-view image with its corresponding identified sensor in the first-view image; (g) sensing and storing values of the cardiac parameter with each of the plurality of sensors; (h) determining the 3D location of each of the plurality of sensors from the first-view and second-view images using back-projection calculations; (i) associating each of the parameter values with its corresponding sensor location; (j) generating the parameter map from the first-view and second-view images; and (k) displaying the parameter map on a display device.

Rapid 3D cardiac parameter mapping

A method for automatically determining the 3D position and orientation of a radio-opaque medical object in a living body using single-plane fluoroscopy comprising capturing a stream of digitized 2D images from a single-plane fluoroscope (), detecting the image of the medical object in a subset of the digital 2D images, applying pixel-level geometric calculations to measure the medical-object image, applying conical projection and radial elongation corrections () to the image measurements, and calculating the 3D position and orientation of the medical object from the corrected 2D image measurements.

An automatic method of determining local activation time (LAT) from at least four multi-channel cardiac electrogram signals including a ventricular channel, a mapping channel and a plurality of reference channels. The method comprises (a) storing the cardiac channel signals, (b) using the ventricular and mapping channel signals and a first reference channel signal to compute LAT values at a plurality of mapping-channel locations, (c) monitoring the timing stability of the first reference channel signal, and (d) if the timing stability of the monitored signal falls below a stability standard, using the signal of a second reference channel to determine LAT values. Substantial loss of LAT values is avoided in spite of loss of timing stability.

An automatic method of determining local activation time (LAT) in multi-channel cardiac electrogram signals including a ventricular channel, a reference channel and a mapping channel wherein selection of at least one of a reference-channel activation and a mapping-channel activation is based on one or more activation times in the ventricular channel.

Multi-channel cardiac measurements

An automatic method of measuring parameters of multi-channel cardiac electrogram signals including at least one ventricular channel and at least two other cardiac channels, the method comprising: (a) digitizing and filtering a ventricular-channel signal and a first other cardiac signal over a first preset time window to generate corresponding absolute-value velocity signals; (b) estimating a pulse interval in the ventricular absolute-value velocity signal; (c) autocorrelating the first other cardiac absolute-value velocity signal; (d) selecting a peak value of the autocorrelation based on ventricular pulse-interval estimates; and (e) setting the cycle length of the first other cardiac signal to the lag value of the selected peak in the autocorrelation. Local activation times are measured, and several multiple-channel configurations decrease measurement time, decrease the impact of signal degradation, and increase the amount of data generated during a procedure.

An automatic method of determining local activation time (LAT) in multi-channel cardiac electrogram signals including at least two cardiac channels, one of the cardiac channels being a mapping channel and another cardiac channel being a reference channel, the method comprising: (a) estimating a cycle length CL in the reference channel; (b) determining a mapping-channel fiducial time tin the mapping channel signal; (c) determining a plurality of reference-channel fiducial times t; (d) computing adjusted reference-channel fiducial times tby adding or subtracting multiples of cycle length CL to each time tsuch that each time tis within a time window of length CL surrounding mapping-channel fiducial time t; (e) computing a single final reference-channel fiducial time tfrom the plurality of adjusted reference-channel fiducial times t; and (f) computing the LAT as tminus t.

A method for 3D reconstruction of the positions of a catheter as it is moved within a human body, comprising: (a) ascertaining the 3D position of a point on a catheter for insertion into the body; (b) acquiring fixed-angle, single-plane fluoroscopic image data of the body and catheter; (c) transferring the image data and catheter-point position to a computer; (d) determining 2D image coordinates of the point on the catheter; (e) changing the insertion length of catheter by a measured amount; (f) acquiring additional single-plane fluoroscopic image data of the body and catheter from the same angle, transferring the length change and image data to the computer, and determining image coordinates of the point on the catheter; (g) computing the 3D position of the catheter point; and (h) repeating steps e-g. A 3D model is constructed by assembling the plural 3D positions of the catheter point.

3D model creation of anatomic structures using single-plane fluoroscopy

An imaging system for use in a medical intervention procedure is disclosed. A first image acquisition system is configured to produce a fluoroscopy image of an anatomical region. A second image acquisition system is configured to produce a 3D model of the anatomical region. An interventional tracking system, which includes a position indicator, is configured to maneuver within the anatomical region. A first anatomical reference system is common to both the first and the second image acquisition systems, and a second anatomical reference system is common to both the first image acquisition system and the interventional tracking system. A processing circuit configured to process executable instructions for registering the second image acquisition system with the first image acquisition system to define a first registration, registering the interventional tracking system with the first image acquisition system to define a second registration, and in response to the first and second registrations, registering the interventional tracking system with the second image acquisition system.

Method and apparatus for registering 3D models of anatomical regions of a heart and a tracking system with projection images of an interventional fluoroscopic system

A method for generating a registered image relative to a cardiac cycle and a respiratory cycle of a person is provided. The method includes generating a plurality of 2-D images of an anatomical region of the person. The method further includes generating a 3-D model of the anatomical region of the person. The 3-D model is associated with a predetermined phase of the cardiac cycle and a predetermined phase of the respiratory cycle. The method further includes selecting a first 2-D image from the plurality of 2-D images associated with the predetermined phase of the cardiac cycle and the predetermined phase of the respiratory cycle. The method further includes generating the registered image of the anatomical region utilizing the first 2-D image and the 3-D model. The method further includes storing the registered image in a memory device.

Method for generating a registered image relative to a cardiac cycle and a respiratory cycle of a person

A system and method for a medical intervention procedure within a cardiac chamber having an imaging system to obtain image data of the cardiac chamber and to create a 3D model from that image data, an interventional system to register the 3D model with a real-time image of the cardiac chamber and to display the 3D model, and an interventional tool positioned in the cardiac chamber to be displayed upon the interventional system and to be navigated in real-time over the registered 3D model. Preferably, the method and system also includes a storage medium to store the 3D model and wherein the interventional system receives the stored 3D model to register with the real-time image of the cardiac chamber.

Method and apparatus for medical intervention procedure planning and location and navigation of an intervention tool

Methods for displaying a location of a point of interest on a 3-D model of an anatomical region of a person are provided. In one exemplary embodiment, the method includes generating first and second 2-D images of the anatomical region utilizing an image acquisition system when an X-ray source of the image acquisition system is disposed at first and second positions, respectively, in a 3-D coordinate system of the image acquisition system. The method further includes selecting first and second points in the first and second 2-D images, respectively. The method further includes utilizing a triangulation technique utilizing the first and second points to determine a point of interest in a 3-D coordinate system of the 3-D model of the anatomical region.
