A wearable cardioverter defibrillator (WCD) system is adapted for use by an ambulatory patient who already has an implanted pacemaker. If implanted, such a pacemaker may occasionally be emitting, without the knowledge of the WCD system, pacing signals that may be adding pacing artifacts to the ECG signal. The WCD system may use multiple electrodes to sense ECG signals along multiple vectors that have channels. In embodiments, the WCD system identifies a preferred one of its available channels according to how easily the pacing artifacts are detected within its ECG signal. When the patient's parameters indicate that an alert criterion is met, the WCD system may attempt to read the ECG signal from the preferred channel first. This way the WCD system may be able to identify faster the pacing artifacts, and therefore analyze the ECG signal faster, with an opportunity to react faster to the alert criterion being met.

A wearable cardioverter defibrillator (WCD) including a support structure configured to be worn by an ambulatory patient, an energy storage module configured to store an electrical charge, a discharge circuit coupled to the energy storage module, electrodes configured to render an electrocardiogram (ECG) signal of the patient while the patient is wearing the support structure, a user interface configured to output an alarm in response to a noise alarm signal, and a processor. The processor is configured to receive the ECG signal, determine whether noise is present on the ECG signal, determine from the ECG signal whether a shock criterion is met, and cause the user interface to generate the noise alarm signal when the noise is present on the ECG signal and the shock criterion is met and not generate the noise alarm signal when noise is present on the ECG signal and the shock criterion is not met.

A Wearable Medical System (WMS) includes one or more pacing capabilities. The WMS may detect when the patient's heart rhythm starts to deteriorate, but not necessarily in a way that requires defibrillation. In particular, the WMS may detect bradycardia of one or more types, and then confirm the detection before pacing to treat the detected bradycardia.

Rhythm sensing during external pacing

In one embodiment, a method to monitor a patient's heart health is described. The method may include processing at least one electrocardiogram (ECG) signal and diagnosing an episode of nonsustained ventricular tachycardia (NSVT) based at least in part on the processing of the at least one ECG signal. In some embodiments, the NSVT episode may satisfy an NSVT time duration and a QRS criterion. The NSVT time duration may be between 5 seconds and 15 seconds. In some instances, the QRS criterion may be a temporary QRS template of two sequential incoming QRS complexes. In some embodiments, the method may determine a similarity between the temporary QRS template and at least two subsequent QRS complexes by calculating a feature correlation coefficient between the QRS template and the subsequent QRS complexes.

Detecting nonsustained ventricular tachycardia in a wearable cardioverter defibrillator

In one embodiment, a method to determine reliable electrocardiogram (ECG) signal is described. The method includes receiving at least one ECG signal for a period of time from a patient. The method also includes analyzing the at least one ECG signal to determine a first heart rate using a first method and analyzing the at least one ECG signal to determine a second heart rate using a second method different from the first method. The method includes comparing the first and second heart rates to each other and classifying the at least one ECG signal as reliable when a reliability threshold is satisfied.

Identifying reliable vectors

A wearable cardioverter defibrillator (WCD) comprises a plurality of electrocardiography (ECG) electrodes. An episode is opened responsive to a string of consecutive segments meeting one or more shock criteria, and a shockable rhythm is confirmed for the episode responsive to a subsequent string of consecutive segments meeting one or more confirmation criteria.

In one example, a cardiac monitoring system comprises a processor to receive a segment of an electrocardiogram (ECG) signal of a patient, and a memory to store the segment of the ECG. The processor is configured to identify QRS complexes in the segment of the ECG signal, generate a supraventricular (SV) template for SV complexes in the QRS complexes, identify SV complexes in the QRS complexes using the template, identify normal sinus rhythm (NSR) complexes in the segment of the ECG signal, obtain an atrial template for atrial waveforms in the NSR complexes, measure a range of a P-wave of the atrial waveforms from the NSR complexes, save the measured P-waves, and classify the identified SV complexes as either atrial fibrillation (AF) or supraventricular tachycardia (SVT) using the atrial template. Other examples and related methods are also disclosed herein.

A wearable cardioverter defibrillator (WCD) having a processor configured to receive a signal indicating a position and/or movement of the ambulatory patient while the ambulatory patient is wearing the support structure receive the ECG signal, determine from an ECG signal whether a shock criterion is met, determine a confirmation time period and/or a response time period based on the position and/or movement of the ambulatory patient, determine from the ECG signal whether the shock criterion is met after the confirmation time period has elapsed, cause the user interface to generate the shock alert signal based on the shock criterion determined after the confirmation time period has elapsed, and control the discharge circuit to discharge the stored electrical charge when a predetermined time period has elapsed after the shock alert signal.

In embodiments a wearable cardioverter defibrillator (WCD) system is worn by an ambulatory patient. The WCD system analyzes an ECG signal of the patient, to determine whether or not the patient should be given an electric shock to restart their heart. If the WCD system determines that such a shock should be given, then it also determines whether or not a High Frequency (H-F) noise criterion is met by the ECG signal. If that H-F noise criterion is not met, the patient can be shocked. If, however, that H-F noise criterion is met, then the WCD system can confirm before shocking, by sensing another portion of the ECG signal, analyzing again, and so on. Thanks to the confirmation before shocking, the possibility is diminished that the ECG signal will indicate that a shock is needed falsely, due to H-F noise. This can further reduce false patient alarms, and so on.

The method includes connecting to at least three sensing electrodes and obtaining a signal from each of the at least three sensing electrodes. The method also includes defining at least three channels between the at least three electrodes and obtaining heart data for a predetermined time period from the at least three channels wherein the heart data includes heart rate and QRS width. A Wearable Cardioverter Defibrillator (WCD) system has a processor that performs two different analyses to an ECG of the patient. A first-level analysis can be computationally economical, while a fuller second-level analysis can give shock/no-shock advice with more certainty. In some of these embodiments the second-level analysis of the ECG is performed only if the first-level analysis of the ECG detects a possible shockable condition. As such, the first-level analysis may operate as a gatekeeping function, often preventing the more computationally intensive second-level analysis from being performed. An advantage can be that the WCD system needs to store less charge, for powering the processor. In turn, this permits portions of the WCD system to be less bulky and weigh less.

A wearable medical includes a walking detector module with a motion sensor that is configured to detect when the patient is walking or running. In embodiments, a parameter (referred to herein as a 'Bouncy' parameter) is determined from Y-axis acceleration measurements. In some embodiments, the Bouncy parameter is a measurement of the AC component of the Y-axis accelerometer signal. This detection can be used by the medical device to determine how and/or whether to provide treatment to the patient wearing the medical device. For example, when used in a WCD, the walking detector can prevent “false alarms” because a walking patient is generally conscious and not in need of a shock.
