The present invention provides biodegradable polymer particle delivery compositions based on polymers, such as polyester amide (PEA) and polyester urethane (PEUR) polymers, that contain amino acids in the polymer. The polymer particle delivery compositions can be formulated as a liquid dispersion of polymer particles with the bioactive agents dispersed in the particle or conjugated attached to polymer molecules or particle surfaces. The bioactive agents can include drugs, polypeptides, DNA and cells for cell-based therapies using particles sized for local, mucosal or circulatory delivery. Methods of treating a disease by administering to a subject the polymer particle delivery composition, which incorporates a bioactive agent suitable for treatment of the disease, or its symptoms, are also included.

Polymer particle delivery compositions and methods of use

A method and apparatus for using a secondary refrigerant to precool and liquefy a primary refrigerant, then vaporizing and expanding the primary refrigerant to cool a cold tip of a cryosurgical instrument for ablation of biological tissue, such as cardiovascular tissue, in particular endocardiac tissue and tissue inside a cardiac blood vessel. The secondary refrigerant has a critical temperature above the critical temperature of the primary refrigerant, and a cooling temperature below the critical temperature of the primary refrigerant, thereby facilitating the use of the precooling step to provide liquid primary refrigerant in an operating room environment in which the primary refrigerant could not otherwise be provided in the liquid phase.

Precooled cryogenic ablation system

Apparatus and methods are provided for treating female urinary incontinence by injecting a bulking agent at the interface between the mucosal and submucosal layers of the urethral wall. The remodeling device comprises an elongated shaft preferably having means for forming a localized inward protrusion of the urethral wall, and a needle, movable between retracted and deployed positions, for delivering a bulking agent into the urethral wall in the vicinity of the localized inward protrusion to thereby reduce or eliminate symptoms associated with urinary incontinence.

Apparatus and methods for treating female urinary incontinence

Balloon catheters according to the present invention have surface contours with protrusions and grooves to make non-continuous lesions. There is a heating element inside the balloon. The balloon will be filled with a fluid before ablation to expand the balloon for tissue contact, and to transfer heating energy from the heating element to the tissue in contact with the balloon. Since the balloon surface has contours with grooves and protrusions, the tissue contact will not be continuous along the balloon surfaces, and therefore the ablation on the tissue will not be continuous. The energy transmitter can be ultrasound transducer, electrical resistant heater, and microwave antenna.

Balloon catheters for non-continuous lesions

The invention tissue injection catheters are designed to facilitate injection of a therapeutic or diagnostic agent to an interior body cavity, especially the myocardium or epicardium of the heart with precisely controlled depth penetration. The invention injection catheters, which include mechanisms for precisely controlling the length of needle tip exposed for tissue penetration, are designed to be used in an assemblage with a sterile container and delivery actuator for reinjection of autologous cell-containing fluids, such as autologous bone marrow aspirate. In preferred embodiments, the assemblage is designed to be hand-held. When used in such an assemblage, the invention catheters are particularly useful as a gene delivery device to effect angiogenesis of ischemic areas of the heart.

Flexible tissue injection catheters with controlled depth penetration

The invention provides systems containing a sterile container for aspiration, filtering, treating and reinjection into a subject of a bodily fluid in a sterile environment. The systems are particularly designed to facilitate transfection of cells within the sterile container by gene therapy molecules and reinjection of the transfected cells into the donor at controlled depth in precisely controlled volumes such as is useful for revascularization of ischemic cardiac tissue. For injection, a pressure actuator, preferably hand-held, applies force to liquids held within the sterile container in precisely controlled increments, thereby expressing liquids through a hollow needle or injection catheter in precisely controlled microvolumes. The invention systems include a hand-held injection device with audible cues that correspond to an operator-selected injection volume and/or audible cues that correspond to needle penetration depth selected by the operator.

Sterile aspiration/reinjection systems

A method and apparatus for inflating a balloon at a distal portion of an elongated delivery catheter to contact surrounding biological tissue, and expanding a refrigerant adjacent the balloon to cool the biological tissue to render it non-viable. The inflation of the balloon can be accomplished with the expanded refrigerant or with a separate pressurized fluid. The balloon can act as a heat transfer element, or there can be a separate heat transfer element on the catheter adjacent the balloon. The apparatus can be used to perform a dilation procedure, such as angioplasty, in conjunction with cooling of the surrounding tissue.

Balloon cryogenic catheter

An apparatus and method for automatic operation of a refrigeration system to provide refrigeration power to a catheter for tissue ablation or mapping. The primary refrigeration system can be open loop or closed loop, and a precool loop will typically be closed loop. Equipment and procedures are disclosed for bringing the system to the desired operational state, for controlling the operation by controlling refrigerant flow rate, for performing safety checks, and for achieving safe shutdown.

Control system for cryosurgery

Apparatus for remodeling in the urinary tract of a human female comprising a remodeling device having a shaft with a profile and with proximal and distal extremities. A handle is secured to the proximal extremity of the shaft. A member is mounted on the distal extremity of the shaft. A plurality of needle electrodes are carried by the distal extremity of the shaft and are disposed circumferentially of the shaft. A pusher is carried by the handle for moving the needle electrodes from a retracted position in which they are within the profile of the shaft and an extended position in which they extend sidewise beyond the profile of the shaft. A pump is connected to the handle for supplying a cooling liquid to the distal extremity of the shaft so that it exits from the shaft in the vicinity of the needle electrodes. An RF generator is coupled to the handle and to the needle electrodes for supplying radio frequency energy to the needle electrodes.

Apparatus and method for treating female urinary incontinence

The present invention provides a cryogenic device and system for treating biological tissue, which includes at least two media-flow lumens and a media-expansion element for cooling media flowing within the device. A cooling portion of the device is located along a portion of one of the media-flow lumens. The cooling portion is of a construction suitable for good contact with tissue selected for treatment. Preferably, the device and system employ one or more closed media-flow pathways for efficient recycling of the media employed. Preferably, the device and system also employ efficacious heat-exchange relationships between media-flow conduits for optimal cooling. The device and system are particularly designed for the formation of lesions in biological tissue, most particularly, the formation of deep, elongated and continuous lesions in cardiopulmonary tissue. The present invention also provides a method of using the cryogenic device and system in the treatment of biological tissue.

Cryogenic device, system and method of using same

An optimum gas mixture formulated from a group of component fluids, for use in a miniature mixed gas refrigeration system. The gas mixture has appropriate components, in appropriate concentrations, to optimize refrigeration power and heat transfer capacity, and to minimize plugging of the Joule-Thomson expansion element. The gas mixture is pressurized by a compressor to a pressure less than 750 psia, and preferably less than 420 psia, for safety reasons, and supplied to a heat exchanger. The high pressure outlet of the heat exchanger is connected to a Joule-Thomson expansion element where the high pressure gas is expanded isenthalpically to a lower temperature at least as low as 183K. This low temperature gas cools a heat transfer element mounted in the distal end of the probe, to cool an external object. Return gas flows back through the heat exchanger to pre-cool the incoming high pressure gas mixture.
