Systems and methods for operating switches of a power supply are disclosed. The method includes generating power with a power source and switchably coupling the power source to an output of the power supply. One or more switches are opened and closed to generate a voltage waveform at the output, and in addition, the one or more switches are opened and closed to reduce a temperature differential that occurs between a low temperature and a high temperature of each switch by increasing the low temperature of each switch.

System and method for prolonging the life of switching components

An implantable medical device can include a therapy circuit coupled to a therapy delivery terminal, the therapy circuit configured to generate a specified electrostimulation therapy for delivery to a tissue site via the therapy delivery terminal, and a measurement circuit for measuring at least two impedances of a first terminal combination including the therapy delivery terminal, the two impedances corresponding to at least two instances of excitation separated enough in time to capture an impedance artifact due at least in part to a motion of the heart, such as to determine an electrostimulation therapy lead status at least in part using the at least two impedances.

Lead fault detection for implantable medical device

Various approaches are described for managing refractory periods based on the detection of an evoked response. The heart is paced and an evoked response is detected. At least one refractory period is adapted based on the detected evoked response. Adapting the refractory period may involve, for example, initiating a refractory period, terminating a refractory period, extending a refractory period or modifying one or more parameters of the refractory period. Adapting the refractory period may involve modifying a sensing threshold based on the detected evoked response. The sensing threshold of an atrial sensing channel may be adjusted, for example, to avoid sensing a ventricular evoked response on the atrial channel while allowing sensing of a P-wave on the atrial channel.

Refractory period management based on detection of an evoked response

A method and system for generating a characterization of one beat of a patient's supraventricular rhythm (SVR) involves performing such characterization while the heart is being paced. During SVR characterization, various pacing parameters are modified and the patient's supraventricular rhythm is characterized while the pacing parameters are modified. The SVR characterization process is effective in single and multiple chamber pacing modes.

Method and system for characterizing supraventricular rhythm during cardiac pacing

Methods and systems are directed to detecting atrial tachyarrhythmia. A plurality of A-A intervals is detected. The detected A-A intervals are selected and used to detect atrial tachyarrhythmia. Selecting A-A intervals may be based on determining that A-A intervals are qualified. Qualified A-A intervals may be determined if a duration of the particular A-A interval falls outside a predetermined duration range, for example. Qualified A-A intervals may also be determined based on events occurring between consecutively sensed atrial events of the particular A-A interval, and whether the duration of the particular A-A interval falls within the predetermined duration range, for example.

Atrial tachyarrhythmia detection using selected atrial intervals

A method and system provides for generating a snapshot representative of one beat of a patient's normal cardiac rhythm. Cardiac rate channel signals and shock channel signals are sensed. A fiducial point is determined for a predefined number of the cardiac rate channel signals. A predefined number of the shock channel signals are aligned using the fiducial point. A template is generated using the aligned shock channel signals, whereby the template is representative of one of the patient's normal supra-ventricular conducted cardiac beats. The template is updated on a periodic basis.

Cardiac rhythm template generation system and method

A combination pacer/defibrillator is tailored for bradycardia patients. In one example, its shock-delivery specificity exceeds its sensitivity to shockable ventricular tachyarrhythmias. In another example, its specificity exceeds 95%, or 99%, or even 99.5%. Sensitivity is programmed to a high desired sensitivity value, but only if it can be done without decreasing the specificity below the desired specificity threshold value. This can be conceptualized as 'avoiding at all costs' delivering false shocks, even at the expense of failing to deliver a shock to a treatable ventricular tachyarrhythmia. Specificity enhancements include, among other things, inhibiting shock delivery when the patient is breathing or not supine, using multiple channels or a high rate VT/VF detection threshold. The present pacer/defibrillator device could potentially save the lives of bradyarrhythmia patients who are not presently clinically indicated for a defibrillator/pacer, but who have an increased risk of sudden cardiac death due to one or more risk factors.

Pacer with combined defibrillator tailored for bradycardia patients

A cross-chamber refractory period with an adaptively adjusted sensing threshold for use by a cardiac pacemaker is disclosed. In response to a cross-chamber event, the sensing threshold of a sensing channel is raised to an adaptively adjusted cross-chamber maximum refractory value. The cross-chamber maximum refractory value is derived from a peak amplitude measurement of the far-field signal resulting from the cross-chamber event.

Adaptive sensing threshold for cross-chamber refractory period

An atrial defibrillation system includes an implantable atrial defibrillator having an atrial defibrillation detector for detecting atrial fibrillation and a cardioverter for cardioverting the atria of a heart. The implantable atrial defibrillator further includes a receiver for receiving commands from an external nonimplanted transmitter. A nonimplantable communication device includes a transmitter for transmitting a command to the receiver. The command causes only the atrial fibrillation detector to be activated resulting only in the detection for atrial fibrillation without committing to atrial fibrillation cardioversion should atrial fibrillation be detected.
