Components for an endoscopic vessel harvesting system suitable for harvesting target vessels such as the saphenous vein or radial artery for cardiac artery bypass graft surgery. The main components of such systems include a vessel dissector and a vessel harvester, both of which work in conjunction with a separately provided endoscope. The vessel dissector is an elongated cannula having a blunt tip for separating layers of facial around vessels. The tip may be movable, and is typically transparent to permit viewing forward of the tip using the endoscope. Internal features of the tip may reduce glare back to the endoscope. Several devices improve visibility through the tip by reducing interference from tissue or fluid on the tip. The vessel harvester also has an elongated cannula for receiving the endoscope. Several tools within the harvester permit manipulation, severing, and sealing of vessels forward of the distal end. The tool for manipulating vessels may have a low-profile for increased visibility of operation, and may be coupled to the cannula with a damping mechanism to reduce the possibility of avulsion of the vessels. Various vessel cutting and sealing devices are provided that may accommodate various sizes of vessels and improve cutting and sealing efficacy.

Endoscopic vessel harvesting system components

A vessel dissector used in conjunction with an endoscope is provided with a lumen for receiving the endoscope; a blunt dissecting tip provided on a distal end of an elongated cannula permitting tissue dissection while viewing internal body structures via an objective lens of the endoscope; and a vessel severing tool extending through at least a portion of the elongated cannula and translatable longitudinally relative to the elongated cannula, and the vessel severing tool comprises an electrode-carrying member translatable circumferentially within an arcuate slot extending through an outer surface of the vessel dissector, wherein the arcuate slot comprises an inner radius, an outer radius and a central radius, wherein the outer radius is greater than the inner radius and the central radius is located between the outer radius and the inner radius so the arcuate slot provides a uniform opening through which the electrode-carrying member can extend and retract.

Components for an endoscopic vessel harvesting system include a vessel dissector and a vessel harvester. In this case, a vessel dissector is provided that is used in conjunction with an endoscope, and comprises: an elongated cannula including a lumen therein for receiving the endoscope; a blunt dissecting tip provided on a distal end of the elongated cannula that permits an operator of the vessel dissector to dissect tissue while viewing internal body structures via an objective lens of the endoscope; and a vessel severing tool extending through at least a portion of the elongated cannula, wherein the vessel severing tool is capable of translating longitudinally relative to the elongated cannula, and wherein the vessel severing tool comprises an electrode-carrying member that is capable of translating circumferentially within an arcuate slot extending through an outer surface of the vessel dissector.

A surgical apparatus and methods for severing and welding tissue, in particular blood vessels. The apparatus includes an elongated shaft having a pair of relatively movable jaws at a distal end thereof. A first heating element on one of the jaws is adapted to heat up to a first temperature and form a welded region within the tissue, while a second heating element on one of the jaws is adapted to heat up to a second temperature and sever the tissue within the welded region. The first and second heating elements may be provided on the same or opposite jaws. A control handle provided on the proximal end of the elongated shaft includes controls for opening and closing the jaws, and may include an actuator for sending current through the first and second heating elements. The first and second heating elements may be electrically connected in series, and the first heating element may be bifurcated such that it conducts about one half of the current as the second heating element. A force-limiting mechanism provided either within the control handle, in the elongated shaft, or at the jaws limits the pressure applied to the tissue by the jaws to ensure that the tissue is severed and the ends effectively welded within a short amount of time.

Apparatus and method for regulating tissue welder jaws

Forming a proximal anastomosis on an aortic wall includes method and instrumentation and apparatus for forming an aortic puncture and inserting into the vessel through the puncture a fluid-impervious sealing element with a protruding retainer. An anastomosis of a graft vessel over the puncture is partially completed with the retainer of the sealing element protruding through the partial anastomosis. The retainer facilitates removal of the sealing element from the partial anastomosis prior to completion of the procedure.

Temporary anastomotic seal and method

Surgical treatment of tissue includes electro cauterization of blood vessels interposed between spaced sets of electrodes of opposite polarity, and includes transection of such tissue by a cutter that is mounted between the spaced sets of electrodes for translational and lateral movement relative to the sets of electrodes. Orientations of the sets of electrodes within a range of angles about an elongated axis of a supporting body are controlled by manual movement of an actuator mounted near a proximal end of the body for movement through a smaller range of angles via linkage connecting the actuator to the electrodes. Tissue dissection with gas insufflation to form an anatomical space in tissue is facilitated by a fluid outlet port located near the tissue-dissecting tip at the distal end of the elongated body that delivers through the tissue-dissecting tip to the dissected tissue a fluid under pressure that is supplied along a lumen within the elongated body.

Apparatus and method for integrated vessel ligator and transector

Devices and methods are provided for stabilizing tissue, e.g., a beating heart, within a patient's body. In certain embodiments, the subject devices are characterized by having a shaft and at least two contacting members operatively associated with the shaft where the contacting members are capable of assuming a substantial coaxial alignment when in a low profile configuration and also capable of assuming an open, working configuration. In other embodiments, the devices are characterized by having a shaft and at least one tissue contacting member associated with the shaft using a ball and socket mechanism, wherein the at least one tissue contacting member may be caused to pitch and/or roll and/or yaw. In the subject methods, a subject device is advanced to the target site and stabilizes the target tissue using compression, negative pressure or both. The subject invention also includes kits for use in practicing the subject methods.

Tissue stabilizer and methods of using the same

Integrated vessel ligator and transector

Methods and delivery devices for maximizing injectate dispersion in lesioned tissue using needle-based injection devices are herein disclosed. The delivery devices include injection devices with various needle arrays and/or modified needle tip configurations. The needle tip configurations can include linear, hooked or corkscrew tips and/or multiple circumferential openings.

Needle array devices and methods

A tissue dissector and method for welding and severing blood vessels includes advancing the tip of a tissue-dissecting surgical instrument through tissue, with a tissue welder substantially concealed within the tip during tissue dissection. Blood vessels encountered in the tissue being dissected are selectively captivated and compressed in substantial contact with the tissue welder to elevate the temperature of the compressed tissue sufficiently to hemostatically weld and sever the compressed blood vessel.

Dissection and welding of tissue

A ring for use in anastomosis. Preferably, the ring is integrally formed from metal, and includes a ring portion and tines and docking members that extend from the ring portion. The ring portion and tines are malleable, and preferably also the docking members are malleable. The ring portion and tines are malleable in the sense that once deformed from a first shape into a second shape, they will not relax back into the first shape from the second. To install the ring in a vessel with the ring portion extending around an incision or other orifice, the tines pierce the tissue around the orifice and are curled against an anvil. The action of curling the tines inverts the tissue near the orifice edges to expose the inside surface of the vessel or organ. Other aspects of the invention are a method and apparatus for installing an anastomosis ring in an incision or other orifice in a vessel or other organ, a method and apparatus for precisely aligning two anastomosis rings (each installed in an incision or other orifice of a different organ) and fastening the aligned rings together. The clips can be crimped onto the aligned rings, or they can be spring clips which are sprung onto the aligned rings to clamp the rings together by spring force. Also within the scope of the invention are crimping and spring clips for use in listening together two aligned anastomosis rings. In other embodiments, the invention is a method for performing an anastomosis by installing an anastomosis ring in an orifice in an organ, installing another anastomosis ring in an orifice in another organ, precisely aligning the two installed anastomosis rings, and fastening the aligned rings together.
