Surgical elongate blade assembly with interchangeable inner member, kit and method relating thereto

A surgical micro-resecting instrument including an outer tubular member, an inner tubular member, a hub assembly, wiring, and an electrical insulator. The outer tubular member is formed of an electrically conductive material and defines a proximal section, a distal section forming a cutting window, and an internal lumen. The inner tubular member is disposed within the lumen and defines a proximal end and a distal end forming a cutting tip. The hub assembly maintains the outer tubular member and the inner tubular member. The wiring is permanently electrically connected to the proximal section of the outer tubular member with the electrical connection being encompassed by the hub assembly. The electrical insulator covers a region of the outer tubular member distal the hub assembly, at least the cutting window free of the insulator. In one preferred embodiment, the hub assembly includes an outer hub insert molded to the outer tubular member, and forms a generally radially extending shroud.

A flexible inner tubular member for being rotated in an outer tubular member of a rotary tissue cutting instrument includes an elongate inner tube having a rotatably driveable proximal end, a distal end having a cutting member exposed from the outer tubular member to cut anatomical tissue and a flexible region allowing the inner tubular member to rotate within the outer tubular member while conforming to the shape of the outer member. The flexible region is defined by a helical cut in the inner tube defining a plurality of integrally, unitarily connected tube segments angularly movable relative to one another and a spiral wrap disposed over the helical cut.

A laryngeal cutting blade has an outer blade assembly including a tubular member having a bend closer to a proximal portion than a distal end, a hollow inner blade assembly rotatably received in the outer blade assembly for mounting to a powered handpiece and having a flexible portion disposed adjacent the bend and a suction passage extending through the inner blade assembly to permit aspiration of cut tissue, the angled configuration of the laryngeal cutting blade and toothed and smooth profile cutting ends therefore are particularly effective for debulking large, firm lesions and for delicate shaving of superficial lesions of the vocal cords after removal of bulk mass.

An angled rotary tissue cutting instrument includes an outer member or blade, having a rigid outer tube with proximal and distal portions connected by a bend, and an inner member or blade rotatably disposed within the outer member and including an inner tube of integral one-piece construction having a spiral cut formed therein between proximal and distal ends thereof to define a flexible region adjacent the bend, and at least one layer of a spirally wound strip of material superimposed over the spiral cut portion of the inner tube. The spiral cut preferably extends to a cutting tip at the distal end of the inner tube so that the inner member can be bent closer to the distal end thereof to access difficult to reach areas of the head and neck, and other parts of the body.

Angled rotary tissue cutting instrument with flexible inner member

An endoscope lens cleaning system for use in removing surgical debris from the objective lens of an endoscope has a sheath adapted to receive the endoscope, carrying a control switch and having an open distal end in fluid communication with a proximally mounted luer lock for removably engaging the distal luer connection of a tubing set. The tubing set is connected to and engaged with a peristaltic pump including a microprocessor or controller adapted to respond to actuation of the control switch and execute software including program steps for operation of the pump. The pump controller is programmed to execute a scrub cycle each time the clinician actuates the control switch mounted on the preferably disposable sheath. Scrub cycles are executed in a sequence of steps including a forward or outflow motion of the pump to dispense a first selected volume of irrigation fluid through the sheath and over the endoscope objective lens; a brief pause follows, to allow any residual fluid pressure to dissipate, and a reverse or inflow motion of the pump next withdraws or retracts any residual fluid droplets from the endoscope objective lens and into the sheath. The pump controller is programmed to perform two normal scrub cycles followed by a clear or purge cycle wherein the forward portion of the cycle dispensing irrigation fluid over the endoscope objective lens is longer than usual, thereby flushing any residual body fluids, other surgical debris or air from the sheath and restoring optimal lens cleaning system performance. The finger activated control switch is preferably releasably mounted directly on a disposable sheath, as opposed to being incorporated in a foot pedal.

Method and apparatus for cleaning an endoscope lens

A dermabrasion instrument includes an outer member and an inner member rotatably disposed in the outer member with an abrading member of the inner member exposed from the outer member. An aspirating passage in the abrading member establishes communication with an aspirating channel of the inner member from externally of the abrading member so that debris from abrasion of the skin by the abrading member is evacuated through the inner member. The outer member may include a shield disposed over the abrading member to contain the debris. The instrument may include an irrigating port for discharging irrigating fluid adjacent the abrading member. A dermabrasion instrument assembly includes an abrading instrument releasably coupled to a powered surgical handpiece. The instrument has an aspirating channel and the handpiece has a suction port connectible with a source of suction to effect evacuation of debris through the abrading instrument and the handpiece. A method of dermabrasion includes the steps of rotating an inner member of an abrading instrument within an outer member of the instrument to rotate an abrading member of the inner member, abrading the skin with the rotating abrading member and aspirating debris from abrasion through the inner member.

Dermabrasion instrument, instrument assembly and method

A method of performing sinus surgery utilizes a sinus debrider instrument having an outer tubular member, an inner member rotatably disposed within the outer member and carrying a tissue cutting surface at a distal end thereof, a suction passage through the instrument for removing cut tissue from a sinus and an annular space between the inner and outer members forming a fluid passage for supplying fluid to the tissue cutting surface. The method includes the steps of positioning the distal end of the instrument at an operative site within the sinus, cutting tissue at the operative site within the sinus by rotating the tissue cutting surface, removing the cut tissue from the sinus through the suction passage and supplying fluid to the tissue cutting surface through the fluid passage to facilitate the removal of cut tissue without introducing fluid to the operative site.

Method of performing sinus surgery utilizing & sinus debrider instrument

A rhinoplasty bur for use with a powered handpiece includes an outer tubular member configured to form a pocket for receiving a bur and for protecting the bur during operation. In one embodiment, the pocket has an end wall extending from a bottom wall of the pocket to form a distal lip providing protection for distal movement of the bur. In another embodiment, with or without the lip, the pocket is formed with opposed, curved wings extending laterally from longitudinal edges of the pocket for protection and stabilization of the bur during operation.

The present invention is generally characterized in an angled rotary tissue cutting instrument including an outer blade assembly, having a rigid tubular member with proximal and distal portions connected by a bend, and an inner blade assembly rotatably disposed within the outer blade assembly and including a tubular drive shaft at a proximal end, a cutting tip at a distal end, and a flexible coupling disposed between the drive shaft and the cutting tip. The drive shaft and cutting tip include neck portions which are disposed telescopically within proximal and distal ends of the coupling. The flexible coupling is formed of a flexible polymeric material, and each of the neck portions includes a lateral opening defining a predetermined flow path for the polymeric material during fabrication so that the flexible polymeric coupling includes flow portions extending into the openings in the neck portions of the drive shaft and the cutting tip to form permanent, interlocking mechanical joints therewith capable of receiving and transmitting torque.
