An apparatus is provided for applying to a surface of mammalian tissue including soft, living tissue an initially fluent material and then activating the material by exposure to an energy source. The material may be a liquid capable of polymerization to a non-fluent state by exposure to actinic light. The device, and methods that may be practiced in association with the device, enable a wide range of medical conditions to be treated including, for example, the application of a barrier to soft tissue to prevent post-surgical adhesions.

A purse string suturing instrument is provided for quickly and effectively forming a purse string suture in tubular tissue through relatively small cannulas placed in the body. The instrument includes a pair of elongated jaws, each having a row of spaced-apart uniform-size teeth. The jaws are hinged together at one end to clamp the tubular tissue therebetween. The two rows of teeth are offset with respect to each other so that they mesh when the jaws are closed. The jaws are inserted through a cannula and positioned to close around the tissue to be sutured. The rows of teeth then bend the tissue between them into a wavelike configuration. A pair of needles, the ends of each being connected to a single length of suture, is then inserted through passageways provided through the teeth. An encircling series of stitches are thus formed in the wall of the tubular tissue to create a purse string suture. The needles and the suturing instrument are then withdrawn through the cannula. The tubular tissue can be snugly fitted over an anastomosis ring or the like by drawing and tying the purse string suture.

An endoscopic insertion device for inserting an anastomosis ring having two unitary members in an anatomic tubular body member comprises an anastomosis ring adapter for mounting the unitary members in an engaging position, an inserter for inserting the ring adapter in the tubular body member, and an actuator for actuating the adapter to close the anastomosis ring. A method of using the endoscopic insertion device includes the steps of inserting the anastomosis ring in an engaged position into the tubular body member, securing opposite ends of the tubular body member to the engaged anastomosis ring, applying a biasing force to close the anastomosis ring, releasing the biasing force, and axially withdrawing the insertion device from the closed anastomosis ring.

A two-part ligation clip applicator comprises a handle assembly having a squeezable trigger and an elongated shaft connected at its proximal end to the handle assembly. The elongated shaft houses a sliding advancer which is actuated by squeezing the trigger to move axially in a forward direction in the shaft. In addition, a module is mountable at a distal end of the elongated shaft and supports a ligation clip. The module has a cartridge housing, latches pivotably mounted within the housing for gripping first part of the ligation clip, and a sliding pusher slidable within the housing by actuation of the advancer to engage the second part of the ligation clip and advance it relative to the first part to close the first part.
