In some embodiments, the inventive subject matter is directed to a multimode electrosurgical system having an electrosurgical instrument with a first electrode configuration that operates in a bipolar mode and second electrode configuration that operates in a monopolar mode. In other embodiments, the inventive subject matter is directed to an instrument and system that provide multiple modes of surgical and/or therapeutic treatments, at least one being an electrosurgical mode of treatment, the instrument including at least one active electrode on a working portion, and the working portion including or supporting at least one non-electrosurgical functional element, the instrument including at least one operational return electrode configuration of selectively variable surface area.

Surgical instruments and systems with multimodes of treatments and electrosurgical operation

An electrosurgical device can have a housing and an electrode defining an energizable surface at least partially positioned externally of the housing. The electrode can move longitudinally relative to the housing. A shank of the electrode can be positioned within the housing and configured to urge against an energizable element in the housing. The energizable element can have a longitudinally movable electrical coupler configured to electrically couple with the shank portion when the shank portion urges against the energizable element. Related methods are also disclosed.

Electrosurgical systems and methods

An electronic imaging device () includes an optical objective () for collecting optical radiation from an object, the objective having an optical axis, and an image sensor (), including a matrix of optical detectors arranged in a plane that is substantially non-perpendicular to the optical axis, the image sensor having a lateral dimension in the plane. A turning mirror () has an optical surface that is positioned so as to reflect the radiation collected by the objective in order to form a focused image in the plane of the image sensor, while a maximum distance from the optical surface to the plane of the image sensor is substantially less than the lateral dimension of the image sensor.

An optical probe, for example, a medical endoscope, having a removable lens cell. In an embodiment, the optical probe has an elongated tube with a proximal portion terminating at a proximal end and a distal portion opposite the proximal portion terminating at a distal end. The elongated tube defines an axial lumen wherein a lens cell holder is disposed proximal the distal portion. The lens cell holder has an axial holder bore for at least partially containing the lens cell within and removably coupled with the holder bore.

A device for morcellating tissue within a body cavity of a patient comprises a stationary tube () having a distal end portion (), and a bipolar electrosurgical electrode assembly () located at the distal end of the tube. The electrosurgical electrode assembly () comprises first and second electrodes () separated by an insulating member (), the bipolar electrosurgical electrode assembly extending around the circumference of the distal edge of the tube (). When a pulsed electrosurgical cutting voltage is applied to the electrode assembly () and relative movement is initiated between the tube () and the tissue, a core of severed tissue is formed within the tube such that it can be removed from the body cavity of the patient. Temperature sensors () can be used to measure the temperature at the distal end of the tube (), and to vary the duty cycle of the pulsed cutting voltage.

Surgical instrument

Apparatus for performing a medical procedure includes an invasive probe having opposite distal and proximal ends. The probe includes a transmitter, which is arranged to transmit an energy field, and a receiver, which is arranged to receive the energy field, wherein the transmitter and the receiver are disposed at the opposite ends of the probe. A control unit is adapted to determine an orientation of the distal end relative to the proximal end responsively to the energy field received by the receiver.

A medical instrument comprising a Veress needle, a dilating obturator, and a cannula. The Veress needle has a tubular safety obturator with an opening in its front section at a needle tip of the Veress needle. A guide with a guide slot can be positioned in a patient's vagina against the posterior fornix of the vagina to guide the Veress needle through the vaginal and peritoneal walls. A guide wire may be passed through the Veress needle and through the hole in the peritoneum wall formed by the Veress needle to guide the dilating obturator through the hole.

A valved device is provided for controlling flow into or out from a surgical patient. It includes a body defining a flow passageway and a valve connected to the body in flow communication with the body's passageway. The valve includes a barrel and a piston extending into the barrel. Means is provided for displacing the piston within the barrel along the barrel's axis to open a flow path that is preferably adapted for liquid suction. Means is also provided for rotating the piston within the barrel around the barrel's axis to open a flow path that is preferably adapted for smoke or gas evacuation.

A video signal compensator and method for compensating for differential picture brightness of an optical image due to uneven illumination is shown. The video signal compensator includes a device for generating a compensating signal substantially representing at least one parameter of a compensating waveform required to produce a video signal representing an optical image having a substantially uniform brightness. An adder operatively coupled to the compensating signal generating device and a video signal adds the compensating signal and the video signal to produce a compensating video signal used as an input to a video signal processor adjusting its gain both vertically and horizontally compensated by increasing the gain of the video signal representing that part of the optical image which is less bright than a reference and reducing the gain of the video signal representing that part of the optical image which is brighter than a reference compensating the video signal to represent an optical image having a substantially uniform brightness. The compensating signal can be generated by either an analog signal generating device or a digital signal processing device.

A medical instrument for ligation of tissue and anatomical structures, including fallopian tubes comprises at least two slideable shafts concentrically disposed relative to each other. A first shaft has a tissue receiving recess into which tissue is retracted by a grasper. The grasper is disposed on the end of a second shaft that is extendible from the first shaft. A ring retainer on the second or a third shaft can hold one or more elastic rings that are displaceable by the instrument around tissue extending from the tissue receiving recess. The rings are held on a tapered or necked down part of the instrument so that the rings contribute little or nothing to the largest outer diameter (OD) of the portion of the instrument that is inserted into a patient's body. Because the ring or rings do not contribute to the OD of the instrument, the instrument may be constructed to fit within insertion channels of 5 mm or less.

Apparatus and method for surgical ligation

A pivotable handle assembly for an endoscope such as an operative laparoscope having a proximal end is shown. The handle assembly includes a handle support operatively connected to the proximal end of a laparoscope. A handle including a mounting end is operatively connected by a member to the handle support. The mounting end is pivotally attached to the handle support enabling the handle to be pivoted relative to the operative laparoscope. In the preferred embodiment, the operating laparoscope is a video operating laparoscope and includes an image transferring member for producing an optical image at the proximal end of the laparoscope. A video camera is operatively connected to the proximal end to receive the optical image. A method for using the video operating laparoscope with a pivotable handle assembly in a surgical procedure is also shown.

An endoscope having a distally heated distal lens for performing laparoscopic surgery is shown. The preferred embodiment of the endoscope is a laparoscope which includes a rigid elongated sheath tube which encloses means defining a fiber optic light carrying bundle formed of a plurality of optical fibers each of which have a distal end. The fiber optic bundle has a proximal end which is adapted to be operatively coupled to a light source having light energy including infrared radiation and a distal end which is located in the distal section of the sheath tube contiguous the distal lens. The fiber optic bundle has a selected number of the distal ends of the plurality of optical fibers treated to form an energy diverting member for directing thermal or infrared energy from a light source into the periphery of the distal lens to increase the temperature thereof to resist fogging of the distal lens when the distal section of the endoscope is inserted into a warm, moist area such as a body cavity. The laparoscope may include a fluid flow channel which terminates in a nozzle located at the distal tip for directing a fluid flow across the distal lens to remove image impeding material therefrom. The fluid is usually at a cooler temperature and the application of the cooler fluid to the distal ends necessitates the need for continuous heating of the distal end.
