A target volume of ocular tissue is treated with a laser having a direction of propagation toward the target volume, where the target volume is characterized by a distal extent, a proximal extent, and a lateral extent. A layer of tissue at an initial depth corresponding to the distal extent of the target volume is initially photodisrupted using a femtosecond laser by scanning the laser in multiple directions defining an initial treatment plane. Tissue at one or more subsequent depths between the distal extent of the target volume and the proximal extent of the target volume is subsequently photodisrupted using a femtosecond laser by moving a focus of the laser in a direction opposite the direction of propagation of the laser and then scanning the laser in multiple directions defining a subsequent treatment plane. Photodisruption is repeated at different subsequent depths until tissue at the proximal extent of the target volume is photodisrupted.

Surgical system procedure for treatment of the trabecular meshwork and Schlemm's canal using a femtosecond laser

A target volume of ocular tissue of an irido-corneal angle of an eye is treated by moving a focus of a laser through the target volume of ocular tissue, and photodisrupting the target volume of ocular tissue at a plurality of spots as the focus is moved through the target volume of ocular tissue. The focus is moved by transverse scanning the focus between at least one of: a first circumferential boundary and a second circumferential boundary of the target volume of ocular tissue, and a first azimuthal boundary and a second azimuthal boundary of the target volume of ocular tissue, and axial scanning the focus between a distal extent and a proximal extent of the target volume of ocular tissue.

System and method for clearing an obstruction from the path of a surgical laser

A target surface in an eye is located using a dual aiming beam apparatus that transmits a first aiming beam of light and a second aiming beam of light. An optics subsystem receives a laser beam from a laser source, the first aiming beam of light, and the second aiming beam of light, and directs the beams of light to be incident with the target surface and aligns the beams of light such that they intersect at a point corresponding to a focus of the laser beam. An imaging apparatus captures an image of the target surface including a first spot corresponding to the first aiming beam of light and a second spot corresponding to a second aiming beam of light. A separation between the spots indicates that the focus is away from the target surface, while overlapping spots indicate the focus is at or on the target surface.

System and method for locating a surface of ocular tissue for glaucoma surgery based on dual aiming beams

Intraocular pressure in an eye is reduced by delivering a high resolution optical coherence tomography (OCT) beam and a high resolution laser beam through the cornea, and the anterior chamber into the irido-corneal angle along an angled beam path. The OCT beam provides OCT imaging for surgery planning and monitoring, while the laser beam is configured to modify tissue or affect ocular fluid by photo-disruptive interaction. In one implementation, a volume of ocular tissue within an outflow pathway in the irido-corneal angle is modified to create a channel opening in one or more layers of the trabecular meshwork. In another implementation, a volume of fluid in the Schlemm's canal is affected by the laser to bring about a pneumatic expansion of the canal. In either implementation, resistance to aqueous flow through the eye is reduced.

A look-up table for use in determining an energy parameter for photodisrupting ocular tissue with a laser is generated by determining a plurality of individual spot size distributions, wherein each of the plurality of individual spot size distributions is based on a different set of simulated data and includes an expected spot size of a laser focus at each of a plurality of locations within a modeled target volume of ocular tissue. The plurality of individual spot size distributions are combined to obtain a final spot size distribution that includes a final expected spot size of the laser focus at the plurality of locations of the focus within the modeled target volume of ocular tissue. An energy value is assigned to the plurality of locations of the focus within the modeled target volume of ocular tissue based on the final expected spot size at that location.

System and method for laser treatment of ocular tissue based on patient biometric data and apparatus and method for determining laser energy based on an anatomical model

A method of imaging and treating ocular tissue of an eye having a cornea, an iris, an anterior chamber, an irido-corneal angle, and a direction of view includes establishing a common optical path through the cornea and the anterior chamber into the irido-corneal angle for each of an optical coherence tomography (OCT) beam and a laser beam, where the common optical path is offset from an optical axis within the direction of view. The method also includes obtaining a circumferential OCT image of the irido-corneal angle, obtaining an azimuthal OCT image of the irido-corneal angle, and determining a treatment pattern for a volume of ocular tissue of the irido-corneal angle based on the circumferential OCT image and the azimuthal OCT image. The method further includes delivering optical energy through the laser beam in accordance with the treatment pattern.

Method, system, and apparatus for imaging and surgical scanning of the irido-corneal angle for laser surgery of glaucoma

A device for visualizing an irido-corneal angle of an eye through a window of a patient interface configured to be placed on the eye includes and optics structure and at least one imaging apparatus. The optics structure is configured to engage with the patient interface to provide a line of sight through the window in the direction of the irido-corneal angle, and to subsequently disengage from the patient interface. The imaging apparatus is associated with the optics structure and aligned with the line of sight to enable capturing an image of the eye including the irido-corneal angle.

A first optical subsystem includes a window with a refractive index nand an exit lens having a refractive index n. An ophthalmic laser surgical system includes a pulsed laser source configured to generate a pulsed laser beam, optics configured to direct the laser beam towards a target region in a lens of an eye, and a processor configured to control the optics to form a regular array of cells in the target region by creating layers of photodisrupted bubbles to generate cell boundaries. The layers are created by causing the optics to scan the pulsed laser according to a curvature of a focal plane of the optics to track a natural curvature of the lens.

An initial treatment pattern defining an initial volume of ocular tissue to be modified for treating glaucoma is designed. An initial laser treatment is delivered by scanning a laser beam across ocular tissue at an initial placement in the eye in accordance with the initial treatment pattern to thereby photo disrupt the initial volume of ocular tissue. A postoperative measure of intraocular pressure (IOP) is evaluated relative to an IOP criterion to determine if the treatment was successful. If the treatment was not successful, meaning the IOP criterion was not satisfied, then a subsequent treatment pattern that defines a subsequent volume of ocular tissue to be modified, and/or a subsequent placement in the eye is determined. A subsequent laser treatment is delivered by scanning a laser beam across ocular tissue at the subsequent placement within the eye in accordance with the subsequent treatment pattern to thereby photo disrupt the subsequent volume of ocular tissue.
