A bioprosthetic heart valve includes an outer frame configured to attach to a tissue annulus of a heart and an inner bioprosthetic valve having an exterior shape which substantially matches an interior shape of the outer frame. The exterior shape of the inner valve mates in substantial alignment with the interior shape of the outer frame. An outer frame-inner valve attachment mechanism is configured to couple the inner valve to the outer frame.

Improved sizers and markers include a cylindrical section and a handle extending from the cylindrical section. The cylindrical section has a diameter approximately equal to the diameter of an aortic heart valve prosthesis. The cylindrical section can include a marking element configured to mark tissue at fixed positions relative to the cylindrical section. Marking can be performed with a variety of techniques.

A valve prosthesis includes a plurality of flexible leaflets and a stent having a central lumen. A slit in the stent extends from the central lumen to an outer surface. An occluding portion of the flexible leaflets extends across the central lumen and an attachment portion extends from the central lumen through the slit and forms a sewing cuff for attachment to a patient's tissue. The attachment portion forms a sewing cuff for attachment to native heart tissue.

A driver tool drives multiple fasteners simultaneously through the outer ring of a heart valve component into the surrounding tissue annulus of a heart. The driver tool includes two stacked satellite gear layers that drive flexible shafts that simultaneously drive the fasteners into tissue surrounding the heart valve component. The flexible shafts from the second drive gear layer can pass through gaps between satellite gears in the first drive gear layer. The number of fasteners is increased while maintaining the mechanical advantage of the tool and the size of the tool, and reducing the concentration of stress in the tissue annulus.

Improved tools for visualizing, sizing and/or marking include a prosthesis template with a diameter approximately equal to the diameter of an aortic heart valve prosthesis. In preferred embodiments, the prosthesis template has an annulus end and a sinotubular junction end wherein the diameter at the annulus end is less than the diameter at the sinotubular junction end. The prosthesis template can include a marking element configured to mark tissue at particular positions relative to the prosthesis template. Marking can be performed with a variety of techniques.

A prosthetic heart valve for implantation in a heart includes an outer orifice ring for coupling to a tissue annulus of a heart. An inner orifice ring includes at least one leaflet occluder carried in a lumen of the inner orifice ring which is movable between an open position, which allows blood flow through the lumen, and a closed position which blocks blood flow through the lumen. The inner orifice ring is adapted to be coupled to the outer orifice ring after the outer orifice ring has been attached to the tissue annulus.

A driver tool which drives helical fasteners through a heart valve component into tissue. The tool has a tool housing with a distal end couplable to engage the implanted component. A drive shaft at the proximal end of the driver tool couples to a driving force. Multiple driver tips couple to helical fasteners for the heart valve component. A drive train in the tool housing distributes the drive to the driver tips.
