A spinal screw assembly is provided. The assembly may be provided with a screw and a movable head in which angulation of the screw relative to the movable head is constrained so that angulation within limits is allowed in one plane but lesser or no angulation is allowed in another plane. Methods to assemble the disclosed apparatus are also disclosed.

A screw for use in spinal surgery provided with a screw shaft and a screw head and at least one extension tab extending proximally from the screw head, the extension tab being defined relative to the screw head by a stress concentration feature. At a proximal end of the extension tab, the extension tab may be narrower in a circumferential direction than the extension tab at a distal portion thereof. A particular shape of stress concentration may be provided.

Embodiments are disclosed which provide a spinal cage with a wall extending in a longitudinal direction defining an interior space. There is also provided a spin-plate that may be assembled into the spinal cage. The spin-plate may comprise a blade and a disc on a common shaft, and may be rotatable from an undeployed position to a deployed position. In some embodiments, the spin-plate may be assembled into the spinal cage by a translational motion perpendicular to a rotational axis of the spin-plate. The spin-plate may be captured in the spinal cage for certain angular positions. In some embodiments, there may be a snap-in feature when the spin-plate is assembled into the spinal cage, and there may be a rotational feature such that increased torque is required to advance beyond certain points.

Various embodiments are provided with a polyaxial spinal screw assembly that comprise a threaded screw having a bulbous or curvate head, and a receiver for receiving the head of the screw, and a slotted collet between the receiver and the screw head. The collet may, in an un-locked condition, bear against the head of the screw to frictionally retain the screw's angular position. The collet may have a lip that may engage with a corresponding internal feature of the receiver. The screw head may be able to be inserted into the receiver through the bottom of the receiver. The screw may be cannulated and also may be fenestrated. Embodiments may also include a bone screw having fenestration holes that include a component in the rotational direction, such as backward-facing with respect to the forward-rotational direction of the screw.
