A vial adaptor () may comprise a body portion (). The body portion includes a vial connection port (), a syringe connection port (), an access passageway () between the vial connection port and the syringe connection port, and a regulation passageway (). The vial adaptor may further comprise an expandable and/or contractible chamber () impermeable to gas and/or liquid, the regulation passageway being between the vial connection port and the chamber, and an expandable housing () casing the chamber. Such a vial adaptor constitutes an improved vial adaptor.

A luer lock adaptor including a housing defining an axis, an internal luer lock element having a threading, the internal luer lock element being located internally of the housing and being rotatably mounted thereto for rotation about the axis relative to the housing, in a manner which permits rotation of the luer lock element relative to the housing in a first rotation direction about the axis and limits rotation of the luer lock element relative to the housing in a second rotation direction about the axis, opposite to the first rotation direction, whereby the location of the internal luer lock element internally of the housing prevents manual access to the internal luer lock element for limiting rotation thereof in the first rotation direction.

A syringe adaptor includes a body, shaped to define a body lumen, a syringe fitting at a proximal end of the body, configured to connect to a distal end of a syringe, a needle extending distally from the syringe fitting into the body lumen, a septum housing, and an elastomeric septum mounted inside the septum housing. The septum includes a proximal face, shaped to define an aperture having an aperture diameter, and a distal face. The septum is shaped to define a cavity, joined to the aperture, having a cavity diameter that is larger than the aperture diameter. The septum housing is configured to slide proximally within the body lumen such that a distal end of the needle contained in the cavity passes through the distal face of the septum. Apparatus for use in a drug mixing system including a vial adapter assembly including a main body element having a vial receiving portion and a needle puncturable port, the main body element including an axial hollow tubular portion which is in fluid flow engagement with a bore of a vial puncturing spike, the main body element further including a membrane support surface that supports a first membrane which is in fluid flow engagement with the vial puncturing spike via the bore and via a recess formed in an intermediate portion of the main body element, and a second membrane supported by a membrane support member and separated by a gap from the first membrane.
