Aspects of the present disclosure are presented for a single surgical instrument configured for grasping tissue, performing sealing procedures using electrosurgical or ultrasonic energy, suctioning fluid, and providing irrigation. An end effector of the surgical instrument may include multiple members arranged in various configurations to collectively perform the aforementioned functions. The suction and irrigation elements may comprise one or more fluid paths configured to deliver fluid to or evacuate fluid from a surgical field. In this way, a user, such as a clinician or surgeon, may rely on a single surgical instrument to perform these tasks typical in surgery while having an extra hand available and so as to not need to divert his or her concentration away from the surgical site in order to access multiple devices. Further, the timing for performing each of the functions may be made quicker, due to not needing to switch using multiple devices.

Suction and irrigation sealing grasper

A surgical instrument includes an anvil selectively coupleable to a stapling head assembly and a trigger operable to fire staples into tissue compressed between the anvil and the stapling head assembly. In some versions, a lockout member may engage a securing feature to prevent actuation of the anvil relative to the stapling head assembly. For instance, a tab may engage a slot on an actuator, a screen door lock may provide frictional resistance or engage teeth on the actuator, a door may actuate into engagement with one or more recesses, geared teeth may mesh with teeth on the actuator, the lockout member may include a ratcheting assembly to engage actuator, and/or a push button may actuate into a recess while disengaging the lockout member. Alternatively, in some versions, the trigger actuation assembly may be disengaged prior to firing. An anvil position indicator may restrict engagement of the trigger actuation assembly.

Feature to reengage safety switch of tissue stapler

An apparatus includes a staple cartridge. The staple cartridge includes staples, a deck, and a buttress. The deck defines a plurality of openings. Each opening of the plurality of openings is associated with a corresponding staple of the plurality of staples, such that each staple is configured to pass through a corresponding opening of the plurality of openings. The buttress is configured to detach from the staple deck in response to the staples being driven through the corresponding openings of the deck. The buttress is resiliently biased to transition from a first configuration while attached to the deck to a second configuration when detached from the deck. The buttress has a first length in the first configuration and a second length in the second configuration. The first length is longer than the second length.

Apparatus and method for pleating a bodily lumen

A surgical instrument includes a handle assembly having a trigger operable to fire a staple driver to staple tissue. The instrument includes a pointed rod to which an anvil may be coupled. An anvil detection feature is included to determine when the anvil is coupled to the rod. In some versions, the anvil detection feature comprises a translatable rod that inhibits a lockout feature from disengaging. In other versions, an anvil sensing tube is disposed about the pointed rod and interferes with actuation of the trigger in a first position. A recess in the tube permits trigger to actuate when the anvil sensing tube is in the second position. Alternatively, a resilient tab is coupled to the pointed rod and resists actuation of the staple driver. A trigger lockout assembly may include a spring-loaded button that 'pops' out when a push rod is actuated, thereby freeing a pivotable lockout feature.

A surgical instrument is disclosed. The surgical instrument includes an actuation system, a drive system, and a tool head. The drive system is coupled to the actuation system and includes a rotatable shaft which defines a longitudinal axis. The tool head is coupled to the drive system and includes an annular cartridge and a pliable bunchable ring. The annular cartridge includes a plurality of staples. The pliable bunchable ring surrounds the annular cartridge. The surgical instrument is configured to drive the staples through the pliable bunchable ring substantially transverse to the longitudinal axis.

A surgical system comprises an end effector for operating on tissue and a screen positioned around the end effector. The screen is fluidly coupled with a suction source by a conduit such as a tube. The screen is configured to communicate suction through the screen while preventing tissue from contacting the end effector. The screen is further configured to retract proximally relative to the end effector to thereby expose the end effector.

A medical device includes an end effector configured to apply bipolar energy to target tissue along a working portion thereof and a fluid control system to control the flow of a fluid produced when the end effector applies the bipolar energy to heat the target tissue. The fluid control system includes a fluid path element defining a fluid path, a distal fluid port configured to intake the fluid adjacent to the working portion of an end effector for transport through the fluid path, and a proximal fluid port configured to intake the fluid transported through the fluid path and to exhaust the transported fluid.

A surgical stapling instrument for performing a circular anastomosis comprises a stapling head assembly, an actuator handle assembly, a shaft assembly, a safety latch, and a locking member. The stapling head assembly includes an anvil that moves relative to a staple holder and a staple driver to drive staples from the staple holder into tissue and against the anvil. The actuator handle assembly has a first actuator that controls motion of the anvil and a second actuator that controls motion of the staple driver. The shaft assembly couples the stapling head assembly to the actuator handle assembly. The safety latch prevents operation of the second actuator when the gap between the anvil and staple holder is outside a predetermined range. The locking member is configured to prevent adjustment of the anvil gap after the desired staple height has been set inside the predetermined range.

Surgical stapling instrument with locking feature to lock anvil actuator

A cuff is used to provide a form for curing fluid about the exterior of an end-to-end anastomosis site. The cuff includes a port through which fluid may be injected to reach an interior space defined between the exterior of tissue at the anastomosis site and the inner surface of the cuff. A circular stapler may be used as a mandrel for the cuff and fluid. Alternatively, inflatable balloons may be used as a mandrel. The curing fluid may comprise a mixture of fibrin and thrombin.

Method and apparatus for sealing end-to-end anastomosis

A surgical instrument includes a body, a pivotable trigger, and a ratcheting assembly. The ratcheting assembly may include a rotary ratchet coupled to the trigger and a pawl coupled to the body. The rotary ratchet may further include a ramp that disengages the ratchet from the pawl. A release feature may be included to selectively disengage a second member of the ratcheting assembly from a first member. In some versions, the release feature may include a rotation knob or a slidable handle. In another configuration, the ratcheting assembly may have a first member coupled to an actuator and a second member coupled to the body. The assembly may include a lock member coupled to the body that selectively engages a plurality of teeth disposed on the actuator. Alternatively, the assembly may include a pivotable pawl coupled to the actuator that engages one or more notches formed in the body.

A treatment instrument comprises an end effector and a plunger. The end effector comprises a plurality of cutting features and an interior cavity. At least a portion of the plunger is slidably positioned within the interior cavity of the end effector. The plunger comprises a distal head attached to the distal end of a central shaft. The plunger is configured to rotate within the end effector, and comprises a cutting member that is operable to mince tissue specimens captured by the cutting features of the end effector. An alternative treatment system comprises an end effector, a plunger, and a plug. The central shaft of the plunger and the plug are slidably located within an inner tube of the end effector, with the plug being positioned distal to the central shaft.

Instrument for debriding fistula and applying therapeutic cells

An apparatus comprises a strap and an inflatable bladder secured to the strap. The apparatus is sized and configured to wrap along a first plane to encompass an anatomical feature such as an esophagus or stomach. The bladder may have a cross-sectional profile that is asymmetric about the first plane, such as a tapered profile or a contoured profile. The wall thickness of the bladder may be substantially uniform or may vary about the perimeter of the bladder. The bladder may include an expansion section that allows the bladder to transition from having a substantially flat cross-sectional configuration in a non-inflated state to having a substantially tapered cross-sectional configuration an inflated state. The expansion section may include folds, pleats, gussets, or the like. The strap may have a cross-sectional profile that is asymmetric about the first plane. Such a strap may include annular ribs of varying sizes.
