A panning stereo endoscope which maintains an up-down orientation as the stereo endoscope pans an operative field, the panning stereo endoscope includes a shaft having an axis; first and second optical channels extending along the shaft, each of the first and second optical channels having an off-axis direction of view, and an actuating mechanism carried by the shaft including at least one cam follower having at least one follower point and adapted to (i) synchronously rotate the first and second optical channels about their respective axes, and (ii) synchronously, inversely piston the first and second optical channels along their respective axes.

A surgical instrument includes a movable member having a proximal portion, the movable member defining an opening at the proximal portion. The instrument includes a stationary member defining an aperture. The stationary member is arranged relative to the movable member with a close fit between the stationary member and the movable member such that aspiration occurs through the movable member substantially only during a portion of a repetitive motion of the movable member. The instrument includes a surgical handpiece coupled to the movable member and the stationary member to provide suction and motion to the movable member.

A steerable stereoscopic endoscope including a shaft having a distal end, a proximal end, and an articulating region therebetween; a stereo image acquisition mechanism disposed at the distal end of the shaft for acquiring stereo images of a remote site; and a mechanism for steering the portion of the shaft distal to the articulating region.

The moving member and the stationary member are coupled to a surgical handpiece to provide suction and motion to the moving member.

A stereoscopic coupler for mechanically and optically coupling an endoscope to a portion of a camera, the stereoscopic coupler including means for mechanically and optically coupling the stereoscopic coupler to an endoscope; and means for mechanically and optically coupling the stereoscopic coupler to a portion of a camera; wherein the stereoscopic coupler includes a pair of optics; a mechanical actuator for operation by a user; and means for physically moving the pair of optics as a unit in response to movement of the mechanical actuator by the user.

Adapter

A method for removing a vein includes making an incision through a skin layer of a patient, inserting a surgical instrument through the incision, visualizing the vein through the skin layer using a light source positioned subcutaneously and in proximity of the vein, and cutting the vein using the surgical instrument. The vein can be visualized by directing light from the light source at the vein from underneath the vein or from a side of the vein. A device for illuminating a body structure has a housing with a distal end configured for subcutaneous insertion into a patient's tissue and positioning in proximity of the body structure, a light path supported by the housing and configured for directing light at the body structure, and a fluid line supported by the housing.

Methods and apparatus for removing veins

Endoscope

A cutting instrument includes an outer member having a cutting portion, and a helical knife coupled to the outer member for rotation relative to the outer member. At least a portion of the helical knife extends distally beyond the cutting portion. The instrument includes an inner member received within the outer member. The helical knife is at a distal end of the inner member. A method of cutting includes slicing into tissue with a helical knife to draw tissue proximally toward a cutting portion, and cutting the tissue with the cutting portion. The helical knife is provided on an inner member, and the cutting portion is provided on an outer member. Cut tissue is transported along a helical channel defined by the helical knife and aspirated through a hollow interior in the inner member.

Cutting instrument

One surgical device includes a first member and a second member defining a lumen for receiving the first member. The first member is configured to be movable relative to the second member to cut tissue. An electrical connector is physically and electrically coupled to the second member, to electrically couple the second member to a source of electricity. A tension device holds a distal region of the first member in electrical contact with a distal region of the second member. The first member includes a blade used both to cut tissue mechanically, and to coagulate cut tissue. The blade is electrically conductive and serves as an active electrode in a bipolar arrangement with a return electrode. Electrical energy is transferred to the blade through a point contact at a distal tip of the first and second members.
