Open surgery and endoscopic versions of an electrosurgical cutting instrument incorporating a unique cutting arrangement for opening and closing the cutting edge parallel to an anvil surface resulting in a simultaneous cutting of a linear section of tissue, large vessels, or a group of vessels. Simultaneous with the cutting, the instrument further being capable of either a monopolar or bipolar cauterization of tissue. Also disclosed herein, are methods for use of the various electrosurgical cutting instruments.

A bipolar electrosurgical scissors having a first cutting blade which has a first cutting portion, the first cutting portion having a corresponding first cutting edge; a second cutting blade which has a second cutting portion, the second cutting portion having a corresponding second cutting edge in cutting cooperation with the first cutting edge; and electrical insulation disposed on portions of the first and second cutting blades, the electrical insulation extending into the first and second cutting portions except for the corresponding cutting edges for preventing electrical conduction from the electrically insulated portions of the cutting blades. In a first embodiment of the bipolar electrosurgical scissors of the present invention, the electrical insulation extends into the first and second cutting blades such that &frac13; of their distal tip is uninsulated. In a second embodiment of the bipolar electrosurgical scissors of the present invention, the electrical insulation extends into the first and second cutting members such that they are entirely insulated except for their corresponding cutting edges.

Bipolar electrosurgical scissors for fine or delicate surgical dissection

A method of forming a combination bipolar scissors and graspers surgical instrument. The method has the steps of: providing a predetermined blade configuration of a bipolar scissors surgical instrument, the predetermined blade configuration having first and second jaw members each having a respective first and second cutting edge and first and second pivoting surfaces; defining a first grasping surface on the first jaw member, the first grasping surface being distal to the cutting edge, the first grasping surface being contained within the confines of the predetermined blade configuration; defining a second grasping surface opposing the first grasping surface on the second jaw member, the second grasping surface being contained within the confines of the predetermined blade configuration; providing a pivot pin pivotally connected to the jaw members wherein at least one jaw member is pivotable in relation to the other jaw member about the pivoting surfaces, the jaw members being capable of pivoting between an open and closed position such that the cutting edges form a scissor and the grasping surfaces form a clamp when the jaw members are in their closed position; and electrically isolating the first jaw member from the second jaw member. A further method is provided in which the cutting edges are distal to the grasping surfaces. Also provided are combination scissors/graspers having blade configurations as illustrated in FIGS.,A-B to,A-B.

Combined bipolar scissor and grasper and method of forming thereof

An electrical connecting cable for bipolar electrosurgical scissors. The cable has a pair of insulated parallel conductors having insulating coatings. The coatings are separably connected. A slidable grommet is mounted to the cable. The conductors are partially separable from each other. A plug is mounted to one end of the cable. Spring loaded connectors are mounted to the other end of the cable.

Cable for bipolar electro-surgical instrument

Open surgery and endoscopic versions of a combined bipolar electrosurgical cutting and grasping instrument where the grasping surfaces are contained within the shape of a standard surgical scissor. This unique arrangement results in a combined scissor and grasper which has the feel of a standard scissor but which allows surgeons to cauterize tissue and vessels while both cutting and grasping thus making the instruments well suited to perform coaptation of vessels. Also disclosed herein, are methods for using the various combined electrosurgical cutting and grasping instruments.

Combined bipolar scissor and grasper

A radiation-curable prepolymer of a polyurethane endtipped with an hydroxy (acrylate or methacrylate) is disclosed. The polyurethane is derived from the reaction of a polyfunctional isocyanate with a hydrolyzable oligomer of an anhydrous cyclic ester of an hydroxy acid. Crosslinked polymers prepared by irradiating the prepolymers are also disclosed. The crosslinked polymers are bioabsorbable and biocompatible with bodily tissue, yet still maintain the outstanding mechanical properties one would expect from a polyurethane. The prepolymers and crosslinked polymers are especially well-suited for the fabrication of surgical devices, particularly wound closure devices such as surgical staples and clips, in a stereolithography apparatus.

A mechanism and applier therefor which is capable of inverted anastomosis of two lumens by an absorbable fastener. The fastener is made from two washer-like plate assemblies and a pair of introducers. Each such plate assembly has slots to receive latching prongs protruding from the other plate, and pins to impale tissue. Fastening is done through an applier that causes the pins to pierce the tissue, and causes a knife blade to cut through excess tissue. Fastening is done by attaching fastener halves. Insertion and removal of the fastener and applier optimally is done in an endoscopic procedure through a trocar or through the surgical incision in an open procedure. The system can be used such that the plates can be placed to properly set gaps between and anastomose tissue. The introducer portion will soften intraluminally and be expelled, in about 24 hours. The coupler portion will remain in place for 2-3 weeks, typically, and then fragment and be expelled with the feces.

Laparoscopic absorbable anastomosic fastener and means for applying
