Delivery systems, methods and associated devices for management of an elongate element having sections with differing mechanical properties, in particular, for management of a loop of suture-wire element extending proximally from a delivery catheter for deployment of a heart implant. Such management devices can include an inner feature (e.g. outward facing groove or series of tabs) along an inner circle about which a suture can be coiled and an outer feature (e.g. inward facing groove or tabs) to engage and constrain a wire section having increased stiffness within a coil having a larger diameter than the coiled suture. Such devices can be provided separately or can be integrated within a handle of the delivery catheter. Methods of loading suture-wire management devices and utilizing such devices during deployment of an implantable anchor are also described herein.

Suture management device and methods

Delivery systems, methods and associated devices to facilitate delivery and deployment of a heart implant. Such delivery systems and methods of delivery include use of a pair of magnetic catheters, including an anchor delivery catheter carrying an anchor, which can be stacked with or can be axially offset from the magnetic head. Such systems further include use of a puncturing guidewire advanceable through the magnetic head of the anchor delivery catheter to establish access to a chamber of a heart and which is attached to a bridging element such that continued advancement of the guidewire draws a bridging element attached to the first anchor across the chamber of the heart while the bridging element remains covered by the magnetically coupled catheters. Methods and devices herein also allow for cutting and removal of a bridge element of a deployed heart implant.

Methods and apparatus for delivery of substances or apparatus to target sites located outside blood vessels within the body of a human or animal patient. A vessel wall penetrating catheter is inserted into the vasculature, positioned and oriented within a blood vessel near the target extravascular site and a penetrator is advanced from the catheter so as to penetrate outwardly through the wall of the blood vessel in the direction of the target site. Thereafter, a delivery catheter is passed through a lumen of the penetrator to the target site. A desired substance or apparatus is then delivered to or obtained from the target site. In some applications, the penetrator may be retracted into the vessel wall penetrating catheter and the vessel wall penetrating catheter may be removed, leaving the delivery catheter in place for chronic or continuous delivery of substance(s) to and/or obtaining of information or samples from the target site. Alternatively, a delivery catheter having an occlusion member or balloon may be advanced into a vein or venule and the occlusion member or balloon may be used to occlude the lumen of the vein or venule during and after injection of a substance through the catheter, such that the substance will not be carried away by normal venous blood flow and will remain in the vein or venule for a sufficient period of time to have its intended effect (e.g. to enter adjacent tissues through capillary beds drained by that vein or venule).

Catheters, systems and methods for percutaneous in situ arterio-venous bypass

Devices and methods for penetrating from a vessel lumen in which a catheter device is positioned to a target location outside of that vessel lumen. A tissue penetrator advances from a catheter positioned within the vessel lumen to the target location when properly aimed. An imaging transducer fixedly mounted on or within the catheter body provides an imaging signal from which an image of the target location can be obtained. An imageable marker may be present on the catheter to form on the image obtainable from the imaging signal to predict the path that will be followed by the tissue penetrator when the tissue penetrator exits from the catheter. Alternatively, or addition thereto, the imaging transducer may comprise a plurality of imaging elements which are located so that the penetrator path indication can be obtained electronically without use of an imageable marker on the catheter.
