The form includes a second line of weakness in the central area to divide the central area into a first side and a second side. The form includes a third line of weakness formed in the perimeter area to define a sleeve area. The form includes a fourth line of weakness formed in the sleeve area to divide the sleeve area into a first sleeve layer and a second sleeve layer.

Forms containing a removable hang tag

Methods and apparatus for a form for a postcard mailer are disclosed. An example form includes a substrate sheet having top and bottom edges, first and second side edges, and first and second faces. The form also includes at least first and second fold lines dividing the sheet into at least first, second, and third panels with a hang tag removably coupled to at least a portion of one of the first, second or third panels. Finally, the form has a first adhesive area provided on the first face of at least one of the second or third panels and a second adhesive area provided on the second face of at least one of the first and second panels, wherein the first and second adhesive areas adhere the third panel to the first face of the second panel and the first panel to the second face of the second panel when the substrate sheet is folded about the fold lines.

Forms containing removable hang tags and methods of producing the same

A window decal form is suitable for laser printing and enables ready installation of a decal to the inside of a vehicle window or the like. The window decal form construction includes a liner sheet having a front side and a back side. An adhesive release coating is disposed on the back side of the liner sheet framing a decal area. The decal area and the adhesive release coating define a label area. The label is affixed to the back side of the liner sheet covering the label area via a permanent adhesive. The decal area on the front side of the liner sheet is die cut such that the decal area is removed when the label is peeled from the back side of the liner sheet.

Laser printable window decal from construction

A high temperature RFID tag is provided that is adapted to receive thermal transfer print to provide indicia on an outer surface thereof and adapted to be disposed on a part that is subject to high temperature exposure. The RFID tag is formed as a lamination of a high temperature material such as high temperature polyester and including a window for an RFID generator component, a layer of high temperature resistant material that overlies the opening, and an adhesive material disposed in the window to secure the RFID generator component therein. The layer of high temperature resistant material is preferably transparent so that the indicia can be seen therethrough.

High temperature tag having enclosed transceiver

A medical test form having a biological sample receiving material mounted thereto. In one embodiment, the sample receiving material is incorporated in a sample receiving sub-assembly that is secured to a multi-panel carrier substrate that has instructions and/or patient information printed thereon. In another embodiment, the sample receiving material provided on one panel of a V-fold mailer. If necessary or desirable one or more additional panels can be secured to the panels of the V-fold mailer.

Medical test form

A laser printable sheet with vehicular registration and permitting decals and/or stickers is produced in a manner that insures the sheet lays-flat, rather than being crooked, so that it does not cause significant jamming problems in laser printers. Adhesive-release material is spot coated on a web back surface, including a first patch, the back surface is imaged with static indicia, and a substantially transparent plastic pressure sensitive adhesive laminate is applied over the back surface, including over the adhesive release material patch and first static indicia. The face is die cut at the first patch within the parameter of the patch to form the first die cut portion, which is then sucked off the substrate to provide an open area with exposed transparent plastic and pressure sensitive adhesive, and then a piggy back decal is blown onto the open area. The decal is preferably a license plate decal having a reflective visible surface in vehicular registration and permitting indicia. Preferably the sheet also includes an inspection sticker, may include a second license plate decal, as well as variably printed registration information.
