Embodiments of the present disclosure include a cardiac device comprising a band configured for deployment within a heart. The band may include a first end and a second end, an actuatable clasp associated with the first end of the band and configured to transition, upon actuation, from an open configuration to a closed configuration for forming the band into a fixed length loop after the second end is moved beyond the clasp. The clasp may be configured for actuation via a catheter. The cardiac device may include a clasp retainer associated with the clasp, the clasp retainer being configured to hold the clasp in the open configuration and the clasp being configured to be actuated upon movement of the clasp retainer, and a clasp actuator configured to move the clasp retainer thereby actuating the clasp.

The present disclosure provides methods of improving cardiac function, including transcutaneously inserting at least one catheter into a heart, delivering a band to a ventricle of the heart via the at least one catheter, guiding the band through a plurality of spaces among a trabeculae in the ventricle, tightening the band in a single loop, locking the band in a loop, and removing the at least one catheter from the heart.

Embodiments of the present disclosure include a cardiac device comprising a band configured to form a loop within a heart and including a first end and a second end, a plurality of sequential locking segments located in a region of the band near the second end, and an actuatable clasp located at or near the first end. Each locking segment may have a ledged region and a ramped region. The actuatable clasp may be configured to form a fixed length loop by locking onto the ledged region of a locking segment after the second end has been inserted into the clasp. Adjacent locking segments may be configured to flex relative to each other, thereby enabling adjacent ramped regions to cooperate with each other to facilitate a sliding of the segments into the clasp.

Embodiments of the present disclosure include a method of transcatheterly delivering a band to encircle multiple papillary muscles in a heart. The method may comprise transcutaneously inserting a catheter into a heart and delivering a first end of a first guidewire to a ventricle in the heart via the catheter. The method may further comprise looping a first end of a first guidewire around a first papillary muscle and a second end of a second guidewire around a second papillary muscle. The first end of the first guidewire and the second end of the second guidewire may be brought out of the body while the first and second guidewires remain looped around the first and second papillary muscles, respectively. The first and second guidewires may be interconnected, and at least one end of the first and second guidewires may be pulled to establish a single loop around the papillary muscles.

Embodiments of the present disclosure include a device for repositioning papillary muscles. The device may comprise a band configured to encircle a cluster of papillary muscles. The cluster of papillary muscles may have an outer peripheral boundary defined by the encircling band, and the band may comprise a first end and a second end. The device may further comprise an adjustable clasp coupled to a location proximate the first end of the band. The clasp may be configured to transition from an open configuration to a closed configuration to connect the second end of the band to the first end of the band, thereby forming a loop. The clasp may be selectively actuatable to adjust a length of the loop.

Systems and methods disclosed provide for guiding a tool using a source of radioactivity. System and methods disclosed provide a device for inserting an implant with a medical function into body tissue. The device can include a channel passing through the implant, a cannulus with a channel sized to hold the implant, and a pusher with a channel and sized to be situated inside the channel of the cannulus behind the implant, and adapted to push the implant through a distal end of the cannulus into the tissue. The device can also include a restraining element that extends through the channel of the pusher and at least partly through the channel of the implant when they are so situated in the channel of the cannulus, which restrains the implant, but allows the implant to exit the distal end of the cannulus when the implant is pushed with a force small enough so as not to damage the function of the implant in the tissue.
