A patient wristband form has a transparent ply with an upper surface and a lower surface, and a pressure sensitive adhesive coating on the lower surface. A release ply has an upper surface and a lower surface, with a release coating on the upper surface. The transparent ply is removably mounted on the upper surface of the release ply by the pressure sensitive adhesive coating. A die cut in the transparent ply defines an elongated wristband. An opaque coating is provided on the upper surface of the transparent ply in a central portion of the elongated wristband. A die cut in the transparent ply defines an overlaminate label that is sized to cover at least a part of the central portion of the elongated wristband, thereby covering indicia printed on the opaque coating. A paper ply may also be mounted on the release ply by pressure sensitive adhesive. The paper ply defines a plurality of labels that may be printed at the same time as the wristband.

A business form according to the present invention including a ply of label stock, a ply of release liner stock, and a ply of message or record bearing stock, may be configured as either a continuous or cut sheet form. The ply of label stock has one side coated with a pressure sensitive adhesive. The ply of release liner stock has a release side that engages with the adhesive on the one side of the label stock. The release liner ply and the label stock ply have edges that are laterally offset from one another so that a narrow longitudinal strip of adhesive is unengaged with the release liner. The ply of message bearing stock is adhered at one edge to the label stock at the narrow longitudinal strip of adhesive. A plurality of cuts is defined by, and extends through, the release liner from the back of the business form, whereby the structural integrity of the business form is enhanced.

An information band for mounting an electrically conductive circuit, such as a radio-frequency identification (RFID) tag. The band comprises a base strip and a top strip. A single sheet of label stock can include a plurality of such bands such that the sheet can be passed through a conventional printer to accept printed indicia onto a surface of one or more bands. These bands can be die cut to facilitate individual removal. The top strip can be either integral with the base strip or a separate piece that can be affixed to a surface of the base strip. Once indicia has been printed to the band, the RFID tag may be manually affixed between the base strip and the top strip, where adhesive disposed therebetween secures the RFID tag. The adhesive may be peripherally deposited such that an adhesive-free zone is formed between the two layers so that when the RFID tag is attached, it does not come in contact with the adhesive.

Gift card forms and methods of making and using gift card forms are disclosed. The gift card form permits uniquely messaging each gift card. An appropriate gift card form can be printed at a time after manufacturing to allow a vendor to personalize the gift card. Additionally, order information, such as billing information, can be printed on the gift card form at the same time that the gift card is personalized. The gift card form can be kept as a record with the order information.

A pattern of coating is applied to a label, such that the coating covers the periphery, including a portion of the top surface of the label, and overlaps onto the liner ply area adjacent to the label. The coating is selected to form a stable and resistant film that spans continuously along the label edge and liner surfaces. The coating reduces adhesive ooze when the label is exposed to elevated ambient temperatures and pressures.

An apparatus is provided for in-line folding and affixing a label to an article, the label having a fold line thereacross defining a foldable portion and a cover portion of the label. The apparatus comprises a transfer grid having one or more rollers to convey the label from a first end of the transfer grid to a second end of the transfer grid. An opening is defined between two of the rollers intermediate the first and second ends of the transfer grid. The transfer grid conveys the label in a substantially unfolded orientation from the first end of the transfer grid to the opening of the transfer grid such that the foldable portion of the label is projective through the opening. A folding station is adjacent the opening and receives the foldable portion of the label, urging a projecting portion of the foldable portion over the cover portion to define a substantially folded-over orientation of the label. The transfer grid conveys the label being in the substantially folded-over orientation from the opening of the transfer grid to the second end of the transfer grid, whereat an affixing station is provided adjacent the second end of the transfer grid to receive the label and affix the label to the article.

A shipment form is provided including a face sheet, a liner ply, a reusable adhesive laminate, and a laminating adhesive. The face sheet defines first and second major faces and includes printed indicia on the first major face. The printed indicia being arranged to indicate an attempted delivery. The reusable adhesive laminate includes a reusable adhesive and a removable ply. The reusable adhesive and the removable ply are configured such that the reusable adhesive retains its properties of adhesion following removal of the removable ply therefrom. The laminating adhesive is interposed between the liner ply and the face sheet so as to secure the liner ply to the second major face of the face sheet. The laminating adhesive is further interposed between the reusable adhesive laminate and the face sheet so as to secure the reusable adhesive laminate to the second major face of the face sheet.

A security label for a specimen container comprising a generally elongated strip of label material having a plurality of lines of perforations is disclosed. Also disclosed is a security label for a specimen container having a line of perforations. The line or lines of perforations of the disclosed invention extend across the width of the generally elongated strip of label material. A method of securing a specimen container is also disclosed. The method disclosed involves providing a security label comprising a generally elongated strip of label material defining a first surface and a second surface. A plurality of lines of perforations extend across the width of the generally elongated strip of label material. The security label is then adhered to a specimen container so that the center portion of the security label is aligned with the lid of a specimen container.

This need is met by the present invention wherein an improved shipment form is provided comprising a partially releasable liner ply, a peel-out package label portion, and a peel-out signature tab. In accordance with one embodiment of the present invention, a shipment form is provided comprising a face sheet, a liner ply, and an adhesive interposed between the face sheet and the liner ply. The face sheet includes printed indicia for indicating an attempted delivery. The adhesive includes a laminating adhesive portion and a reusable adhesive portion. The liner ply defines a releasable liner ply portion adjoining a laminated liner ply portion. The releasable liner ply portion is configured to releasably adhere to the reusable adhesive portion while remaining adjoined to the laminated liner ply portion. The reusable adhesive portion and the releasable liner ply portion are configured such that the reusable adhesive portion retains its properties of adhesion following release of the releasable liner ply portion therefrom. The laminated liner ply portion is configured to adhere to the laminating adhesive portion with an adhesion strength substantially greater than an adhesion strength of the releasable liner ply portion to the reusable adhesive portion.

A linerless continuous series of pressure sensitive adhesive labels is provided which have on one or more selected portions thereof a series of imagable coatings which may be activated to form colored images or areas. The series of labels include a web having first and second surfaces, with a series of imagable coatings of color formers and color developers on the first surface of the web. A coating of a release material overlies the imagable coatings on the first surface of the substrate, and a coating of a pressure sensitive adhesive is positioned on the second surface of the web. The series of labels are wound or stacked so that the coating of pressure sensitive adhesive contacts the release material to permit individual labels to be dispensed.

In accordance with the present invention, a multiple-ply label form is provided which includes a plurality of readily removable elements for use in facilitating business operations, such as package shipment. The removable elements include at least one adhesive backed element securable elsewhere by means of the adhesive, and a single-ply and/or multiple-ply element readily separable from the balance of the label form. The adhesive backed element may have one or more plies. Mailing information or other indicia may be applied on each element as desired. The single-ply and/or multiple-ply element may have an adhesive free backing for filing, mailing or other applications related to the business employing the label form.

A form having detachable labels and a wristband is provided. The form includes a face ply adhered to a liner ply by a pressure sensitive adhesive. The face ply includes a first portion and a second portion, where the first portion is die cut to form a wristband and the second portion is die cut to form a series of detachable labels. The form may be printed in a single pass through a printer to provide the wristband and labels with correlating printed indicia.

Multiple ply labels include removable single ply portions for use in facilitating package shipment operations. Two-ply and a three-ply embodiments are disclosed with release material being applied to portions of a second or bottom ply which are overlain by tab portions of a first or top ply having adhesive applied to the underside of the tab portions and around the periphery of the top and bottom plies to adhere the plies to one another to assemble the labels. The tab portions are preferably formed by die cuts; however, lines of weakness created for example by creasing or lines of perforations can be used to make the tab portions readily separable from the top ply. Card portions are also formed by lines of perforations or other lines of weakness in the top ply with the card portions being formed clear of any adhesive coating such that they are free of adhesive for filing, mailing or other applications related to the package shipment operations.
