An apparatus and method for use in performing ablation or coagulation of organs and other tissue includes a metallized fabric electrode array which is substantially absorbent and/or permeable to moisture and gases such as steam and conformable to the body cavity. Following placement of the ablation device into contact with the tissue to be ablated, an RF generator is used to deliver RF energy to the conductive regions and to thereby induce current flow from the electrodes to tissue to be ablated. As the current heats the tissue, moisture (such as steam or liquid) leaves the tissue causing the tissue to dehydrate. Suction may be applied to facilitate moisture removal. The moisture permeability and/or absorbency of the electrode carrying member allows the moisture to leave the ablation site so as to prevent the moisture from providing a path of conductivity for the current.

Inactivation of smooth muscle tissue

A catheter for insertion into and treatment of tissue in a patient comprises a radio frequency (RF) electrode having an elongated body that conducts electrical RF energy to a conductive tip. An insulating sleeve surrounds the elongated body. In a first mode of operation, the conductive tip is exposed outside the insulating sleeve and the RF electrode delivers RF energy that erodes the tissue of the patient and creates a tunnel through which the catheter can advance into the tissue of the patient. In a second mode of operation, the insulating sleeve and/or the RF electrode is retracted and a second treatment apparatus treats the tissue.

Tissue-penetrating guidewires with shaped tips, and associated systems and methods are disclosed. A patient treatment system in accordance with one embodiment of the disclosure includes a tissue-penetrating guidewire that in turn includes a flexible segment having a distal portion and a proximal portion. The flexible segment is elongated along an elongation axis. A penetrating member is positioned at the distal portion and includes at least one blade segment having a tapered outer peripheral surface and an adjacent generally sharp edge. The blade segment extends to a distal end of the penetrating member to form a generally blunt tip. In operation, the guidewire can be connected to an electrical current source to deliver high frequency current to the penetrating member.

Tissue-penetrating guidewires with shaped tips, and associated systems and methods

Systems and methods for transeptal cardiac procedures are disclosed. A method for treating a patient in accordance with a particular embodiment includes positioning a tissue penetrating guidewire adjacent to a cardiac septum, directing pulses of energy to the guidewire, and advancing the guidewire into and through the septum by moving the guidewire in a distal direction in a series of discrete steps. Individual steps can be of a predetermined distance measured outside the patient's body. The method can further include passing a catheter over the guidewire after the guidewire has passed through the septum.

Systems and methods for transeptal cardiac procedures, including tissue penetrating members and associated methods

A catheter for insertion into and treatment of tissue in a patient comprises a radio frequency (RF) electrode having an elongated body that conducts electrical RF energy to a conductive tip. An insulating sleeve surrounding the elongated body prevents leakage of RF energy from the elongated body when the catheter is being inserted into the tissue of the patient. In a first mode of operation, the conductive tip is exposed outside the insulating sleeve and the RF electrode delivers first RF energy capable of producing sparks that erode the tissue of the patient and create a tunnel through which the catheter can advance into the tissue of the patient. In a second mode of operation, the insulating sleeve is retracted to expose a portion of the RF electrode. Second RF energy is delivered to the volume of tissue around the catheter to necrotize the tissue by heating without producing sparks.

Methods, systems, and devices for transseptal access into the left atrium of a heart. In one embodiment an apparatus for transseptal left atrial access comprised of a catheter adapted for insertion into a vessel and one or more RF devices adapted to be extendable from the distal end of said catheter and configured for the penetration or sealing of septal tissue.

The present invention provides for therapeutic treatment methods, devices, and systems for the partial or complete closure or occlusion of a patent foramen ovale ('PFO'). In particular, various methods, devices, and systems for joining or welding tissues, in order to therapeutically close a PFO are described. An interventional catheter assembly comprises an operating head for removing obstructive material from a target site in a body lumen or cavity and at least one aspiration port located proximal to the operating head and penetrating the catheter assembly, the aspiration port being in communication with a sealed lumen that communicates with a vacuum system for withdrawing aspirate fluid and obstructive material from the target site. A rotatable member is positioned inside the catheter assembly at the site of the aspiration port and rotates during operation of the vacuum system. The rotatable member is provided with at least one upstanding bar that is sized to cooperate with the walls of the aspiration port and the inner surface of the catheter assembly to macerate debris that is drawn into the aspiration port.

Interventional catheters incorporating an active aspiration system

Methods and systems for securing tissues, e.g., cardiovascular tissues, are disclosed. A method in accordance with one embodiment of the invention includes inserting a portion of a catheter into an opening between a first portion of cardiovascular tissue and a second portion of cardiovascular tissue. The method can further include drawing the first and second portions of the cardiovascular tissue into contact with each other by drawing a vacuum in a region adjacent to the first and second portions of cardiovascular tissue via the catheter while the catheter is positioned between the first and second portions. The tissue portions can be fused by heating the tissue from within the opening, e.g., via radio frequency energy. This technique, and associated catheter system, can be used to close a patent foramen ovale or other openings in cardiovascular tissue.

Systems and methods for securing cardiovascular tissue

The intralumenal material removal system includes a cutter assembly positionable in the lumen of a mammalian subject and operably connected to system controls. The cutter assembly is axially advanceable by translating the drive shaft and rotatable by rotating the drive shaft. One cutter assembly comprises an adjustable cutter that is adjustable between a smaller diameter condition and a larger diameter condition by rotation of the drive shaft in opposite directions. The cutter may thus be introduced to and withdrawn from the material removal site in a retracted, smaller diameter condition that facilitates translation and navigation of the device through various lumens. The adjustable cutting assembly may be selectively expanded at the material removal site to facilitate cutting, removal and aspiration of the occlusive material. One composite cutter assembly comprises both a distal, fixed diameter cutter and a proximal adjustable diameter cutter.

The electrode carrying member is mounted to the distal end of an elongate shaft, and an array of electrodes is mounted to the surface of the electrode carrying member. Following placement of the ablation device into contact with the tissue to be ablated, an RF generator is used to deliver RF energy to the electrodes and to thereby induce current flow from the electrodes to tissue to be ablated. As the current heats the tissue, moisture (such as water vapor or liquid) leaves the tissue causing the tissue to dehydrate. The moisture permeability and/or absorbency of the electrode carrying member allows the moisture to leave the ablation site so as to prevent the moisture from providing a path of conductivity for the current.
