Brugada syndrome and related forms of ion channelopathies, including ventricular asynchrony of contraction, originate in the region near the His bundle or para-Hisian regions of the heart. Manifestations of Brugada syndrome can be corrected by delivering endocardial electrical stimulation coincident to the activation wave front propagated from the atrioventricular (AV) node early enough to compensate for the conduction problems that start in those regions. The stimulation can be delivered in multiple modes and can include waveforms of the same polarity delivered to a site within the region near the His bundle or para-Hisian regions of the heart associated with a low cardiac electrical asynchrony level or can include at least two single-phased superimposed waveforms of opposite polarity delivered through a pair of pacing electrodes relative to a reference electrode, which can be delivered to any site within the region near the His bundle or para-Hisian regions.

One embodiment provides a system for cardiac stimulation optimization utilizing cardiac asynchrony and pulse pressure data. The system includes: an analysis circuitry to receive cardiac signals collected at two locations of a patient's heart during an application to the heart of stimulation in accordance with multiple (VV) delay intervals, calculate an asynchrony index for the VV delay intervals, and determine one of the VV intervals as optimal based on the asynchrony index for that VV interval; an implantable stimulation device to cycle through the VV intervals while applying the stimulation, and further configured to cycle through atrioventricular (AV) delay intervals while applying additional stimulation in accordance with the optimal VV delay interval; and an arterial pulse pressure sensor to measure arterial pulse pressure during the application of the additional stimulation, wherein the analysis circuitry determines one of the AV delay intervals as optimal based on the arterial pulse pressure measured.

One embodiment provides a system for determination and utilization of cardiac electrical asynchrony data. The system includes an analysis circuitry including a processor and a memory, the analysis circuitry configured to: obtain a plurality of sets of cardiac signals collected in at least two locations of a heart of a patient, the signals comprising at least one of surface electrocardiography signals and pseudo-surface ECG signals; detect one or more QRS complexes for each of the sets based on the cardiac signals for that set; obtain one or more cross-correlation signals, each of the cross-correlation signals being between at least two of the signal sets and being obtained using the detected QRS complexes from the signal sets; and calculate one or more asynchrony indices using one or more of the cross-correlation signals, each of the asynchrony indices being indicative of a level of asynchrony between the at least two locations.

Various aspects of the present disclosure are directed toward an asynchrony index that is related to data of a subject's heart. The asynchrony index includes intra-ventricular or inter-ventricular electrical asynchrony data. According to one aspect, various methods and apparatus are used for treating a condition of a patient's heart, and for monitoring cardiac operation. In one approach consistent therewith, an electrode arrangement is placed in a right ventricle of the heart. The electrode arrangement is used to capture the myocardium for re-synchronization of the left and right ventricles by providing first and second signal components having opposite polarity on respective electrodes. The electrode arrangement is connected to an implantable CRM device that has the capability of pacing/sensing atrium, pacing/sensing ventricles, and deliver defibrillation therapy from the right side of the heart. The CRM device captures ventricular contractions to treat conduction abnormalities in one or more of the ventricles.

Various embodiments of the present invention are directed to systems, methods and devices for cardiac applications. One such device is directed to a catheter, and uses thereof, for capturing myocardium of a heart by delivering pacing signals to a location in the heart. The location is near a His Bundle of the heart. The catheter has a proximal end for interfacing with an electrical pacing signal source and a distal end. The distal end includes a fixation mechanism that attaches the catheter to heart tissue. First and second electrodes are also located at the distal end. Each electrode is individually addressable for providing pacing signals to the heart tissue and also arranged to physically contact the heart tissue when the fixation mechanism is attached to the heart tissue.

Methods, devices and systems for cardiac rhythm management using an electrode arrangement

Various systems, methods, devices and arrangements are implemented for use in pacing of the heart. One implementation is directed to methods and systems for determining a pacing location in the right ventricle of a heart and near the His bundle. A pacing signal is delivered to the location in the right ventricle. The pacing signal produces a capture of a left ventricle. Properties of the capture are monitored. Results of the monitored capture are used to assess the effectiveness of the delivered pacing signal as a function of heart function. The heart function can be, for example, at least one of a QRS width, fractionation and a timing of electrical stimulation of a late activation site of a left ventricle relative to the QRS.

Various embodiments of the present invention are directed to systems, methods and devices for cardiac applications including those relating to pacing devices. One such device is directed to a cardiac rhythm therapy (CRT) device designed for dual chamber pacing using two pacing signals each having a positive and negative component that has been modified for single chamber pacing. The device comprises a first output that connects to a pacing lead; a second output that connects to the pacing lead; a third output that connects to a reference point; and electrical circuitry connecting the second electrical connection to the first output, the third electrical connection to the second output, and the first and fourth electrical connections to the third output.

Various systems, methods and arrangements are implemented in connection with ventricular pacing. One such method relates to a method for use in connection with ventricular pacing of a left ventricle of a heart from a pacing lead located in the right ventricle. Ventricular function of the heart is sensed. The sensed ventricular function is used to determine whether a conduction abnormality exists. The ventricular pacing is provided in response to determining a conduction abnormality exists and the ventricular pacing is inhibited in response to determining a conduction abnormality does not exist.
