Gastrointestinal bypass devices are described. In one embodiment, a gastrointestinal bypass device may include an anchoring membrane configured to be attached to a wall of an esophagus with a tissue anchor placed through the anchoring membrane. The anchoring membrane may be configured to be pierced to allow the tissue anchor to be placed through the anchoring membrane. Methods for attaching a device to a tissue wall are described. In one embodiment, a method may include forming a bulge in an anchoring membrane of the device and the tissue wall, piercing the bulge from a first side of the anchoring membrane and a first side of the tissue wall, placing a second retention element of a tissue anchor on a second side of the tissue wall, and, placing a tension element of the tissue anchor through the anchoring membrane and the tissue wall. The tension element may be coupled to the second retention element. The method may further include placing a first retention element on the first side of the anchoring membrane. The first retention element may be coupled to the tension element.

Devices and methods for gastrointestinal bypass

Delivery devices are described. In one embodiment, a delivery device includes a catheter and a sealing pad coupled to an outer surface of the catheter. The delivery device may further include a recess formed in an outer surface of the catheter. The sealing pad may be coupled in the recess. Methods for delivering a tubular device are described. In one embodiment, a method may include inserting a distal end of a catheter into a lumen of a tubular device. The catheter may include a sealing pad coupled to an outer surface of the catheter. The method also includes positioning the sealing pad inside a drawstring coupled circumferentially to the tubular device, and cinching the drawstring around the catheter at the sealing pad to couple the tubular device to the catheter.

A gastrointestinal bypass device for directing food and liquids from an esophagus and/or a proximal portion of a stomach into an intestines is described. The device comprises a receiver, a sleeve, and a device coupling. The receiver may be configured to be positioned in the esophagus and/or the stomach. A proximal portion of the receiver may be configured to open and close to at least partially conform to an inside of the esophagus and/or a proximal portion of the stomach. The receiver may be configured to receive food and liquids from the esophagus and/or a proximal portion of the stomach into a lumen of the receiver. The sleeve may be coupled to a distal portion of the receiver. The sleeve may be configured to be positioned in the stomach and the intestines. The sleeve may have a lumen in communication with the lumen of the receiver. The sleeve may be configured to direct the food and the liquids from the receiver into the intestines. The device coupling may be coupled to the distal portion of the receiver and/or a proximal portion of the sleeve. The device coupling may be configured to be coupled to one or more tissue anchors.

Devices and methods for gastrointestinal bypass are described. A gastrointestinal bypass device includes a gastrointestinal cuff and a gastrointestinal sleeve. The cuff may be configured to be attached in the esophagus, and may be sufficiently flexible to expand and collapse to conform with the inside of the esophagus to allow the esophagus to function substantially normally. The sleeve is configured to be coupled to the cuff, and may be made of a material that is floppy or flaccid but does not substantially expand radially.

Disclosed herein are elongate flexible medical devices which are capable of axial elongation through a mechanism of eversion or toposcopic expansion. In general, this may be accomplished by providing a flexible tubular body having a proximal end and a distal end. Retraction of the distal end in a proximal direction through the tubular body inverts the tubular body upon itself, causing an axial shortening of an overall length of the tubular device. An original length of the tubular device can be restored by coupling a pressurized media to the proximal end of the tubular device. If the distal end of the tubular device is temporarily restricted or closed, the pressurized media causes the distal end of the tubular device to travel distally until a full length of the tubular device has been restored.

Methods for toposcopic sleeve delivery

Disclosed herein are systems and methods for attaching or maintaining the position of a therapeutic or diagnostic device in a body lumen, such as the GI tract without necessarily requiring any penetrating attachments through any body walls. The system can include at least two elements: a proximal orientation element and a distal support element. The proximal orientation element can be configured to reside at least partially within the esophageal lumen and the distal support element can be configured to reside in the stomach, such as along the greater curve of the stomach. An intragastric support system can have a first configuration in which the long axis of the proximal orientation element is substantially parallel and/or substantially coaxial with the long axis of the distal support element, and a second configuration in which the long axis of the proximal orientation element is not substantially coaxial with the long axis of the distal support element. The second configuration can thus advantageously retain the intragastric support system in place and prevent unwanted proximal migration of the distal support element into the esophagus or distal migration into the intestine.
