A mapping catheter comprises a flexible shaft having a proximal end and an opposite distal end, and an expandable mapping element attached to the distal end of the shaft. The expandable mapping element is configured to self-expand from a collapsed configuration to an expanded configuration. The expanded configuration is defined by a proximal portion extending from the distal end of the shaft and forming a proximally-facing concave recess, an intermediate portion extending distally from the proximal portion and defining a maximum diameter of the mapping element when in the expanded configuration, and a distal portion having a conical shape with a distal portion diameter that decreases in a direction distally of the intermediate portion. The mapping element further includes a plurality of electrodes located in one or both of the intermediate portion or the distal portion.

A retention device for use with an implantable medical device (IMD) may comprise an elongate body including a configured to receive the lead of the IMD. The retention device may also include securing mechanisms coupled to the elongate body and configured to push against tissue of a patient. The securing mechanisms may also include linking elements coupled to the elongate body and a portion of the securing mechanisms.

Retention mechanism for an implantable lead

An illustrative anchoring mechanism is provided for attachment to an implantable subcutaneous lead to facilitate anchoring at the distal tip of the lead. The anchoring mechanism is attached to an opening in a distal portion of a subcutaneous lead prior to implantation. The anchoring mechanism may be designed to avoid covering the sensing electrode of the subcutaneous lead, to prevent interference with sensing. Use of such an apparatus may reduce the number of incisions needed to perform implantation.

Retainer devices and methods are configured for securing medical devices subcutaneously in a patient. The retainer device may include first and second arms connected by a bridge, the retainer clip configured to move between a first orientation in which lower surfaces of the first and second arms face each other, and a second orientation in which the first and second arms extend laterally from opposite sides of the bridge, wherein the clip is biased in the second orientation. Another example includes a coil and delivery device in which the delivery device holds the coil in a general straight configuration until release; when released the coil wraps about a medical device and holds the device to the subcutaneous tissue.

Implantable leads of a cardiac stimulus system are disclosed, as well as methods for implanting leads of a cardiac stimulus system. The lead may be comprised of a proximal portion having a coupler for coupling to an implantable pulse generator, an intermediate portion comprising a plurality of electrodes disposed thereon, and a distal portion. The intermediate portion may have a first diameter and the distal portion may have a second diameter. The distal portion may also have an attachment feature for attaching to a lead pulling tool for delivery to an ITV and an intercostal vein. Methods may include pulling a lead from a first position to a second position within the vasculature, exiting the vasculature at the second location, and attaching a portion of the lead that exits the vasculature to an electrode or implantable device for use with the lead.

Implantation of a cardiac stimulus system using the ITV. Superior, intercostal, and inferior access methods are discussed and disclosed. Superior access may be performed using the brachiocephalic vein to access the ITV, with access to the brachiocephalic vein achieved using subclavian vein, using standard visualization techniques. A positioning mechanism may be advanced to the ITV, a location of the positioning mechanism may then be obtained, and an external access may then be established. Inferior external access may be accomplished inferior to the lower rib margin via the superior epigastric or musculophrenic vein. Intercostal external access may be accomplished via an intercostal vein between two ribs. A lead may then be attached to the positioning mechanism and drawn into the ITV.

Implantation of a cardiac stimulus system using the intercostal veins. Superior, intercostal, and inferior access methods are discussed and disclosed. Superior access may be performed by entering the brachiocephalic vein from a jugular, subclavian, or other vein, and then accessing the internal thoracic vein, traversing a portion of the internal thoracic vein and then accessing an intercostal vein therefrom. Inferior access may be accomplished inferior to the lower rib margin via the superior epigastric vein, advancing superiorly into the internal thoracic vein and then accessing an intercostal vein therefrom. Intercostal access may include creating an opening in an intercostal space between two ribs and advancing a needle using ultrasound guidance to enter the intercostal vein directly.

The intercostal vein may be located on a costal groove on an inferior portion of a rib. Advancement from the intercostal vein to the ITV may then be performed in a superior direction, an inferior direction, or both.

Various aspects of the present disclosure are directed toward methods, apparatuses, and systems that include an implantable medical device comprising an implantable lead, a suture sleeve having an interior surface defining a lumen of the suture sleeve that receives the implantable lead, and an engagement feature configured to non-removeably secure the suture sleeve to the implantable lead.

A multipolar lead evaluation device may be configured to easily permit electrical contact to be made between a pacing system analyzer and the terminal pin and each of the terminal contacts of an implantable lead without damaging the implantable lead. Alligator clips may be used to secure electrical conductors from the pacing system analyzer to spring contact clips disposed within the lead evaluation device.

An implantable lead includes a flexible lead body, a plurality of conductor wires and a plurality of electrodes. The implantable lead also includes a terminal connector assembly coupled with a proximal end of the lead body. The terminal connector assembly is sized to be inserted into and received by a connector port of a pulse generator header. The terminal connector assembly includes a plurality of axially spaced terminal ring elements each electrically coupled to at least one of the conductor wires. The terminal ring elements are separated from one another by an electrically insulating material. Each of the terminal ring elements includes an outer surface having a first portion and a second portion, the first portion being electrically conductive and the second portion being electrically non-conductive.

Various embodiments concern a suture sleeve for securing an implantable lead with a suture. The suture sleeve can comprise a tubular body having a proximal end portion, a distal end portion, an exterior surface, and a lumen, the lumen sized to receive the implantable lead. The suture sleeve can further comprise a suture receiving area along the tubular body, the suture receiving area within a channel that extends around a circumference of the tubular body. An exterior surface of the suture receiving area can comprise a tear resistant feature. The tear resistant feature can be configured to resist initiation of a tear in the exterior surface from the suture. The lumen can comprise an inner surface configured to frictionally engage the implantable lead.
