Aspects of the present disclosure are presented for a single surgical instrument configured to grasp, seal, and/or cut tissue through application of therapeutic energy, and also detect nerves through application of non-therapeutic electrical energy. A medical device may include two jaws at an end effector, used to apply therapeutic energy and to perform surgical procedures. The therapeutic energy may be in the form of ultrasonic vibrations or higher voltage electrosurgical energy. One of the jaws may be configured to cut tissue through application of the blade. In addition, one or both of the two jaws may be configured to apply nontherapeutic energy for nerve stimulation probing. The application of therapeutic energy may be disabled while the nontherapeutic nerve stimulation energy is applied, and vice versa. The nontherapeutic nerve stimulation energy may be applied to the use of one or more probes positioned near one or both of the jaws.

An apparatus for operating on tissue comprises a shaft, an acoustic waveguide, and an end effector. The acoustic waveguide extends along the shaft and is configured to transmit ultrasonic vibration. The end effector comprises an ultrasonic blade and a clamp arm. The ultrasonic blade is in acoustic communication with the acoustic waveguide. The clamp arm is pivotable toward the ultrasonic blade. The end effector defines a first longitudinal region and a second longitudinal region. The end effector is configured to clamp tissue between the clamp arm and the ultrasonic blade in the first longitudinal region. The end effector is configured to sever tissue with the ultrasonic blade in the second longitudinal region.

Aspects of the present disclosure are presented for a single surgical instrument configured for grasping tissue, performing sealing procedures using electrosurgical or ultrasonic energy, suctioning fluid, and providing irrigation. An end effector of the surgical instrument may include multiple members arranged in various configurations to collectively perform the aforementioned functions. The suction and irrigation elements may comprise one or more fluid paths configured to deliver fluid to or evacuate fluid from a surgical field. In this way, a user, such as a clinician or surgeon, may rely on a single surgical instrument to perform these tasks typical in surgery while having an extra hand available and so as to not need to divert his or her concentration away from the surgical site in order to access multiple devices. Further, the timing for performing each of the functions may be made quicker, due to not needing to switch using multiple devices.

A generator is disclosed to generate a drive signal to a surgical device. The generator includes an ultrasonic generator module to generate a first drive signal to drive an ultrasonic device, an electrosurgery/radio frequency (RF) generator module to generate a second drive signal to drive an electrosurgical device, and a foot switch coupled to each of the ultrasonic generator module and the electrosurgery/RF generator module. The foot switch is configured to operate in a first mode when the ultrasonic device is coupled to the ultrasonic generator module and the foot switch is configured to operate in a second mode when the electrosurgical device is coupled to the electrosurgery/RF generator module. The generator further includes a user interface to provide feedback in accordance with the operation of any one of the ultrasonic device and the electrosurgical device in accordance with a predetermined algorithm.

A surgical suturing apparatus includes a flexible member having a proximal end and a distal end. The proximal end has a handle attached thereto and the distal end has a suturing body attached thereto. The handle is operatively connected to the suturing body. The suturing body includes a suture housing in which a needle and drive assembly are housed for movement of the needle with a suture secured thereto about an arcuate path facilitating application of a suture to tissue. A mechanism is provided for accommodating and operating different sized needles and different sized sutures while maintaining the operative connection between the handle and the suturing body.

In accordance with the present invention there is provided a device for use with a needle with a suture attached to it. The device is used for suturing bodily tissue and comprises a handle, a right arm and a left arm extending distally from the handle. The distal end of the right arm has a right gripper attached to it for gripping and releasing the needle. The distal end of the left arm has a left gripper attached to it for gripping and releasing the needle. The device further comprises an electrically powered means for operating the left gripper and the right gripper. The left gripper and the right gripper exchange gripping of the needle. One of the left gripper and the right gripper moves near to the other to exchange gripping of the needle and moves apart from the other when not gripping the needle. The device further comprises a control unit for controlling the electrically powered means. In one embodiment, the electrically powered means comprises a drive assembly contained in the handle, and the drive assembly has an electrically powered motor.
