A lithotripsy system can include a lithotripsy device and controller circuitry. The device for acoustic calculi fracture can include an acoustic treatment probe for providing acoustic energy along the probe to treat one or more calculi and an evacuation pathway extending at least partially via the prob, for removing at least a portion of the one or more calculi via the evacuation pathway. The controller circuitry, coupled to the lithotripsy device, can be configured to receive an indication of clogging of the evacuation pathway and at least one of initiate or adjust a anti-clogging mode for changing at least one characteristic of evacuation via the evacuation pathway in response to the received indication of clogging.

The present disclosure includes a calculi fracture device having an acoustic transducer for transferring acoustic energy via a primary fragmentation probe to fracture a calculi mass into calculi fragments, an evacuation tube connecting the probe to a pressure source, and a secondary fragmentation device located in an evacuation pathway extending along the fragmentation probe and the evacuation tube to further break up the calculi fragments to inhibit clogging at a more proximal location in the evacuation pathway. A method of inhibiting clogging of a calculi fracture device can include receiving, from a primary fragmentation device, fragments of a calculi mass along a passage of a evacuation pathway of the calculi fracture device and further breaking up the calculi fragments along the passage of the evacuation pathway at a location that is more proximal to a primary fragmentation location.

A composition of a biological sample, such as tissue of a patient, can be estimated during an ablation procedure. A composition detector system can include a probe, an illumination source, and a spectrometer. A distal end of a probe can convey mechanical, acoustical, or ultrasonic energy to the tissue to ablate the tissue. The probe can extend through a working channel of a viewing scope. An illumination source can illuminate a portion of the tissue at the distal end of the probe or as the portion is being evacuated and collected for disposal. The illumination can generate response illumination from the portion of the tissue. The spectrometer can receive the response illumination, analyze the response illumination, and provide an estimate of the composition of the portion of tissue.

Real-time composition analysis techniques for ultrasonic ablation

A lithotripsy device can comprise a handpiece, a lithotripsy probe extending from the handpiece, an energization source configured to deliver an energy to a distal end of the lithotripsy probe, a suction passage extending from the distal end of the probe and through the handpiece, and a capture device comprising a container comprising a storage space, an entry port configured to couple to the handpiece at the suction passage, and an exit port, and a capture element connected to the container and configured to facilitate capture of stone fragments within the storage space. A method can comprise fragmenting stones with a lithotripsy device, drawing a vacuum through the lithotripsy device to pull stone fragments and waste fluid therethrough, pulling the vacuum through a stone capture device connected to the lithotripsy device, depositing stone fragments within the stone capture device using a capture element, and drawing the vacuum through the device.

A method of treating a calculi mass can include using an ultrasonic probe to produce acoustic energy and fragment the mass. The method can include varying the frequency at which fragmentation occurs to treat the mass with a resonant frequency. The ultrasonic probe can have a distal tip for contact with the mass, where the tip has a morphology for concentrating stress on the mass. The ultrasonic probe can have two or more ultrasonic horns to allow for higher voltage and power levels.

A multiple-modality ablation probe can include a tube configured to transmit mechanical modality energy from a first end to an obstruction in contact with a second end. A first optical transmission media can extend along the tube to transmit laser modality energy from a laser energy source to the obstruction. A connector assembly can be mechanically coupled with the first end of the tube and with the first end of the first optical transmission media. The connector assembly can be configured for user-attachment and detachment of the tube and the first optical transmission media with a mechanical energy source coupling and a laser energy source coupling, respectively.

A reverse retropulsion device can include a lithotripter, a collection passage, and an energy directing device. The lithotripter can be configured to deliver energy to tissue located at a tissue forming region. The collection passage can be positionable at or near the body lumen. The energy directing device can be positionable near the lithotripter and the collection passage. The energy directing device can be configured to propel the tissue toward the collection passage.

A system to deliver energy to treat a mobile calculus, the system can include a drill and a lateral energy emitter. The drill can be configured to drill a recess into the mobile calculus or a passage through the mobile calculus. The lateral energy emitter can be configured to be advanced into the recess or the passage and to transmit the energy internal to the mobile calculus to fragment the mobile calculus. In some examples, the system can include a deployable capture portion to constrain a stone relative to the capture portion.
