A data carrier provides health related information regarding a patient, and facilitates compliance with HIPAA by the obscuring the association between the name of the patient and the health related information when the data carrier is to be discarded. The data carrier includes a release liner, a health information label having an upper surface and a lower surface, the label including a first area on the upper surface for indicia specifying health related information and a second area on the upper surface for indicia specifying the identity of a patient, and a pressure sensitive adhesive coating on the lower surface of the health information label, securing the health information label to the release liner. A masking label is either integral with the health information label, or the liner. By various arrangements the masking label is either removed from the health information label, thereby disassociating the patient identity and the health related information, or the masking label is secured to the health information label so as to cover and obscure the name of the patient or the information, or both.

Data carrier for health related information

A form having a wristband and, optionally, detachable labels, includes a printable face ply and a liner ply. The printable face ply has an upper surface and a lower surface, and a pressure sensitive adhesive on at least a portion of the lower surface. The face ply may include first and second adjacent portions, with the first portion of the face ply die cut to form a detachable wristband having first and second ends and an upper and a lower surface, with pressure sensitive adhesive on the lower surface of the detachable wristband adjacent the first and second ends, and with the second portion of the face ply die cut to form the series of detachable labels. The first portion of the face ply is also die cut to form an extender band having first and second ends and an upper and a lower surface, with pressure sensitive adhesive on the lower surface of the extender band adjacent the first and second ends. One of the extender band and the wristband defines an opening through which one of the first and second ends of the other of the extender band and the wristband can extend. As a result, the wristband and the extender band can be secured together by adhesive at their respective overlapping ends to increase effectively the length of the wristband. Variations in construction include providing adhesive at fewer than all of the wristband and extender band ends, and only having a single portion face ply.

A patient wristband form has a transparent ply with an upper surface and a lower surface, and a pressure sensitive adhesive coating on the lower surface. A release ply has an upper surface and a lower surface, with a release coating on the upper surface. The transparent ply is removably mounted on the upper surface of the release ply by the pressure sensitive adhesive coating. A die cut in the transparent ply defines an elongated wristband. An opaque coating is provided on the upper surface of the transparent ply in a central portion of the elongated wristband. A die cut in the transparent ply defines an overlaminate label that is sized to cover at least a part of the central portion of the elongated wristband, thereby covering indicia printed on the opaque coating. A paper ply may also be mounted on the release ply by pressure sensitive adhesive. The paper ply defines a plurality of labels that may be printed at the same time as the wristband.

A two-way shipping label construction includes a strip of release material, a first printable label, and a second printable label. The first label is adhered to the strip of release material by the first pressure sensitive adhesive coating and includes at least one customer receipt label defined by a die cut having one or more ties. The first label has a first surface with a release coating on a portion thereof, and a second surface with a first pressure sensitive adhesive coating. The second label has a first surface and a second surface with a second pressure sensitive adhesive coating. The second printable label adheres to the strip of release material by the second pressure sensitive adhesive coating adjacent the first printable label. The second printable label includes at least one manifest label defined by a die cut having one or more ties.

A foldable information card that is useful for storing related documents is provided. Specifically, a foldable card that may contain warranty or product information to be given to a consumer is provided. The foldable card has a storage pocket for related documents such as sales receipts. Additionally, the foldable card has a repositionable adhesive that may be used to secure the card in a folded configuration and prevent documents from falling out of the storage pocket. Information may be printed on the foldable card in more than one language.

Two-way shipping label construction and method

A self-adhesive label is provided for displaying useful information about an object to which the label is affixed. The label is formed with a face layer constructed out of heavy paper, such as cardstock, and having a first planar surface which is suitable to receive printed indicia thereon. An adhesive layer coats a second planar surface of the face layer and includes a pocket-forming window therein, which is in communication with a slot in the face layer, and adhesively affixes the face layer to the object. The pocket-forming window defines an item receiving pocket between the object and the face layer, wherein the item-receiving pocket is accessible through the slot, for example, to removably insert a card therein. A sheet of labels is also provided having two or more labels thereon, wherein each of the two or more labels are removable from the sheet for individual use thereof.

A business form includes a carrier sheet having an upper surface and a lower surface, and defining a die cut opening there through, with a patch of liner material mounted on the lower surface of the carrier sheet. The patch extends over the opening. The liner material has an upper surface facing the die cut opening in the carrier sheet, and carries a release coating on the upper surface. A two ply label assembly is mounted on the patch of liner material in the die cut opening. The two ply label assembly includes a transparent ply mounted on the upper surface of the patch of liner material, and a label ply mounted on the transparent ply. The form is manufactured by: a.) die cutting an opening in a piece of carrier material; b.) adhesively affixing a patch of liner material to the piece of carrier material; c.) mounting a transparent ply on the patch of liner material within the die cut opening; and d.) mounting a label ply on the transparent ply. The form is used by: a.) removing the label ply from the transparent ply; b.) adhesively securing the label ply to the label support surface; c.) removing the transparent ply from the patch of liner material; and d.) adhesively securing the transparent ply to the label ply and to the label support surface, such that the transparent ply completely covers the label ply, thereby protecting the label ply while permitting the indicia to be viewed.

A shipment form is provided including a face sheet, a liner ply, a reusable adhesive laminate, and a laminating adhesive. The face sheet defines first and second major faces and includes printed indicia on the first major face. The printed indicia being arranged to indicate an attempted delivery. The reusable adhesive laminate includes a reusable adhesive and a removable ply. The reusable adhesive and the removable ply are configured such that the reusable adhesive retains its properties of adhesion following removal of the removable ply therefrom. The laminating adhesive is interposed between the liner ply and the face sheet so as to secure the liner ply to the second major face of the face sheet. The laminating adhesive is further interposed between the reusable adhesive laminate and the face sheet so as to secure the reusable adhesive laminate to the second major face of the face sheet.

A construction for a laminated window label is provided and includes a transparent film having an adhesive on one surface thereof, liner stock having first and second release-coated surfaces which is adhered on one surface to the transparent film, and an opaque layer which is adhered to the other surface of the liner stock. The transparent film and opaque layer preferably include die cut label portions. Variable and nonvariable information may be printed on the die cut label portion of the transparent film, and the opaque layer may be removed from the construction and replaced onto the surface of the film, thereby laminating the label portion of the opaque layer to the label portion of the transparent film. The two label portions may then be peeled away from the remainder of the construction as a single label which is then applied to a transparent surface such as a window. The opaque layer further includes a tamper-evident feature so that any attempt to remove the label produces a warning message on the transparent surface, the transparent film, and the opaque layer.
