A conversational interface system including an interactive virtual avatar and method for completing and updating fillable forms and database entries. The conversational interface provides a user with the option of inputting data in either text or voice form, and logs user response data and populates fields within form documents. As the user progresses through the system, instructions and guidance are consistently provided via the interactive avatar presented within the system web browser. Without exiting the system, the conversational interface validates user input data types and data while updating entries within cloud-based databases.

An application system capable of providing a continuation of care for a user. This is achieved through the assessing of medication compliance, assessing medical testing compliance and providing appropriate care suggestions. The system incorporates medication reminder, medication compliance analytics and the capacity to provide responses to user specific medication inquiries. The system also incorporates medical test assessment capabilities and the capacity to provide basic medical testing protocols. Additionally, as medication and medical testing data are collected, processed and analyzed, they are stored in a zero-click fashion with results being rapidly presented back to the user via an interactive avatar. These data can also be provided in standard of care summaries to users, HIPAA compliant third parties and/or medical care providers.

Systems and methods for monitoring movement capabilities using clinical mobility based assessments of a user are provided herein. In embodiments, methods include: providing, using a mobile device comprising an inertial measurement device, a clinical mobility based assessment to a user; and generating, using the inertial measurement device, inertial data of the user that is indicative of movement capabilities of the user based on the clinical mobility based assessment. Embodiments include logging the inertial data of the user locally to the mobile device resulting in locally logged inertial data of the user; processing in real-time the locally logged inertial data of the user to determine position and orientation of the mobile device during the clinical mobility based assessment; and determining, using the position and the orientation of the mobile device during the clinical mobility based assessment, a physical movement assessment of the user associated with the clinical mobility based assessment.
