Dual chamber pacemaker systems can lead to two forms of pacemaker-facilitated tachycardia&#x2014;pacemaker-mediated tachycardia (PMT) and tracking of sinus or atrial tachycardia. Current pacemaker algorithms can not always differentiate between these two tachycardias. Various embodiments for differentiating these particular mechanisms of pacemaker-facilitated tachycardia, which is based on the specific termination response to PVARP extension are provided. The response to PVARP extension (V-A-A-V vs V-A-V) is a specific method for differentiation and can be used in conjunction with observations of atrial rate and electrogram morphology (or surface P wave morphology) for distinguishing between the two mechanisms of pacemaker-facilitated tachycardia.

Method, device and program to differentiate pacemaker-mediated tachycardia (PMT) from tracking of sinus or atrial tachycardia (AT)

Nonlinear-dynamical control, also known as chaos control, can modulate human cardiac electrophysiological dynamics by stabilizing an unstable target rhythm. Intracardiac, repolarization alternans detection methods are disclosed that operates on the basis of amplitude or repolarization duration detection. Also disclosed are control methods for stabilizing repolarization alternans.

Intracardiac detection and control of repolarization alternans

An apparatus and method for the treatment of ventricular fibrillation, ventricular tachycardia, or supraventricular tachycardia. The method being to prospectively ascertain a patient's transthoracic resistance by application of a low amplitude current via the apparatus electrodes applied to a patient's chest, charging a capacitor of the apparatus sufficiently for delivery of a minimal peak current, e.g., 25 amperes for treatment of ventricular fibrillation preselected by the operator as appropriate for attaining defibrillation, and the capacitor is discharged through the electrodes to effect the defibrillation. The apparatus can be made by modifying conventional direct current defibrillator devices.

Current-based defibrillating method

Having prospectively ascertained a patient's transthoracic resistance upon application of a low amplitude exploration current via the defibrillator electrodes, the capacitor of the defibrillator is charged sufficiently for delivery of a minimal peak current, e.g., 25 amps, preselected by the operator as appropriate for attaining defibrillation, and the capacitor is discharged to effect the defibrillation.

A defibrillator which will automatically ascertain the transthoracic resistance of the patient and then automatically apply a defibrillation shock according to the transthoracic resistance and an amperes per ohm factor.
